    A Case of Retropharyngeal Abscesses Caused by Cyclic Vomiting Syndrome Due to Chronic Marijuana Use (San Diego Convention Center, Room 31A-C (Upper Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_8100;
    He was diagnosed with epiglottitis with left retropharyngeal abscess that was successfully drained surgically, and was discharged on Amoxicillin-Clavulanic acid...Prevention and good oral hygiene are important to prevent such complications. However, once abscess are present, early and aggressive source control is essential to prevent airway compromise and death.

    Treatment of Acute Respiratory Failure Secondary to Chronic Graft-Versus-Host Disease With Fluticasone, Azithromycin, Montelukast, and High-Dose Glucocorticoids (San Diego Convention Center, Area D (Hall A-B2, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_6242;
    He was recently treated as an outpatient with levofloxacin, amoxicillin/clavulanic acid, and albuterol/ipratropium nebulizers for presumed pulmonary infections...Four months after discharge, he completed his prednisone taper and was asymptomatic from a respiratory standpoint, requiring no supplemental oxygen...Recent data has suggested that FAM and steroids can improve mortality in these patients. This case highlights FAM and an extended steroid taper, extrapolated from non-GVHD bronchiolitis obliterans and organizing pneumonia studies, as a possible therapeutic plan for providers to consider in chronic lung GVHD patients after HSCT.
  • ||||||||||  Systemic Lupus Erythematosus Presenting as Acute Lupus Pneumonitis (San Diego Convention Center, Area H (Hall H, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_5657;
    The patient received 5 days of IV antibiotics and was subsequently discharged on amoxicillin-clavulanate...Antibiotics were broadened to piperacillin-tazobactam and she underwent video-assisted thoracoscopic surgical decortication (VATSD) of the right pleural space...The patient responded well to pulse corticosteroid therapy, followed by high-dose corticosteroids and hydroxychloroquine achieving complete remission of respiratory symptoms...Its nonspecific symptoms often result in delayed diagnosis. Given the rarity of ALP and frequent misdiagnosis, maintaining a high clinical suspicion is vital, particularly when initial diagnoses and treatments for other conditions, such as infections, prove ineffective.

    Unraveling the Cavitary Mystery: Is It Infection, Neoplasm, or Something Else? (San Diego Convention Center, Area J (Hall H, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_4524;
    She was subsequently discharged on a four-week course of amoxicillin-clavulanate...Comprehensive diagnostics in this case negated infection as the leukocytosis cause, pointing to paraneoplastic syndrome. This underscores the challenge of differentiating between infections and neoplasms, especially when symptoms overlap, emphasizing thorough and timely evaluations for effective care.

    A Case of Asymptomatic Mediastinal Lymphadenitis Caused by Streptococcus Pneumoniae (San Diego Convention Center, Area H (Hall H, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_4091;
    The patient was eventually referred to the infectious diseases department and was started on treatment with amoxicillin-clavulanate...The increasing prevalence of such cases may be partly attributed to widespread antibiotic use. Patients in such cases should also be thoroughly evaluated for underlying immunodeficiency as is currently being done for our patient.
  • ||||||||||  Endemic Dangers: A Rare Case of Dual Infection With Legionella and Blastomycosis Requiring Veno-venous Extracorporeal Membrane Oxygenation (San Diego Convention Center, Area H (Hall H, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_4086;
    Case:A 51-year-old immunocompetent female with BMI 40, mild intermittent asthma, and relapsing-remitting multiple sclerosis (not requiring treatment) presented with worsening pneumonia despite outpatient treatment with cefdinir, azithromycin, amoxicillin-clavulanic acid, and methylprednisolone...She was treated with levofloxacin and amphotericin B and was able to successfully decannulate on HD 8...1% but can remain positive as long as 300 days. In summary, this case of coinfection with Blastomycosis and Legionella poses interesting challenges in pulmonary infection diagnostics and serves as a reminder about the risks of endemic pneumonias.

