    Journal:  Focus on Fentanyl in Females: Sex and Gender Differences in the Physiological and Behavioral Effects of Fentanyl. (Pubmed Central) -  Nov 27, 2023
    Fentanyl has distinct pharmacodynamics and enhanced efficacy relative to other opioids that highlights the need to investigate how females may be uniquely altered by its use. Behavioral and physiological responses to fentanyl are found to differ by sex and gender in many cases, including outputs like affective symptoms, analgesia, tolerance, and withdrawal emphasizing the need for further research about the role of biological sex on fentanyl use.

    Journal:  Instrumentation development, improvement, simplification, and miniaturization: The multifunctional plate source for use in mass spectrometry. (Pubmed Central) -  Nov 24, 2023
    The new source concept can be expanded to include multiple ionization methods using the same plate source "front end" without the need to vent the mass spectrometer between the different methods, thus allowing ionization of more compounds on the same mass spectrometer for which any one ionization method may be insufficient. Examples such as fentanyl, gamma-hydroxybutyric acid, clozapine, 1-propionyllysergic acid, hydrocodone angiotensin I and II, myoglobin, and carbonic anhydrase are included.

    Journal, Discordant:  Trends and correlates of discordant reporting of drug use among nightclub/festival attendees, 2019-2022. (Pubmed Central) -  Nov 21, 2023
    We compared trends in self-reported drug use, drug positivity, and "corrected" prevalence, adjusting for unreported use, and delineated correlates of testing positive for ketamine and cocaine after not reporting use (discordant reporting)...Underreporting of the use of these drugs was common, but we also detected underreported exposure to ethylone, fentanyl, 3,4-methylenedioxyamfetamine, metamfetamine, and synthetic cannabinoids...Underreporting of drug use was common, suggesting the need for researchers to better deduce intentional underreporting versus unknown drug exposure via adulterants. Researchers should consider both self-report and toxicology results from biological samples when examining trends in use.

    Journal:  Low-dose buprenorphine initiation in the era of fentanyl and fentanyl analogs: A case series of outpatient inductions. (Pubmed Central) -  Nov 20, 2023
    The growing literature demonstrates promise for alternative induction model of low-dose initiation of buprenorphine for the treatment of OUD, specifically targeting patients averse to withdrawal or using fentanyl. In this case series, we present four clinical cases of outpatient inductions, in which three out of four successfully transitioned from fentanyl to buprenorphine, and one patient transitioned from methadone to buprenorphine using a low-dose induction method.

    Journal:  Time to Reconsider Analgesia in Mass Casualty Incidents. (Pubmed Central) -  Nov 20, 2023
    Poor pain management is commonplace in trauma victims, and inadequate acute pain management can hinder evacuation efforts and may lead to the development of chronic pain and posttraumatic stress disorder. New, safe, and simple methods for administering quality analgesia have proven to be safe and effective in the prehospital setting and, as such, could easily be implemented into mass casualty incident protocols and allow for analgesia at earlier stages in such incidents, thereby improving patient care.
  • ||||||||||  Concerns for Serotonergic Toxicity in a Pediatric Patient During Inpatient Rehabilitation: A Case Report () -  Nov 16, 2023 - Abstract #AAPMR2023AAPMR_984;
    Review of the patient's medications identified several serotonergic agents that have been associated with serotonin syndrome in the literature: Fentanyl, Methadone, Olanzapine, Quetiapine, and Ondansetron.Setting: Tertiary care pediatric hospital.Assessment/ Given the patient's constellation of signs meeting Hunter criteria and otherwise inconclusive workup suggestive of an alternative etiology for his presentation, there remained a high suspicion for serotonin syndrome. Maintaining a broad differential and thorough review of medications, especially in the context of prolonged hospitalization, can be critical in identifying potentially life-threatening conditions such as serotonin syndrome.
  • ||||||||||  An Exotic Presentation: Managing Pain in a Patient with a Rare Axonal Variant of Guillain Barre Syndrome () -  Nov 16, 2023 - Abstract #AAPMR2023AAPMR_901;
    Current literature champions the use of gabapentin and carbamazepine as opposed to opioids in the management of GBS-related pain refractory to NSAIDs or Tylenol. When managing severe and intractable pain in patients with GBS, especially in patients with sensory nerve fiber involvement, utilizing a multimodal pharmacologic approach may be more efficacious than a single agent alone.

    Journal:  Drug supply measures and drug overdose mortality in the era of fentanyl and stimulants. (Pubmed Central) -  Nov 15, 2023
    For drug overdose death rates in the overall population and nearly all subpopulations examined by sex, race/ethnicity, and age, the model including all drug report proportional measures represented the best-performing model (as identified via the lowest Akaike Information Criterion and highest within R-squared value), followed by the model including only the fentanyl/fentanyl-related compounds proportion. Findings support the utility of publicly available drug report composition measures, particularly the proportion of fentanyl/fentanyl-related compounds, as predictors of drug overdose mortality in the US and in various subpopulations.

    Journal:  The Need to Rethink Harm Reduction for People Using Drugs Alone to Reduce Overdose Fatalities. (Pubmed Central) -  Nov 15, 2023
    Using drugs at home (aOR = 1.61[1.19-2.20]) was associated with higher odds of using alone; and being married (aOR = 0.57[0.38-0.86]), having history of illicit drug use (aOR = 0.25[0.08-0.81]) and other person present, who was not using (aOR = 0.58[0.42-0.79]) were associated with lower odds of using alone. New harm reduction approaches targeting people using drugs alone are needed to reduce overdose deaths.

    Retrospective data, Journal:  Safety and Efficacy of Rapid Methadone Titration for Opioid Use Disorder in an Inpatient Setting: A Retrospective Cohort Study. (Pubmed Central) -  Nov 7, 2023
    In this inpatient cohort, rapid methadone titration was well tolerated and resulted in patients reaching higher doses of methadone than would be possible with a standard schedule, with few adverse events. Given the known effective dose range, this approach may result in shorter time to clinical stabilization and suggests that alternative methadone titration schedules may be safe and effective in appropriately selected patients.

    Journal:  Factors Associated With Indirect Exposure to and Knowledge of Fentanyl Among Youth. (Pubmed Central) -  Nov 5, 2023
    Compared to diclofenac, buprenorphine appears to be more effective and equally safe for pain management in AP patients, even in the sub-cohort of moderately severe or severe pancreatitis (Trial Registration number: CTRI/2020/07/026914). While youth may have close proximity to fentanyl exposure and a degree of understanding of fentanyl, there is a general lack of knowledge of the substance, a critical gap that future substance use prevention initiatives could fill.

All methods were found to be effective in relieving the pain of the patients.
  • ||||||||||  Journal:  Shift in hospital opioid use during the COVID-19 pandemic in Brazil: a time-series analysis of one million prescriptions. (Pubmed Central) -  Nov 3, 2023
    It was significantly lower for parenteral morphine parenteral sufentanil, and parenteral tramadol...The number of patients in 2020 was lower but the mean consumption was higher for fentanyl, parenteral methadone, and remifentanil...There was a relevant increase in hospital consumption of some potent opioids during the COVID-19 pandemic in Brazil. These results reinforce the concern about epidemiological surveillance of opioid use after periods of increased hospital use since in-hospital consumption can be the gateway to the misuse or other than the prescribed use of opioids after discharge.
