    A higher proportion of MDR E. coli were noted in urine samples from males compared with females, possibly due to the increased likelihood of prior treatment for UTIs in men. Antimicrobial stewardship interventions could be targeted towards this cohort to address increasing rates of AMR.

    The management was medical including intravenous antibiotic therapy based on amoxicillin-clavulanic acid and exploration by nasofibroscopy completed by hypopharyngoscopy showed a fistula of the piriform sinus. The treatment is based on surgery by cauterization of the sinus.

This report highlights this rare and unusual association as a possibility in our setting; and also brought up the difficulties with diagnostic procedures in resource constrained settings like ours, especially in light of widespread antimicrobial resistance and rational antibiotics use.

    Furthermore, in terms of pathogenicity, three isolates (8.8%) were classified as high threats and two (5.9%) as moderate threats. The degree of resistance, production of virulence factors and the percentage of isolates classified as moderate or high threat means that a constant vigilance of such strains in veterinary units, but also in clinics and hospitals in general, is an important tool in terms of infection prevention and consequent reduction of HAIs.

    The prevalence of those bacteria in the environment, besides other associations, is related to the lack of basic sanitation and the traffic between humans and the river. Observing heightened resistance to Amoxicillin-clavulanic acid and intravenous Cefuroxime corresponds with the prevalence of the bla TEM gene in the samples, since this gene is related to resistance to cephalosporins and penicillin-like antibiotics.

Pakistan's high postpartum sepsis rates necessitate treatment guidelines based on common pathogen susceptibility, preventive strategies are imperative to improve the outcomes of postpartum women and reduce maternal mortality in Pakistan.