    Obstructive Cholangiopathy in an Infant With Cystic Fibrosis: Biliary Atresia, Cystic Fibrosis, or Both? (San Diego Convention Center, Area G (Hall A-B2, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_4044;
    CFTR modulator therapy (lumacaftor-ivacaftor) was not initiated due to persistent elevated transaminases. Early identification and management of biliary atresia in children with CF is important for preventing cirrhotic liver disease that increases risk of poor pulmonary outcomes and ineligibility for CFTR modulator therapy.

    Pneumomediastinum: A Rare Complication of Dental Extraction (San Diego Convention Center, Area L (Hall H, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_3622;
    Maxillofacial surgery was consulted, no surgical intervention was recommended at the time and it was decided to transition him to oral antibiotics (amoxicillin-clavulanic acid)...Further imaging with CT chest and neck is imperative to confirm the diagnosis and evaluate for the presence of pneumomediastinum. The presence of subcutaneous emphysema is relatively benign as these are usually managed with reassurance but the presence of pneumomediastinum warrants more aggressive treatment with prophylactic antibiotics as mediastinal involvement allows the development of more serious complications such as infective mediastinitis.

    Fungal Duo: A Case of Disseminated Histoplasmosis and Pneumocystis Pneumonia Competing for the Spotlight (San Diego Convention Center, Area E (Hall A-B2, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_3512;
    He was discharged on steroids and amoxicillin-clavulanate for a suspected exacerbation of chronic sinusitis...Ultimately requiring intubation, bronchoscopy was performed which yielded a positive culture for Pneumocystis jirovecii for which he was treated with trimethoprim-sulfamethoxazole...Appropriate induction with amphotericin and transition to itraconazole ultimately led to patient improvement...In a case involving a critically ill immunosuppressed patient, it may be necessary to treat both infections as significant contributors rather than innocent bystanders. In conclusion, opportunistic fungal infections can have rapid progression leading to critical illness in patients with rheumatoid arthritis on multiple immune modulating agents.

    A Rare Case of Necrotizing Community-Acquired Pneumonia Complicated by a Broncho-Splenic Fistula (San Diego Convention Center, Area E (Hall A-B2, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_3497;
    We applied similar diagnostic and therapeutic principles in treating this broncho-splenic fistula to that of treating a BPF, including drain placement and low positive end-expiratory pressure/tidal volume ventilatory strategies. A broncho-splenic fistula may be considered in the differential diagnosis for patients with prior abdominal/diaphragmatic trauma who present with hypoxemia.

    An Unusual Case of Arcanobacterium Haemolyticum Causing Acute Respiratory Distress Syndrome Requiring Extracorporeal Membrane Oxygenation (San Diego Convention Center, Area E (Hall A-B2, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_3493;
    She presented to the Emergency Department two weeks prior to her current visit with a flu-like illness, nonproductive cough, sore throat, and mild dyspnea for which she was treated with amoxicillin/clavulanate potassium without improvement of symptoms...Additionally, it can easily pass unrecognized in bacterial cultures due to slow growth, coryneform appearance on gram stain, and weak hemolytic activity. Identifying the underlying pathogen early during the diagnosis process is crucial for effective treatment and can result in significant improvement for patients.

    Not Your Flavor - A Case of Neisseria Flavescens Mimicking Malignancy (San Diego Convention Center, Area I (Hall H, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_1997;
    She was treated empirically with a 4-week course of Amoxicillin-clavulanate...The benefit of serial imaging, with either CT or PET-CT, and close follow up with medical teams is imperative in this population of patients. This patient had no prior history of chemotherapy or radiation but her immunocompromised state and history of RCC was a predisposition for infection.
  • ||||||||||  Pulmonary Nocardiosis in an Asthmatic Patient on Inhaled Corticosteroid (ICS) Therapy: Is ICS Use a Risk Factor? (San Diego Convention Center, Area I (Hall H, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_1987;
    amikacin, carbapenems, third-generation cephalosporins, levofloxacin, minocycline, and amoxicillin-clavulanic acid are second-line and other effective agents used for the treatment of Pulmonary Nocardiosis...Clinical suspicion holds significant importance, as it necessitates specific microbiological tests. The management of nocardiosis often entails prolonged and occasionally combined therapies.

