    PK/PD data, Journal:  Whole Body Physiologically (Pubmed Central) -  Nov 13, 2024
    Ex vivo experiments reported that the unbound voriconazole plasma concentration remained unchanged after adding flucloxacillin. The PBPK model describes the potential mechanism driving the drug-drug and drug-disease interaction of voriconazole and flucloxacillin, highlighting the large substantial influence of flucloxacillin on the PXR gene and the influence of infection on voriconazole plasma concentrations, and suggests a more limited effect on other triazoles.

    Infectious sacroiliitis: an uncommon septic arthritis () -  Nov 8, 2024 - Abstract #BSRCBC2024BSR_CBC_71;
The patient was treated with 21 days of intravenous flucloxacillin, resulting in negative blood cultures and improved inflammatory markers, leading to discharge.One week later, he was readmitted with severe gluteal pain and fever...Modified hydrocortisone may represent a superior option, as it more closely mimics the physiological circadian rhythm of glucocorticoids, normalises the immune cell profile, and reduces the incidence of recurrent infections.

    Recurrent knee swelling- all is not crystal clear () -  Nov 8, 2024 - Abstract #BSRCBC2024BSR_CBC_23;
He was treated for presumed CPPD with Colchicine and covered with intravenous (IV) Flucloxacillin whilst awaiting SF results, which demonstrated numerous pus cells, calcium pyrophosphate crystals and a negative Gram-stain.After two days, the left wrist improved, but his right knee swelling reaccumulated...The clinical suspicion of septic arthritis remained high, and a 16S-PCR was requested.His throat swab subsequently returned positive for Neisseria gonorrhoea, and he was given IV Ceftriaxone, and his CRP improved to 49 mg/l...This will ultimately result in reduced symptom burden and length of inpatient stay.

    Journal:  Flucloxacillin instantly decreases serum levels of valproic acid: A case report. (Pubmed Central) -  Sep 10, 2024
    Because this case showed that free-fraction serum levels remained within therapeutic range, it underscores the need for free-fraction VPA monitoring in bipolar disorder and flucloxacillin therapy. When VPA is used for epilepsy, it is advised to consider alternative antibiotics to avoid this interaction.

    Review, Journal, Surgery:  Review of the national usage of antibiotics in arthroplasty surgery: a need for evidence-based prescribing. (Pubmed Central) -  Sep 1, 2024
    There is considerable variation in the local antibiotic recommendations for surgical site prophylaxis post-primary arthroplasty surgery, with respect to both recommended first-line antibiotic and dosing regimens. With increasing emphasis on the importance of antibiotic stewardship and the emergence of antibiotic resistance, this study highlights the need for an evidence-based approach to prophylactic dosing across the UK.

    Review, Journal:  Beta-lactam-associated hypokalemia. (Pubmed Central) -  Aug 20, 2024
    The PubMed, Web of Science, Cochrane Library, and Scopus databases were searched for articles published between 1965 and 2023, using the following terms: 'hypokalemia' OR 'potassium loss' OR 'potassium deficiency' AND 'beta-lactams' OR 'penicillin' OR 'penicillin G' OR 'cephalosporins' OR 'ceftazidime' OR 'ceftriaxone' OR 'flucloxacillin' OR 'carbapenems' OR 'meropenem' OR 'imipenem' OR 'cefiderocol' OR 'azlocillin' OR 'ticarcillin'...The incidence of ?-lactam antibiotic-associated hypokalemia may be as high as 40%, thus, the issue of ?-lactam-associated hypokalemia remains clinically relevant. Although other causes of hypokalemia are likely to be diagnosed more frequently (e.g., due to diuretic therapy or diarrhea), the possibility of ?-lactam-induced renal potassium loss should always be considered in individuals with so-called 'unexplained hypokalemia'.

    Thoracalgia and facial malignant staphylococcal infection: is there a link? (Poster Area) -  Aug 5, 2024 - Abstract #EADV2024EADV_3355;
    In cases of facial cellulitis (especially if bacteremia is also present) it is important to rule out any embolic complications, which more frequently occur in intracranial locations, such as the ophthalmic or facial veins. Diabetes mellitus, manipulation of lesions and poor hygiene are major risk factors.

    Surface contamination with the antibiotics was widespread and most probably caused by spillage during the preparation in combination with an ineffective cleaning procedure. Strategies should be developed and implemented by institutions for safe handling of antibiotics to reduce environmental contamination and potential exposure of healthcare workers to these sensitizing drugs.

    Journal:  An exception-reporting approach for wound swab culture: effect on post-report antibiotic initiation. (Pubmed Central) -  May 24, 2024
    In this analysis, we have shown that an alternative approach to reporting positive wound swab cultures has resulted in a large reduction in post-report antibiotic initiation, suggesting that the current standard method of reporting generates considerable unnecessary antibiotic use. If these findings are replicated elsewhere, wider adoption of this reporting would represent an opportunity for many clinical microbiology laboratories to have a significant impact on community antimicrobial stewardship.

  • ||||||||||  Review, Journal:  Human Leucocyte Antigen Genetics in Idiosyncratic Drug-Induced Liver Injury with Evidence Based on the Roussel Uclaf Causality Assessment Method. (Pubmed Central) -  Apr 26, 2024
    Among the top-ranking drugs were amoxicillin-clavulanate (290 cases, HLA A*02:01 or HLA A*30:02), followed by flucloxacillin (255 cases, HLA B*57:01), trimethoprim-sulfamethoxazole (86 cases, HLA B*14:01 or HLA B*14:02), methimazole (40 cases, HLA C*03:02), carbamazepine (29 cases, HLA A*31:01), and nitrofurantoin (26 cases, HLA A*33:01). In conclusion, the HLA genetics in 900 idiosyncratic drug-induced liver injury cases with evidence based on the RUCAM are available for studying the mechanistic steps leading to the injury, including metabolic factors through cytochrome P450 isoforms and processes that activate the innate immune system to the adaptive immune system.

On the basis of these findings and considering the need for fast, simple and reproducible sample pretreatment for TDM purposes, we conclude that ultrafiltration of flucloxacillin should be performed at physiological temperature (37

  • ||||||||||  Preclinical, Journal:  The Controversial Effect of Antibiotics on Methicillin-Sensitive S. aureus: A Comparative In Vitro Study. (Pubmed Central) -  Nov 29, 2023
    Ongoing discussions have focused on the potential synergistic effects of combining these agents with rifampicin or fosfomycin to combat infections associated with biofilm formation...Given these findings, it is reasonable to report that ?-lactams combined with rifampicin represent the optimal treatment for MSSA bacteremia. However, caution is warranted when employing this treatment over an extended period, as it may elevate the risk of selecting for small colony variants (SCVs) and, consequently, promoting bacterial persistence.

    Journal:  Single-cell patterning and characterisation of antibiotic persistent bacteria using bio-sCAPA. (Pubmed Central) -  Nov 27, 2023
    Using bio-sCAPA, we analysed the phenotypic characteristics of single Staphylococcus aureus cells tolerant to flucloxacillin and rifampicin killing. We find that antibiotic-tolerant S. aureus cells do not display significant heterogeneity in growth rate and are instead characterised by prolonged lag-time phenotypes alone.
