The Bullous Pemhigoid Steroids and Tetracyclines (BLISTER) trial, published in 2017, showed doxycycline 200 mg daily to be noninferior to prednisolone 0·5 mg kg–1 daily as the initial treatment of bullous pemphigoid (BP) in terms of efficacy at 6 weeks...Of these, 13 (72%) required additional oral prednisolone...Furthermore, doxycycline did not avoid relapses in patients in remission on combined prednisolone and doxycycline, nor did it avoid the need to increase prednisolone doses in some patients. While further studies are required to establish fully the place of doxycycline in the management of BP, real-world data from our study indicate that few patients could be managed on doxycycline without prednisolone, at least in the initial phases of treatment. However, doxycycline may be a useful adjunct, particularly in maintaining remission.

In our case the skin findings were the first presentation of a rare condition.

Vomiting, gastrointestinal tract cramps, ATAQ and relapse rate showed a non-statistically significant difference. Single-dose dexamethasone was at least as effective as 5-day course of prednisolone in controlling asthma, while dexamethasone for 2 days was non-inferior to 5 days of prednisolone in children with asthma exacerbation.

Detailed questioning revealed significant exposure to incense smoke, highlighting a rare presentation of non-cigarette smoke-related PLCH. Avoidance of incense smoke combined with oral prednisolone (0.5mg/kg) tapered over 6 months led to complete resolution of symptoms, disappearance of nodules in high resolution computed tomography (HRCT) of the thorax and improvement in lung function.

Routinely collected electronic health records can be used to inform the feasibility of clinical trials in rare diseases, such as whether recruitment is feasible nationally and how long recruitment might take to meet recruitment targets. Future trials of bullous pemphigoid in England may use the data presented to inform trial design, including eligibility criteria and consent processes for enrolling people with dementia.

Fluorescein angiography should be used periodically to determine recurrence of the tumor or inflammation, and the appropriate treatment should be repeated as necessary. Moreover, regular systemic screening tests for von Hippel-Lindau disease should be performed in RCH patients to ensure that they have no abnormalities other than in the eye.
