    Methods : DNA was extracted from prospectively collected and biobanked esophageal biopsies from newly diagnosed, treatment naïve EoE patients enrolled in a randomized trial of two tCS formulations (budesonide slurry or fluticasone metered dose inhaler)...Conclusion s: We validated three epigenetic biomarkers (CpG methylation sites) for prediction of tCS response in EoE patients in an independent population. While not all previously identified markers replicated, three demonstrated a relatively high predictive probability for response to treatment and thus hold promise for guiding initial treatment in patients with EoE.

    Data from 13 studies demonstrate that CIC is associated with low rates of oropharyngeal AEs, with no indication of clinically relevant systemic effects in children with asthma. The favorable safety profile and demonstrated improvements in asthma control make CIC an ideal inhaled corticosteroid for the treatment of asthma in children.

    Conclusions Moderate doses of fluticasone spray affects urinary calcium excretion and this in turn may influence on bone mineral metabolism which in long term may increase the incidence of complications such as bone fractures. Therefore it is wise to consider this detrimental effect of GCSs especially using them for the long period and also in high doses.

    In a real-world clinical setting of COPD treatment, budesonide-based triple therapy given in two inhalers was generally as effective at reducing exacerbations as fluticasone-based triple therapy. However, the budesonide-based triple therapy was associated with a lower incidence of severe pneumonia and possibly also of all-cause death, especially among patients with no prior exacerbations for whom triple therapy is not recommended.

    Conditioned media from resting or FcεR1-activated human lung mast cells were incubated with serum-deprived ASM cells (1:4 dilution, 24 h) to determine their impact on the anti-inflammatory action of fluticasone on ASM cell chemokine expression induced by TNFα (10 ng/ml)...Our study demonstrates for the first time that conditioned media from FcεR1-activated mast cells blunt the anti-inflammatory action of corticosteroids in ASM cells by altering their transactivation properties. Because infiltration of mast cells within the ASM bundles is a defining feature of asthma, mast cell-derived mediators may contribute to the glucocorticoid insensitivity present in severe asthma.

Finally, our findings highlight the importance of establishing close collaboration both with pediatric allergy specialists to prescribe the most suitable treatment and with manufacturing companies to ensure that adhesives of technological devices are free of harmful well-known sensitizers.

    Although the proportional reduction of airway neutrophilia was significantly larger in the group receiving azithromycin, the lung function and the peripheral and central airway smooth muscle mass decreased similarly in both groups. Despite a better control of airway neutrophilia, azithromycin did not potentiate the other clinical effects of fluticasone.

    These data specify that SG has been demonstrated as a probable therapy for the treatment of allergic skin diseases in combination with FC by decreasing its dose from 50 to 25 µg/mice to avoid the chronic side effects of FC. Hence, it can be concluded that SG and SG + FC combination significantly improved the AD-like symptoms in the DNFB sensitized mice through mitigating the production of proinflammatory mediators and restoration of oxidative stress markers.

    Clustering was also applied to subgroups of ICS naive patients one the hand and high dose ICS treated patients (≥1000 µg propionate fluticasone) on the other hand...Clustering on ICC naïve and high dose ICS treated patients also yielded two clusters mainly structured by age, atopic status, intensity of granulocytic airway inflammation.ConclusionOur results points to different phenotypes in eosinophilic asthmatics with the atopic trait being generally associated with an achievable control of the disease with low dose ICS despite eosinophilic airway inflammation. In contrast there is an aggressive eosinophilic phenotype also usually featuring intense airway neutrophilic inflammation, not associated with atopic trait and leading to impaired lung function and poor asthma control despite heavy treatment burden.

    Ongoing research is needed to learn more about prevention and early recognition of these life-threatening phenomena.AI is due to deficient production or action of glucocorticoids with or without mineralocorticoid and adrenal androgen deficiency.8 Tertiary AI occurs after chronic exogenous steroid use, which leads to suppression of hypothalamic CRH secretion.8 It is diagnosed by low serum cortisol and ACTH, or by ACTH stimulation test.8IIH is diagnosed based on "elevated intracranial pressure with no clinical, laboratory, or radiographic evidence of responsible infection, vascular abnormality, space occupying lesion, or hydrocephalus"4. The pathogenesis of IIH is unclear, but it is associated with obesity, endocrine abnormalities, drugs (including steroid wean), viral infections, nutritional deficiencies, and genetic conditions in children.4

    A significant reduction was observed in the ovulation rate, follicular size (P < 0.001), ovarian hormones, immunoglobulins, body weight, and food consummation (P ≤ 0.05) by administering GC Fluticasone. Although a relative anti-inflammatory improvement was created by Fluticasone in the ovarian condition, the administration of this glucocorticoid resulted in a considerable reduction in ovarian hormones and functions of commercial aged laying hens.

    Thirty Wistar rats were randomized into five groups: (i) Normal control, (ii) Disease control (TNBS), (iii) Sarsasapogenin (SG) (50 µg/rat), (iv) Fluticasone (FC) (50 µg/rat), (v) Sarsasapogenin + Fluticasone (SG + FC) (25 µg/rat)...SG and SG + FC treatment also considerably restored the histopathological changes induced by TNBS. Thus, SG and SG + FC combination could alter the disease progression and could be a hopeful therapeutic target for the management of UC by reducing its dose in combination with FC to elude the long term adverse effects of FC.

    We report the case of a 15-year old boy with stable CFRD who developed acute HHS after treatment with glucocorticoids and itraconazole for presumed allergic broncho-pulmonary aspergillosis (ABPA). This case highlights the dangerous and preventable combination of high glucose intake, glucocorticoids and itraconazole inhibition of CYP3A4 (with resultant glucocorticoid accumulation) that can result in a state of life- threatening HHS in an adolescent with previously stable CFRD.
