Journal:  Early clinical manifestations and laboratory findings before and after treatment of cytomegalovirus infection in kidney transplant patients. (Pubmed Central) -  Jul 4, 2019
...The induction immunosuppression in living related kidney transplants was with antithymocyte globulin or basiliximab, whereas the most common maintenance immunosuppressive regimen was with cyclosporine, mycophenolate mofetil, and prednisolone...All patients with CMV infection were treated with intravenous ganciclovir, 2.5-5 mg/kg q 12 hourly, according to creatinine clearance, for 21 days...Our study shows that CMV infection should be considered in a patient presenting with unexplained rise in serum creatinine, low-grade fever, diarrhea, or anemia. A significant improvement in kidney and liver functions was observed after successful treatment of the infection.

Clinical, Journal:  Childhood idiopathic steroid-resistant nephrotic syndrome at a Single Center in Khartoum. (Pubmed Central) -  Jul 4, 2019
...ISRNS was defined as no remission within four weeks of daily prednisolone at a dose of 60 mg/m2...Therapy included cyclosporine in 38.5%, additional therapy with cyclophosphamide, mycophenolate mofetil or tacrolimus in 20.8%, and steroids ± diuretics ± angiotensin converting enzyme (ACE) inhibitors in 40.7%...This pattern in addition to the lack of adequate therapy may explain the relatively lower CR and higher CKD rates. Impaired renal function and hypertension at presentation were risk factors for progression to CKD.

Journal:  Human herpesvirus-6 pneumonitis following autologous peripheral blood stem cell transplantation (Pubmed Central) -  Jul 4, 2019
...He achieved a complete remission after pirarubicin, cyclophosphamide, vincristine, and prednisolone (THP-COP) therapy and successful autologous peripheral blood stem-cell transplantation (AutoSCT)...The treatment with foscarnet was effective, and no relapse was noticed in the patient...Moreover, most of the above patients were clinically diagnosed using serum or plasma markers. Therefore, examining respiratory symptoms after AutoSCT would enable a more accurate diagnosis as well as treatment of patients with HHV-6 pneumonitis.

Clinical, Journal:  MonoMAC syndrome patient developing myelodysplastic syndrome following persistent EBV infection (Pubmed Central) -  Jul 4, 2019
An 18-year-old man was diagnosed with Epstein-Barr virus (EBV) -associated hemophagocytic syndrome (HPS) and treated with prednisolone (PSL) at a previous hospital...GATA2 mutation p.R230Hfs44 was identified in both bone marrow and control cells (buccal swab) at MDS diagnosis, and he was diagnosed with monocytopenia and mycobacterial infection (MonoMAC) syndrome. Furthermore, an acquired STAG2 mutation (splicing site change, c.820-2A>G) in the bone marrow cells was also identified, which might contribute to MDS progression.

Journal:  Refractory primary immune thrombocytopenia in pregnancy requiring splenectomy and repeated intravenous immunoglobulin therapy (Pubmed Central) -  Jul 4, 2019
...There was no improvement in the platelet count after treatment with 0.5-1.0 mg/kg/day prednisolone, and the 400 mg/day intravenous immunoglobulin (IVIg) administered for 5 days gradually became ineffective; therefore, a laparoscopic splenectomy was performed at 25 weeks of gestation...The patient's platelet count also increased after delivery. Splenectomy and repeated IVIg therapy can be considered for refractory severe ITP during pregnancy.

Journal:  Controversies in Clinical Trials for Alcoholic Hepatitis. (Pubmed Central) -  Jul 1, 2019
Placebo controlled trials without steroids may be necessary and ethically justified in certain populations of AH who have not been adequately investigated. Ultimately, we suggest the field will advance with the development of a plausible animal model of true AH, a consensus on a composite clinical end-point that does not rely solely on mortality for use in future RCTs, and the adoption of the recommendations of the NIAAA Alcoholic Hepatitis Consortia regarding standard definitions and when to require a liver biopsy prior to study entry.
  • ||||||||||  Journal:  Sonic Hedgehog Agonist Protects Against Complex Neonatal Cerebellar Injury. (Pubmed Central) -  Jun 28, 2019
...This was compounded by administration of prednisolone as shown by greater volume loss and Purkinje cell death...Together, these findings indicate that HIF primes both cell types for injury via glucocorticoids, and that hypoxia/HIF + postnatal glucocorticoid administration act on distinct cellular pathways to cause cerebellar injury. They further suggest that SAG is neuroprotective in the setting of complex neonatal cerebellar injury.