    Divergent Findings, Convergent Treatments: Two Cases of Pulmonary Actinomyces (San Diego Convention Center, Area I (Hall H, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_1983;
    His course was complicated by hemoptysis requiring bronchial artery embolization and he was discharged with a long course of amoxicillin-clavulanate...While necessary to rule out malignancy and other infections, diagnostic yield is low. Ultimately, clinical reasoning is essential to organize these non-specific threads into a diagnosis as treatment courses for pulmonary actinomyces is prolonged.
  • ||||||||||  Mediastinal Lymph Node Abscess Due to Strep Viridans Masquerading as a Malignancy (San Diego Convention Center, Area I (Hall H, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_1977;
    The patient responded well to intravenous ceftriaxone, later transitioned to Amoxicillin-Clavulanate to complete a total 6 week course. Unusual presentation of infections should always be on the differential for any patients presenting with mediastinal and hilar lymphadenopathy, even in patients without traditional risk factors such as immunocompromise, diabetes mellitus, malignancy or heavy alcohol use.
  • ||||||||||  A Rare Case of Necrotizing Pneumonia Caused by Prevotella Infection in an HIV-Positive Patient (San Diego Convention Center, Area I (Hall H, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_1976;
    Our case emphasizes the importance of recognizing this organism as a potential pathogen, particularly in immunocompromised patients with poor oral hygiene. Antibiotic resistance due to beta-lactamase production has been associated with treatment failures; therefore, obtaining culture sensitivity results is crucial for guiding antibiotic selection.

    An Indolent Case of Chronic Cavitary Pulmonary Aspergillosis (San Diego Convention Center, Area I (Hall H, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_1945;
    Description of case report: A 51-year-old with a history of Tetralogy of Fallot (TOF), severe pulmonary hypertension, end stage renal disease, and endocarditis (on life-long suppressive cephalexin) presented to the hospital with acute hypoxemic respiratory failure requiring admission to the medical intensive care unit (MICU)...At that time, the patient was not started on treatment for invasive aspergillosis as it was thought to be unlikely given the lack of neutropenia or overt immunosuppression, and instead was treated for pneumonia with empiric amoxicillin-clavulanate...Serum aspergillus antibody was positive, and the patient was treated with Isavuconazole for CCPA coverage...Unlike patients with invasive pulmonary aspergillosis who are generally overtly immunosuppressed, patients with CCPA often have more subtle forms of immunosuppression and/or underlying structural lung disease. Ultimately, increased awareness of diverse risk profiles is crucial for timely intervention and improved outcomes in patients with this uncommon form of pulmonary aspergillosis.

    Diffuse Interstitial Lung Disease With Organizing Pneumonia in 12-year-Old Female With TNFRSF13B Monoallelic Common Variable Immunodeficiency (San Diego Convention Center, Area J (Hall H, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_1883;
    She received amoxicillin/clavulanic acid with trimethoprim/sulfamethoxazole added after PCR was positive for Pneumocystis jiroveci...Evidence has supported use of chronic steroids and rituximab, but other immunosuppressants as well as newer agents like nintedanib have not been extensively studied...In this case, the patient's autoimmune manifestations far preceded her rapidly progressive lung disease which itself preceded her diagnosis of CVID. She has unfortunately shown little response to established treatments, necessitating significant polypharmacy as well as transplant consideration.

    An Unusual Case of Pyo-Pneumothorax Caused by Streptoccocus Constellatus in a Pediatric Patient (San Diego Convention Center, Area J (Hall H, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_1830;
    His hospital course was also complicated by Candida auris central line infection, found to be resistant to Fluconazole, for which he required to complete a 28-day regimen with IV micafungin. During follow up a week after discharge, the patient was found to remain clinically well with complete resolution of respiratory symptoms.

