Clinical, Review, Journal:  New-onset epilepsy in the elderly. (Pubmed Central) -  Oct 13, 2019
There is a need for more RCTs that investigate effective treatments with limited side effects. More research studies on the psychosocial effects of new onset epilepsy, and long-term outcomes, for people aged 60 years are also required.

Clinical, Journal:  Extrapolation of a Brivaracetam Exposure-Response Model from Adults to Children with Focal Seizures. (Pubmed Central) -  Oct 9, 2019
Levetiracetam is significantly more effective than phenytoin for the treatment of convulsive status epilepticus in children who have failed to respond to benzodiazepines. Simulation with the adult brivaracetam PK/PD model in combination with an existing pediatric brivaracetam population PK model allowed characterization of the dose-response curve, suggesting maximum response at brivaracetam 4 mg/kg/day dosing (capped at 200 mg/day, the maximum adult dose) in children aged ≥4 years.

In this setting, immediate switch from LEV to BRV at a 10:1-15:1 ratio without titration is feasible. Further research is needed to examine the long-term tolerability and efficacy of BRV as well as its role in the treatment of other types of epilepsies, particularly dementia-related epilepsy and brain tumor-related epilepsy.

Retrospective data, Journal, Monotherapy:  The Risk Of Seizure-Related Hospitalization Among Older Adults On Levetiracetam Monotherapy: A Retrospective Comparative Cohort Study. (Pubmed Central) -  Oct 3, 2019
Patients on levetiracetam were more than twice as likely to be hospitalized due to seizures within the 24 months of follow-up compared to their counterparts on other AEDs (RR=2.76, 95% CI=1.16-6.53, P=0.021). This study suggests that older adults on old generation AEDs such as phenytoin, carbamazepine, and valproic acid appear to have a lower risk of seizure-related hospitalization compared to their counterparts on levetiracetam.

Clinical, PK/PD data, Journal:  Levetiracetam Pharmacokinetics in a Critically Ill Anephric Patient on Intermittent Hemodialysis. (Pubmed Central) -  Oct 2, 2019
Insufficient intradialytic or post-dialysis levetiracetam concentrations may place patients at risk of breakthrough seizures. This case indicates that dialysis patients on levetiracetam may require higher post-dialysis supplemental doses than currently recommended and tailored therapy supported by therapeutic drug monitoring.

Biomarker, Journal:  Electronic Health Data Predict Outcomes After Aneurysmal Subarachnoid Hemorrhage. (Pubmed Central) -  Oct 2, 2019
Variance in early physiologic data can impact patient outcomes and may serve as targets for early goal-directed therapy. Electronically retrievable features such as ICP, glucose levels, and electroencephalography patterns should be considered in disease severity and risk stratification scores.

Journal, Adverse events:  Adverse Pregnancy Outcomes Associated With Gestational Exposure to Antiepileptic Drugs. (Pubmed Central) -  Oct 2, 2019
These caveats notwithstanding, guidance should err on the side of caution. Therefore, a conservative conclusion is that whereas analyses based on exposures in 2,000-4,000 pregnancies suggest that lamotrigine and carbamazepine are not associated with an increased risk of fetal loss, IUGR, and preterm birth, there is insufficient evidence to either firmly indict or firmly exonerate the other AEDs with regard to these outcomes.

Journal:  Effects of levetiracetam on astroglial release of kynurenine-pathway metabolites. (Pubmed Central) -  Oct 2, 2019
This study demonstrated several mechanisms of LEV: 1) inhibition of AMPA- and AdA-induced astroglial release, 2) inhibition of IFNγ-induced IP3-receptor activation, and 3) inhibition of release of cinnabarinic acid and quinolinic-acid with activation of that of kynurenic-acid induced by IFNγ. These combinations of actions of LEV may contribute to the unique profiles of LEV.

Journal:  Major Congenital Malformations Associated With Exposure to Antiepileptic Drugs During Pregnancy. (Pubmed Central) -  Oct 2, 2019
In the AEDs associated with elevated risks, the extent to which the risks are due to confounding by indication is unknown. Importantly, confounding by indication notwithstanding, at conventional doses lamotrigine, levetiracetam, and oxcarbazepine, and possibly zonisamide and gabapentin, as well, are associated with absolute MCM risks that are no greater than the 2%-3% MCM risk in the general population.

Journal:  Levetiracetam-mediated improvement of decreased NMDA-induced glutamate release from nerve terminals during hypothermia. (Pubmed Central) -  Oct 2, 2019
Therefore, levetiracetam administration significantly mitigated a hypothermia-induced decrease in NMDA receptor response at the presynaptic level and can be used for the targeted neurocorrection to reduce side effects of hypothermia in cardiac surgery. However, levetiracetam-mediated improvement of NMDA receptor response is not applicable in stroke, brain trauma and neonatal asphyxia therapies, where the main neuroprotective action of hypothermia is associated with prevention of damaging consequence of pre-existing acute glutamate exitotoxicity.

Clinical, Journal:  Modeling and simulations to support dose selection for eslicarbazepine acetate therapy in pediatric patients with partial-onset seizures. (Pubmed Central) -  Sep 28, 2019
Model-based simulations were subsequently performed to apply target exposure matching of selected ESL doses for pediatric subjects (aged 4-17 years) to attain eslicarbazepine exposures associated with effective and well-tolerated ESL doses in adults. Overall, model-based exposure matching allowed for extrapolation of efficacy to support pediatric dose selection as part of the submission to obtain FDA approval for ESL (adjunctive therapy and monotherapy) in subjects aged 4-17 years, without requiring an additional clinical study.

Clinical, PK/PD data, Review, Journal:  Pharmacokinetic Considerations with the Use of Antiepileptic Drugs in Patients with HIV and Organ Transplants. (Pubmed Central) -  Sep 28, 2019
Antiepileptic drugs with no or minimal enzyme-inducing properties include brivaracetam, gabapentin, lacosamide, lamotrigine, levetiracetam, perampanel, pregabalin, and vigabatrin. The newer generations of antiepileptic drugs have expanded the therapeutic options in patients with HIV infection or organ transplants.

Journal:  Effect of newer anti-epileptic drugs (AEDs) on the cognitive status in pentylenetetrazol induced seizures in a zebrafish model. (Pubmed Central) -  Sep 28, 2019
The gene expression levels of Brain-Derived Neurotrophic Factor (BDNF), Neuropeptide Y (NPY) and Cyclic Adenosine Monophosphate (CAMP) Responsive Element Binding Protein 1 (CREB-1) were not found to be significantly different in AED treated zebrafish. Based on the experimental results, a decrease in brain glutamate levels due to AED treatment appears to be at least one of the major factors behind the observed cognitive impairment in the treated zebrafish.

    Novel Mutation of NUS1 Gene Presenting With Developmental And Epileptic Encephalopathy and Movement Disorders (Les Muses Terrace, Level 3) -  Sep 24, 2019 - Abstract #MDSCongress2019MDS_734;
Our patient presents with ataxia, intentional tremors and prominent action myoclonus of arms and facial myoclonus, likely due to cortical and cerebellar involvement. We also demonstrate evidence of nigrostriatal degeneration, an interesting finding in light of a recent report of NUS1 as possible candidate gene for early onset Parkinson’s disease. NUS1 encodes for Nogo-B receptor (NgBR) which along with dehydrodolichyl diphosphate synthase (DHDDS) are critical for protein glycosylation.
