    Journal:  Glucocorticoid dysfunction in children with severe malaria. (Pubmed Central) -  Jul 30, 2023
    Interestingly, hydrocortisone induced increased levels of cAMP in AC, but not in clinical malaria patients. Altogether, this study shows that patients with SM have increased cortisol levels, but also a decreased sensitivity to GCs, which may clearly contribute to the severity of disease.

    Myocarditis in Ulcerative Colitis - Complication of Disease or Therapy? (Exhibit Hall) -  Jul 29, 2023 - Abstract #ACG2023ACG_2738;
    Myocarditis can also develop as an extra-intestinal manifestation of UC flare-up. Irrespective of the cause, albeit mesalamine toxicity or UC flare-up, early recognition of myocarditis as an etiology of hemodynamic instability in UC patients and treatment with a combination of increased steroid dose and stopping mesalamine can prevent the cascade of shock and respiratory failure.

    Does Pre-emptive Dexamethasone Provide Prophylaxis Against Sugammadex-induced Bradycardia? A Retrospective Study (Hall D-9) -  Jul 20, 2023 - Abstract #ASA2023ASA_78;
    There was no significant change in heart rate slowing between the dexamethasone and non-dexamethasone group, potentially negating the theory of a transient early binding of sugammadex to corticosteroids with later more avid binding to rocuronium and unbinding the corticosteroids, or, it may be that other corticosteroids, such as hydrocortisone are required to prevent this effect. Prospective trials evaluating this effect with cortisol concentration measurement are needed to further evaluate the hypothesis.

    Journal:  The Hypothalamus-Pituitary-Adrenocortical response to critical illness: a concept in need of revision. (Pubmed Central) -  Jul 6, 2023
    The new findings supersede earlier concepts such as "relative", as opposed to "absolute", adrenal insufficiency and generalized systemic glucocorticoid resistance in the critically ill. They also question the scientific basis for broad implementation of stress dose hydrocortisone treatment of patients suffering from acute septic shock solely based on assumption of cortisol insufficiency.

    Preclinical, Journal:  Anti-fatigue effect of Lubian on kidney Yin deficiency and kidney Yang deficiency mice and mechanism based on PI3K-Akt pathway (Pubmed Central) -  Jul 3, 2023
    The kidney Yin deficiency model and kidney Yang deficiency model were prepared by daily regular oral administration of dexamethasone acetate and hydrocortisone, respectively, and meanwhile, corresponding drugs were provided...Compared with the kidney Yin deficiency model group, the kidney Yin deficiency-Lubian treatment groups showed increased body weight(P<0.01), relieved symptoms of Yin deficiency, increased content of cGMP(P<0.01), decreased cAMP/cGMP(P<0.01), prolonged exhausted swimming time(P<0.01), decreased LD(P<0.01), decreased BUN content(P<0.01), increased liver glycogen content(P<0.01), and increased protein expression of PI3K(P<0.05) and Akt in the liver(P<0.05). To sum up, Lubian can regulate Yin deficiency and Yang deficiency and increase glycogen synthesis by affecting the PI3K-Akt pathway, thereby exerting an anti-fatigue role.

  • ||||||||||  A Rare Case of a Recurrent Epidermal Necrolysis in a Filipino Pediatric Patient () -  Jul 3, 2023 - Abstract #WCD2023WCD_6304;
    OBSERVATION: A 13-year-old Filipino male was admitted at a tertiary hospital due to progressive development of tender erythema with flaccid blisters and erosions on the mouth, trunk, extremities and anogenital areas (BSA >8% to 38%); with associated dysphagia and dysuria <24 hours after ampicillin administration...Past history revealed a diagnosis of Stevens-Johnson Syndrome with Amoxicillin three (3) years prior...A 4mm punch biopsy confirmed Toxic epidermal necrolysis and was managed with hydrocortisone, ranitidine, diphenhydramine, saline compress, emollients and active guidance from other specialties for a comprehensive multidisciplinary approach...For physicians, a keen sense of identifying an emerging epidermal necrolysis and abrupt withdrawal of causal drug is key. A multidisciplinary approach is still best for managing dermatological emergencies with utilization of scoring systems: algorithm of drug causality for Epidermal Necrolysis (ALDEN) for drug causation and Severity-of-Illness Score for Toxic Epidermal Necrolysis (SCORTEN) modified for pediatric population for better prognosis.

    As the lesion worsened, she remained hospitalized in another center, where she was subsequently medicated with hydrocortisone, still showing no improvement...KEY MESSAGE: It is important to consider Fuchs syndrome (FS) in absence of typical cutaneous signs of erythema multiforme or Steven Johnson syndrome with only oral, ocular or genital lesions. Also we would like to emphasize that microbiologic diagnostic methods, including M. pneumoniae serology and PCR (most common cause of FS) must be done along with the notification of adverse effects to medications, specially if it is secondary to a being tested vaccine

    Novel intralesional rituximab regimen in recalcitrant oral pemphigus vulgaris () -  Jul 3, 2023 - Abstract #WCD2023WCD_3884;
    They were started on intralesional rituximab in the ulcers after premedication with hydrocortisone, pheniramine, and paracetamol at 25mg/week (5mg/cm2) for four weeks...The intralesional dose ensures an adequate drug concentration reaching the lesion, which could have led to improvement in our patients, and systemic absorption might have led to a reduction in the circulating antibodies and CD19 cells. Large-scale studies would be required to establish this effective, safer, and relatively cheaper treatment modality in the management guidelines