    Cat Got Your Lungs? An Uncommon Cause of Chronic Cough in an Immunosuppressed Child (San Diego Convention Center, Area J (Hall H, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_1826;
    At 4 years of age, he was unfortunately also diagnosed with very early onset bowel disease with secondary hemophagocytic lymphohistiocytosis and started on infliximab...He was treated with a prolonged course of amoxicillin-clavulanic acid and started on routine airways clearance with improvement of cough and tolerance of colds...Given our patient's immunosuppressed status and chronic respiratory symptoms, it is likely that nasopharyngeal colonization led to his pulmonary disease. This case raises the important point of considering a broader range of "less common" pathogens in children with immunosuppression and evaluating exposures.
  • ||||||||||  A Rare Presentation of Hydropneumothorax in a Previously Healthy Adolescent (San Diego Convention Center, Area J (Hall H, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_1822;
    There are no established guidelines for management of hydropneumothorax and this case demonstrated a complicated course not completely responsive to chest tube evacuation or straightforward VATS. Surgical intervention, specifically decortication of the restrictive rind, was necessary to see clinical improvement.

    Though it is important to identify the inciting drug discontinuation and supportive treatment started immediately, it was difficult to diagnosis that this case has diverse symptom such as gallbladder, Intestinal tract and leg with AEP. Clinicians should consider this diagnosis when managing with AEP.
  • ||||||||||  Metastatic Papillary Thyroid Carcinoma Discovered by Paramalignant Effusion (San Diego Convention Center, Area C (Hall A-B2, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_1521;
    Of the 51 reported cases of papillary thyroid carcinoma leading to a pleural effusion, none have featured ovarian metastasis. Conclusion This patient exhibits a rare case of papillary thyroid carcinoma leading to an exudative pleural effusion and extends the clinical spectrum of metastatic potential of papillary thyroid cancer to the ovaries and fallopian tubes.
  • ||||||||||  Parapneumonic Effusion Arising Due to Long Standing Biliary Disease (San Diego Convention Center, Area C (Hall A-B2, Ground Level)) -  Feb 20, 2024 - Abstract #ATS2024ATS_1491;
    Case reportA 75-year-old male had a history of choledocholithiasis status post ERCP with stone removal 6 days prior, during which time he was also treated with Ampicillin- Sulbactam for a right-sided pneumonia...He had been taking Amoxicillin-clavulanic acid since discharge for pneumonia...The patient was initially treated with piperacillin-tazobactam and vancomycin, later tailored to ceftriaxone...Hence, it is important to expeditiously treat cholecystitis to avoid such complications. It is also important to differentiate the cause of pleural effusion in the setting of cholangitis by testing for bilirubin level to rule out the rare biliary-pleural fistula.

    Journal:  Antibiotic consumption in French nursing homes between 2018 and 2022: A multicenter survey. (Pubmed Central) -  Feb 19, 2024
    The decreasing trend in antibiotic use and control of fluoroquinolone use over the study period suggest compliance with antibiotic use guidelines. However, changes in the use of broad-spectrum antibiotics and the substantial use of amoxicillin-clavulanic acid, although it is rarely a first-line antibiotic, highlight the need for antimicrobial stewardship activities and the usefulness of antibiotic consumption surveillance to identify priorities.
  • ||||||||||  Journal:  Susceptibility and Synergistic Effects of Guava Plant Extract and Antimicrobial Drugs on Escherichia coli. (Pubmed Central) -  Feb 16, 2024
    This finding indicates that guava extract enhances the effectiveness of commonly used antibiotics for treating UTI, an effect mainly attributed to the flavonoid compounds and their derivatives in the guava leaf extract, which inhibit bacterial growth. This study demonstrated the antibacterial properties of guava, suggesting that combining antibiotics with guava extract can help delay the emergence of bacterial resistance.
