    Journal:  Direct Urine Resistance Detection Using VITEK 2. (Pubmed Central) -  May 30, 2022   
    Cefepime, imipenem, and tobramycin had no major errors (ME)...The antibiotic with the most minor errors (mE) was ceftazidime...The direct AST from the urine samples proposed here was shorter by one day, without significant loss of sensibility regarding the standard diagnosis. Therefore, we hypothesize that this method is more realistic and better suited to human antibiotic concentrations.

    Conclusion Based on the data, amoxicillin/clavulanate and nitrofurantoin are appropriate first-line options for empiric treatment of symptomatic cystitis in this long-term care facility, with sulfamethoxazole/trimethoprim as an alternative. Ciprofloxacin was overprescribed despite its low susceptibilities to commonly encountered pathogens, which emphasizes the need for a UTI empiric treatment algorithm tailored towards residential care.

    The nitrofurantoin derivative 1 featuring propargyl moiety was distinctively the most active of all, was nontoxic to human cells and possessed submicromolar cellular activity selectively directed towards the pathogens of the life threatening visceral leishmaniasis. Hence it was identified as potential antileishmanial lead for further investigation into its prospective to act as alternative to therapies.

    The antibacterial activity of cold and soxhlet extract of pomegranate was studied and compared with (Cefuroxime, Ciprofloxacin, Nitrofurantoin, Amoxicillin, Ofloxacin, Ceftazidime, cefoxitin) against clinical isolates 30 Staphylococcusi aureus, 30 Escherichia coli from Yastabshiron Hospital from September 2021 to November 2021 and standard staphylococcus aureus ATCC(6538),Escherichia coli ATCC (25922). The antibacterial of pomegranate peels extracts was more effective against Staphylococcus aureus than Escherichia coli, the high concentration yielded far superior results than the low concentration.

    When Fosfomycin was combined with >= 0.003% ITC there was increased efficacy against S. marcescens resistant strains S1-S5 which was within EUCAST inhibition break point zone > 15mm considered as sensitive strains.The combination of ITC with antibiotics contributed to increased efficacy in Fosfomycin, Imipenem,Nitrofurantoin and Gentamicin against the uropathogens tested. More investigation is required to identify the microbial genetic changes and synergistic effects leading to such effects.

    High empiric use of oral antibacterials (ciprofloxacin, trimethoprim-sulfamethoxazole, amoxicillin-clavulanate, cephalosporins, nitrofurantoin) and intramuscular ceftriaxone can select for pathogens with altered targets, decreased transport or producing extended-spectrum beta-lactamases...In addition, we compared susceptibility to ciprofloxacin, nitrofurantoin, 3rd-generation cephalosporins including ceftriaxone, beta-lactam-beta-lactamase inhibitor combinations, carbapenems, aminoglycosides, tigecycline, polymyxin B and colistin by agar method or broth microdilution utilizing supplemented cation-adjusted Mueller-Hinton Broth...Remarkably, synergy between F and TS occurred regardless of resistance to F or TS, or any other antibacterial agent/class tested. The antibacterial combination of fosfomycin-trimethoprim-sulfamethoxazole may offer an important alternative for the treatment of UTIs and merits clinical evaluation.

    The data demonstrate the new test‘s potential for achieving excellent analytical and clinical performance directly from urine specimens, determining ID in 35 minutes and antimicrobial susceptibility in 4.5 hours. Robustness to the challenges of direct from specimen AST including variable inocula, presence of commensals, and matrix effects offer the possibility of rapid automated ID and AST for a broad range of syndromic infections.

    The most common organism was Escherichia coli (76%) and this demonstrated resistance to all commonly available antibiotics (amoxicillin 100%, cotrimoxazole 77.8%), and 41% were extended-spectrum beta-lactamase species. The most common antibiotics to which these UTI were sensitive nitrofurantoin and imipenem.

    Isolates with an ESBL-producing phenotype (13.2% and 8.6% for inpatient and ambulatory settings, respectively) and multidrug resistance (inpatient and ambulatory rates of 23.4% and 17.7% for ≥2 drugs; 9.9% and 6.4% for ≥3 drugs) were also prevalent. Statistically significant differences by geographic region (P ≤ 0.005) were observed for AMR classes in both inpatient and ambulatory settings, but rates remained above thresholds recommended for empiric UTI therapy across most regions.

    Molecular detection of enterohemorrhagic E. coli (EHEC) pathotype virulence genes, confirmed the presence of stx1 gene in O157:H7 (MA2), O26:H11, O145:H328 and O125:H6 serogroups; stx2 gene in (O157:H7 (MA1), O128: H2 and O25: H2; while all serogroups except (O125:H6) carried the eaeA intimin virulence gene. A phylogenetic tree, based on the nucleotide sequences of toxin-encoding genes, demonstrates that Shiga toxin E. coli (STEC) isolates have considerable genetic variation and belong to various phylogenetic groupings.

    Like trimethoprim/sulfamethoxazole, nitrofurantoin carries a warning for patients with known sensitivity to oxidant drugs, notably glucose-6-phosphate dehydrogenase (G6PD) deficiency, in whom it may cause haemolytic anaemia...Early studies of erythrocyte survival following nitrofurantoin suggest it is less likely to cause oxidant haemolysis in individuals with G6PD deficiency than primaquine...The evidence suggests a total daily dose of 200 mg nitrofurantoin may be used for short (3-5 day) course urinary tract infection treatment without G6PD screening when accompanied by appropriate advice. Pharmacovigilance in countries with high prevalence of G6PD-deficiency is recommended to monitor for serious adverse events.

However, nitrofurantoin and fosfomycin are the most effective drugs against uropathogens in UTIs.
