Cough induced haemothorax. () - Sep 10, 2019 - Abstract #EUSEM2019EUSEM_1344; Past medical history was significant for osteoporosis, hypertension and asthma; and the patient had been started on 20mg of Prednisolone for 3 months prior to this presentation...Patient Consented. Educationa/Clinical Relevance/Learning Points: Fits of high intensity coughing can predispose patients to rib fractures The more rib fractures present, the more likely patients are to develop a haemothorax Osteoperosis should be identified and treated early Have a low threshold for suspecting fractures in patients on long term steroids Never exclude the potential of a non-traumatic haemothorax until ruled out by CT CT Images Informative.

Angioedema in a rare genetic disorder of Capillary leak syndrome (Clarkson’s disease). () - Sep 10, 2019 - Abstract #EUSEM2019EUSEM_1283; The patient was discharged 1 day after this admission with recommendation for Prednisolone and Immunoglobuline treatment as per Immunology team advise...The above patient required 2 years for appropriate diagnosis and treatment. We have to consider this pathology as differential diagnosis, even it is a rare condition, in patients presenting with angioedema, joints swelling and abdominal distension, not responding to classical allergy/anaphylaxis treatment.

Journal: Progressive renal insufficiency in a 55-year-old man with psoriasis (Pubmed Central) - Sep 8, 2019 However, little is known about renal complications and possible causality of renal injury due to this treatment. The following case of a patient with psoriasis demonstrates the difficulties in classifying renal complications of anti-TNF-α therapy versus kidney involvement caused by the underlying disease.

Journal: Hunter syndrome with persistent thrombocytopenia. (Pubmed Central) - Sep 6, 2019 ...This was followed by oral prednisolone...Severe thrombocytopenia is an extremely rare complication of acute or chronic EBV infection. In Asians, EBV infection should be considered in children presenting with thrombocytopenia, bleeding diathesis and anaemia along with organomegaly.

Retrospective data, Review, Journal: Use of steroids for abdominal tuberculosis: a systematic review and meta-analysis. (Pubmed Central) - Sep 1, 2019 The data on use of steroids for abdominal tuberculosis are limited to peritoneal tuberculosis. Although steroids seem to have some benefit in patients of tubercular peritonitis, the poor quality of studies limits the generalisability of the findings.

Clinical, Journal: Long-Term Safety and Efficacy of Tacrolimus in Myasthenia Gravis. (Pubmed Central) - Aug 30, 2019 Tacrolimus may be effective in reducing the severity of MG and may permit a reduction in the steroid dose prescribed to the patients. Adverse events due to tacrolimus treatment were not serious.

Clinical, Journal: Steroid response in primary childhood nephrotic syndrome in a tropical african environment. (Pubmed Central) - Aug 30, 2019 ...The recruited patients received a 4-6 weeks' course of prednisolone at 60 mg/m/day followed by alternate day doses of 40 mg/m up to total steroid therapy duration of 6 months in steroid sensitive patients...This study has shown a better steroid sensitivity of 63.3% in children with primary NS compared with the previously reported 36.8-42.9% in patients with highly selective proteinuria. This improved steroid response and zero mortality show a remarkable departure from the past.

Clinical, Journal: Tacrolimus for ulcerative colitis in children: a multicenter survey in Japan. (Pubmed Central) - Aug 29, 2019 The adverse event rate was 53.7%; 6 patients required discontinuation of tacrolimus therapy. Tacrolimus was a safe and effective second-line induction therapy for steroid-dependent and steroid-refractory ulcerative colitis in Japanese children.

IFNβ is NOT effective in MOG-IgG+ disease: a study in 26 MOG-IgG+ Japanese patients (Hall D) - Aug 28, 2019 - Abstract #ECTRIMS2019ECTRIMS_1381; IFNβ-1a (Avonex) was used in 12, and IFNβ-1b (Betaferon) in 12...Concomitant use of other immunosuppressive drugs such as prednisolone or azathioprine were seen in 13... IFNβ therapy is NOT recommended in MOG-IgG+ disease.
- |||||||||| Late Breaking Abstract - Matrix metalloproteinases in serum and sputum reflect distinct processes of relevance to asthma (N103) - Aug 26, 2019 - Abstract #ERS2019ERS_5968;
MMPs (1, 2, 3, 7, 8, 9, 10, 12, 13, 14, 16, 17) were measured by proteomic array (SomaScan®, SomaLogic), MMP-3 by Luminex® (R&D Systems) and urinary prednisolone by mass spectrometry...Conclusion As local airway MMPs emerge as biomarkers of novel asthma phenotypes we find that serum and sputum MMPs differ regarding associations with disease severity and steroid use. Serum MMP-3 stands out as being strongly upregulated by steroid use, questioning its usefulness and highlighting a need to determine therapeutic effects on new biomarkers.

A Combined Rheumatology/ILD Service. The Wolverhampton Experience (TP-42) - Aug 26, 2019 - Abstract #ERS2019ERS_4985; Conclusions, Combined Rheumatology/ILD service reduces number of outpatient clinic appointments for patients with CTD-ILD. Moreover, it alters clinical management in a significant proportion of patients, improving diagnostic accuracy and clinical therapeutics.

COPD self-management exacerbation action plans: predictors of adherence (TP-19) - Aug 26, 2019 - Abstract #ERS2019ERS_4517; Only patients with ≥1 exacerbation or ≥1 performed self-treatment action (starting a course of prednisolone) during 1 year follow-up were included...Our study results show that dyspnoea score and cardiac comorbidity are predictors of patient adherence to exacerbation action plans. Associations between adherence categories and health outcomes of COPD exacerbation action plans need to be explored to identify effective self-managers, define the clinical consequences of non-adherence, and further tailor support to improve adherence.

Adherence to steroid treatment in asthma (TP-14) - Aug 26, 2019 - Abstract #ERS2019ERS_4409; Methods We evaluated adherence to oral corticosteroids (OCS) in 64 patients with severe asthma (GINA step 5) by measuring prednisolone and cortisol levels in the blood using mass spectroscopy...Patients were more likely to be adherent to OCS than ICS. Many patients on OCS are considered for novel biological therapy and therefore more time needs to be spent exploring reasons for poor adherence and engaging with patients to improve adherence prior to escalation to expensive therapies.

Is electronic diary alert a false alarm in clinical studies? (TP-04) - Aug 26, 2019 - Abstract #ERS2019ERS_4218; Aim To capture COPD exacerbations that required prednisolone or antibiotic from patient's home...Remaining were coryzal symptoms (7), increased chest symptoms with no treatment (12) & rhinitis (14) Conclusion E diary is not a diagnostic tool and a prorportion of patients favour reporting exacerbation events to care team. Doctor is responsible to investigate each alert and initiate treatment early to prevent hospitalisation.
