    Clinical, Journal:  Longterm Data on Sirolimus Treatment in Patients with Lupus Nephritis. (Pubmed Central) -  Nov 13, 2019
    Given the pathophysiologic mechanisms of vascular complications after dermal filler injection, HBO2 should be considered when treating these complications. The preliminary evidence suggests that sirolimus may serve as an alternative treatment for patients with LN who do not tolerate standard treatment or who had a history of malignancy, and it has an acceptable longterm safety profile.

    Clinical, Journal:  Spontaneous remission of acquired factor XIII inhibitor concurrent to development of IgA-λ type multiple myeloma (Pubmed Central) -  Nov 12, 2019
    Although prednisolone was administered, factor XIII activity was not recovered...Immunoglobulin G inhibitors of factor XIII were present at admission, but later decreased with the onset of the IgA-λ-type myeloma. Thus, it is possible that the level of factor XIII inhibitors and polyclonal immunoglobulins could have been suppressed by the progression of myeloma, resulting in the normalization of factor XIII activity.

    Clinical, Journal:  Rhabdomyolysis as an initial presentation of systemic lupus erythematosus: a case report. (Pubmed Central) -  Nov 11, 2019
    Thus, it is possible that the level of factor XIII inhibitors and polyclonal immunoglobulins could have been suppressed by the progression of myeloma, resulting in the normalization of factor XIII activity. We report with a view to recommend autoimmune screening in young patients for whom a cause of rhabdomyolysis is not clearly identified.

    Clinical, Review, Journal:  Clinical management of autoimmune hepatitis. (Pubmed Central) -  Nov 9, 2019
    The cornerstone of treatment is steroid induction therapy followed by maintenance therapy with azathioprine, which is effective in most cases...Specialist consultation should be sought in difficult-to-treat patients. Future studies and networking initiatives should result in optimization of current treatment strategies in autoimmune hepatitis.

    A CASE OF EOSINOPHILIC CYSTITIS SUCCESSFULLY TREATED WITH MEPOLIZUMAB (Monitor 4) -  Nov 9, 2019 - Abstract #ACAAI2019ACAAI_730;
    The year prior to her visit she was on prednisolone 5-10 mg daily for 8 months...As she refused cyclosporine or azathioprine as potential steroid-sparing agents the decision to begin a trial mepolizumab was made...She has had infrequent exacerbations of her eosinophilic cystitis which now respond well to antihistamines and NSAIDs. She reports no adverse reactions to the biologic agent.This is the first report describing mepolizumab as a novel therapeutic agent for eosinophilic cystitis.

    ORAL BUDESONIDE DESENSITIZATION: A CASE REPORT (Monitor 1) -  Nov 9, 2019 - Abstract #ACAAI2019ACAAI_664;
    A limitation of this case was that the diagnosis of budesonide allergy was made through clinical history, we were unable to confirm the diagnosis by skin testing. The patient has been seen in our clinic for follow-up and has been tolerating oral budesonide well without any reoccurrence of angioedema.

    BRENTUXIMAB DESENSITIZATION AFTER SEVERE ANAPHYLAXIS IN A PATIENT WITH REFRACTORY HODGKIN'S LYMPHOMA (Monitor 1) -  Nov 9, 2019 - Abstract #ACAAI2019ACAAI_652;
    She was premedicated with IV chlorpheniramine(10mg), IV ranitidine(50mg), methylprednisolone(60mg) and ondansetron(8mg) before the infusion.A total of 180 mg of Brentuximab was given as four infusions with increasing rate and concentration(Table1)...Discussion This is the first report of successful desensitization to this drug in severe anaphylaxis. This particular patient had multiply relapsed disease; therefore, being able to continue to have brentuximab vedotin as a potential therapeutic option was important.

    EXCIPIENTS ASIDE, ANAPHYLAXIS TO CORTICOSTEROIDS IS A REAL THING (Monitor 1) -  Nov 9, 2019 - Abstract #ACAAI2019ACAAI_626;
    Discussion Reactions to two CS, positive CS ST and negative PEG ST suggest that this patient’s reaction was due to CS. Although infrequent, hypersensitivity to CS should be considered when there is no evidence of sensitization to excipients in the CS.