Journal:  Recurrence of malignant lymphoma immediately after treatment for hepatitis C virus using direct-acting antivirals (Pubmed Central) -  Jun 28, 2019
Seven years later, he received the direct-acting antivirals (DAAs) sofosbuvir/ledipasvir for hepatitis C virus (HCV) genotype 1b...However, the relationship between DAAs and hepatocellular carcinoma recurrence remains unclear. Nonetheless, possible cancer recurrence should be considered in patients with a history of lymphoma who are prescribed DAAs to treat HCV.

Clinical, PK/PD data, Review, Journal:  Characterizing the pharmacokinetics and pharmacodynamics of immunosuppressant medicines and patient outcomes in elderly renal transplant patients. (Pubmed Central) -  Jun 27, 2019
This review examines what is currently known about the pharmacokinetics and pharmacodynamics of commonly prescribed immunosuppressant medicines, tacrolimus, cyclosporine, mycophenolate and prednisolone, in elderly renal transplant recipients, and reported patient outcomes in this cohort...Elderly transplant recipients do not appear to have higher dosage-adjusted exposure to mycophenolic acid (MPA)...This suggests that an optimal balance between immunosuppressant medicine efficacy and toxicity is not achieved in elderly recipients, and further studies are needed to foster long-term graft and patient survival. Lower maintenance immunosuppressant targets in elderly recipients may decrease patient susceptibility to drug side effects, however, further studies are required and appropriate targets need to be established.

Biomarker, Clinical, Journal:  Downregulation of miRNA-29, -23 and -21 in urine of Duchenne muscular dystrophy patients. (Pubmed Central) -  Jun 26, 2019
Lower maintenance immunosuppressant targets in elderly recipients may decrease patient susceptibility to drug side effects, however, further studies are required and appropriate targets need to be established. miR-29c-3p, miR-23b-3p and miR-21-5p are promising novel noninvasive biomarkers for DMD, and miR-29c-3p levels are differentially affected by different steroid regimens, supporting the antifibrotic effect of steroid therapy.

Clinical, Journal:  Nivolumab-induced severe pancytopenia in a patient with lung adenocarcinoma. (Pubmed Central) -  Jun 26, 2019
Therefore, further chemotherapy for lung adenocarcinoma could not be initiated, and the patient died due to progression of lung cancer 118 days after the onset of pancytopenia. The possibility of severe pancytopenia as an immune-related adverse event should be considered as a mandatory prerequisite for nivolumab therapy.

Clinical, Journal:  Necrosis of the small intestine leading to a diagnosis of polyarteritis nodosa: a case report. (Pubmed Central) -  Jun 26, 2019
We suggest that successful pregnancy outcomes could occur with close monitoring and daily dialysis in female kidney transplant patients with resistant rejection. In cases of idiopathic intestinal necrosis or perforation, systemic diseases such as polyarteritis nodosa should be considered in the differential diagnosis.

Clinical, Journal:  Differences in clinical Pneumocystis pneumonia in rheumatoid arthritis and other connective tissue diseases suggesting a rheumatoid-specific interstitial lung injury spectrum. (Pubmed Central) -  Jun 22, 2019
...Patients who underwent methylprednisolone "pulse" therapy or sufficient antibiotics to cure bacterial pneumonia were excluded...The PC-PCR-negative patients, observed only in the RA group, were all receiving methotrexate (MTX) therapy except one patient who was receiving high-dose prednisolone alone...No patient with MTX alone had positive PC-PCR results. Clinical PCP in RA patients differed from that in non-RA CTD patients and may be understood as only a part of the rheumatoid-specific interstitial lung injury spectrum influenced by multiple, synergistic factors including MTX, Pneumocystis, and RA itself.

Clinical, Journal, Combination therapy:  Comparison of the Effect of Disease: Modifying Antirheumatic Drugs Alone or in Combination with Biologic Drugs in the Outcome of Patients with Rheumatoid Arthritis. (Pubmed Central) -  Jun 21, 2019
...Patients were divided into two groups of 55: Groups A and B. For the treatment of Group A, prednisolone along with one or two drugs from the DMARDs combinations was used...DAS-28 activity index had a significant difference between both two groups at the beginning of the study (P = 0.05), but at the end of the study, the difference was not statistically significant (P = 0.1). Simultaneous use of DMARDs and biologic drugs in RA patients could lead to improvement the disease symptoms and decrease the severity and activity of the disease.

Review, Journal:  Important problems in the diagnosis and treatment of autoimmune hepatitis (based on the Russian consensus 2017). (Pubmed Central) -  Jun 17, 2019
The analysis of publications devoted to the Russian Consensus on the Diagnostic and Treatment of Autoimmune Hepatitis (AIH), which was considered at the 43rd annual Scientific Session of the CNIIG From Traditions to Innovation (March 4, 2017) is carried out. The presence of clear algorithms and recommendations for the diagnosis and treatment of AIH significantly help the doctor in real clinical practice, but do not exclude a personified approach to the patient.