    METHIMAZOLE INDUCED V.S. IDIOPATHIC LUPUS IN A PEDIATRIC PATIENT (Monitor 1) -  Nov 9, 2019 - Abstract #ACAAI2019ACAAI_617;
    The other positive serologies and her history of autoimmune thyroid disease increases her risk of developing additional autoimmune conditions, such as idiopathic SLE. A high index of suspicion should be maintained for such clinical presentation with prompt discontinuation of the suspected agent.

    Journal:  H/C chemical shifts and cation binding dataset of the corticosteroid Prednisolone titrated with metal cations. (Pubmed Central) -  Nov 8, 2019
    In this analysis, the H and C chemical shifts were measured for the Prd/cation mixtures using a Bruker AV 500 MHz spectrometer (Bruker BioSpin GmbH, Rheinstetten, Germany), equipped with a 5 mm z-gradient Prodigy BBO 500 MHz probehead at 298 K, and simulation of the H spectra were determined from the Daisy software package (Bruker BioSpin GmbH). Binding affinity and free energy values were deduced from the C NMR peak intensities involved in the cation interaction, for more insight on the steroid/cation interactions please see Magnesium and Calcium Reveal Different Chelating Effects in a Steroid Compound: A Model Study of Prednisolone Using NMR Spectroscopy [1].

    Clonal Dynamics and Publicness of CMV-Specific TCR Repertoire after Allogeneic Stem Cell Transplantation (Hall B, Level 2 (Orange County Convention Center)) -  Nov 7, 2019 - Abstract #ASH2019ASH_6304;
    Median duration from SCT to first CMV reactivation was 39 days (range: 16-55) and 7 patients (43.8%) were administered systemic corticosteroid at the time of reactivation (prednisolone 10-30mg/day)...[Conclusion] TCR repertoire of CMV-CTLs at the time of CMV reactivation after SCT is highly oligoclonal and frequently shared among different patients, but can dynamically change in a short period in some patients. Functional analyses of the dominant TCRs to understand their reactivity against CMV epitope and elucidation of the clinical significance and developmental mechanisms of clone shift are strongly warranted.

    Interim Serum Soluble Interleukin-2 Receptor Levels Are Strongly Associated with Prognosis in Newly Diagnosis DLBCL Patients (Hall B, Level 2 (Orange County Convention Center)) -  Nov 7, 2019 - Abstract #ASH2019ASH_5855;
    IL2R levels should be measured during chemotherapy, and I-IL2R-positive patients could be targeted with high-dose or novel therapies. As this study was based on a retrospective analysis and involved a small cohort and a limited follow-up period, further studies are needed to confirm the prognostic impact of I-IL2R.

    High Rates of Venous and Arterial Thrombotic Events in POEMS Patients: Results from the UK-Based POEMS Registry, Highlighting the Need for Therapeutic Guidelines (W414AB, Level 4 (Orange County Convention Center)) -  Nov 7, 2019 - Abstract #ASH2019ASH_4993;
    Lenalidomide and dexamethasone (LD) was the most common prescription 30/51 (59%), followed by cyclophosphamide and dexamethasone in 10/51 (20%), LD plus cyclophosphamide in 3/51 (6%), melphalan and steroids 3/51 (6%), cyclophosphamide, dexamethasone and thalidomide 2/51 (4%), LD plus ixazomib 1/51 (2%), velcade, thalidomide and dexamethasone 1/51 (2%) and carfilzomib with dexamethasone 1/51 (2%)...Other TP agents used include: aspirin (6/30), treatment dose LMWH (tLMWH) (2/30), rivaroxaban (2/30), warfarin (2/30) and clopidogrel (1/30)...Treatment comprised LMWH (4), warfarin (4), or the direct oral anticoagulants, rivaroxaban (1) and apixaban (1)...Three occurred on anti-POEMS therapy: one patient (VEGF 4555) developed a second stroke on LD and LMWH prophylaxis; one patient on LD (VEGF 10640) and prophylactic rivaroxaban 10mg developed PVD, and another on melphalan and prednisolone (VEGF 2000-4000) and warfarin for atrial fibrillation (target INR 2.0-3.0) developed PVD...Thromboprophylaxis is commonly used in POEMS patients receiving outpatient-based chemotherapy with the most common agent being prophylactic LMWH. With the high incidence of arterial events in the presence of active disease, the role for anti-platelet agents or indeed DOACs remains undefined in this patient group.

    Patterns of Care and Clinical Outcomes in Peripheral T-Cell Lymphoma Patients Receiving First-Line Treatment in Routine Clinical Practice in France and the United Kingdom (Hall B, Level 2 (Orange County Convention Center)) -  Nov 7, 2019 - Abstract #ASH2019ASH_4693;
    In the 1L setting, 59.6% and 26.6% patients were treated with CHOP (cyclophosphamide, doxorubicin, vincristine, prednisolone) and with CHOEP/CHOEP‐like regimens (Table)...Brentuximab vedotin (19%) and GemOx (19%) were the most common 2L treatments...CHOP-based regimens were used to treat 59.6% of the study patients. Understanding distribution of PTCL subtypes and outcomes of treatment in routine clinical practice is complementary to data derived from clinical trials and crucial to facilitate improvement of survival outcomes through the introduction of novel therapies for this challenging group of rare malignancies.

    Antibody-Associated Autoimmune Disease (AAD) in Children with Cancer in Immune Recovery Phase Following Cessation of Chemotherapy (Hall B, Level 2 (Orange County Convention Center)) -  Nov 7, 2019 - Abstract #ASH2019ASH_1856;
    Prednisolone was administered in 3 patients (AIHA and ITP) to treat AAD...It was reported that IRS was also observed in HIV-negative patients who were administered alemtuzumab, and in those who received stem cell transplantation...We also speculate that AAD mimic autoimmune cytopenia such as ITP, AIHA, and autoimmune neutropenia in infancy because the diseases are caused by autoantibodies, occurring in immune-naive age, and they naturally recover in most cases although some patients need immunosuppressive treatment. Further studies are needed to clarify the pathogenesis of AAD in children.

    Retrospective and Prospective Study of Childhood Autoimmune Hemolytic Anemia. a Preliminary Report from the Red Cell Working Group of the Paediatric Hemato-Oncology Italian Associations (AIEOP) (Hall B, Level 2 (Orange County Convention Center)) -  Nov 7, 2019 - Abstract #ASH2019ASH_1745;
    A high number of children used additional treatment: red blood cell transfusions (51.4%), high dose Prednisolone (59.7%), high dose i.v. Immunoglobulin (49.7%) and Plasma Exchange (1.4%)...IgG/IgG+C3d positivity was a negative prognostic factor, as compared to positivity to C3d only, with the need of a second line treatment (prevalently Mabthera or Mycophenolate Mofetil) in 31.7% vs 0, respectively (p 0.009)...The management of the patients diagnosed after November 2013 was mostly in agreement with our recommendations, whose comprehensive therapeutic algorithm is reported in table 2, with prolonged steroid tapering in order to extend the treatment for at least 6 months. The most important difference between the retrospective and prospective study was the duration of first line treatment: 6 months or more, for steroid dependence, in 71.6% of patients in the prospective study versus 52.3% of the retrospective (p 0.031) and, more importantly, the percentage of relapsed patients: 8.3% in the prospective study versus 29.8% of the retrospective (p 0.001), these data need a longer follow up (median follow up: 24 months in the prospective study versus 63 in the retrospective)

    Clinical, P3 data, Journal:  Safety of IBI-10090 for inflammation associated with cataract surgery: Phase 3 multicenter study. (Pubmed Central) -  Nov 4, 2019
    Changes of systemic bone turnover induced by switching BPs to DMAb or TPTD may affect not only systemic bone mass, but also local joint destruction, and its clinical relevance should be considered comprehensively. The safety and efficacy of IBI-10090 and prednisolone 1.0% were similar, with IBI-10090 preferred over drops.
