    This case illustrates that CSF findings in patients with transverse myelitis due to MOGAD can demonstrate marked pleocytosis and elevated protein in the acute phase - similar to findings expected in infectious flaccid myelitis. The possibility that infections trigger MOGAD remains under investigation, though some case series demonstrate EBV, HSV and Influenza as possible triggers of MOGAD encephalomyelitis 3,4.

    She was tested for total Covid-19 antibodies which was significantly positive with 55.25 COI. Patient was treated initially with IV Methylprednisolone with no significant response. Atypical Pharyngo-cervico-brachial variant of GB Syndrome with gross asymmetrical upper limb weakness and progressed over six weeks associated with positive SARS Cov-2 antibodies. During the pandemic, unusual neurological manifestations should be evaluated for possibility of SARS-CoV-2 associated antibodies as neuropathogenesis has shown both vascular and post infectious demyelinating disorders.

    Background: We report a case with successful outcome of pembrolizumab associated anti-striational antibody myasthenia gravis (MG) after being treated aggressively with methylprednisolone, Intravenous immunoglobulin (IVIG) and plasmapheresis (PLEX). Early aggressive treatment with IVIG or PLEX and steroids, regardless of initial symptoms severity, may lead to better outcomes in patients presenting with pembrolizumab associated anti-striational antibody MG.

    Lupus cerebritis and PRES have overlapping clinical features, radiographic findings, and CSF profiles.4-9  The finding of FLAIR hyperintense lesions in bilateral occipital and parietal areas on brain MRI, in the setting of uncontrolled hypertension and encephalopathy, is a classic radiographic pattern of PRES; however,  other areas may also be involved radiographically.5,9 Microinfarctions and inflammatory CSF findings, such as pleocytosis and elevated protein,  may be present in either condition.6 These overlapping features highlight a need for appropriate interpretation considering the clinical context. Our patient’s history of prior CNS lupus, encephalopathy in the setting of an active lupus flare and concurrent uncontrolled hypertension prompted initiation of treatment of both potential etiologies with subsequent improvement.

    Due to atypical/overlapping pathological characteristics, it was initially misdiagnosed as extramedullary plasmacytoma and treated with two cycles of bortezomib, lenalidomide, and dexamethasone...The treatment was adjusted to etoposide, prednisone, vincristine, cyclophosphamide, doxorubicin, rituximab, and lenalidomide (R2-EPOCH), but no response was observed after three cycles of treatment and he developed hemophagocytic syndrome during treatment...The response assessment was partial for EBV-positive DLBCL, subsequent anti-CD19 chimeric antigen receptor-T (CAR-T) cell therapy resulted in complete remission including lumps, immunoglobulins, and negative EBV-DNA 1.5 months later. The present case study proved the possibility of PD-1 blockade in controlling EBV infection and associated hemophagocytic syndrome and offered an example of the combination of CAR-T therapy and PD-1 blockade for refractory EBV-positive DLBCL in clinic.

    Changing sphere sizes, pore structure and encapsulating dexamethasone could effectively modulate the zein-induced immune responses, especially the pore structure which also inhibited neutrophil recruitment and promoted macrophages polarizing towards M2 phenotype. Furthermore, the pore structures in zein nerve conduits was proved to alleviate the early inflammation and promote M2 macrophages polarization to accelerate nerve regeneration.

    This prospective phase II trial provides a suggestion that the antiviral effects of CIP from early in the COVID-19 pandemic may delay progression to critical illness and death in specific patient populations. This study informs the optimal timing and potential population of use for CIP in COVID-19, particularly in settings without access to other interventions, or in planning for future coronavirus pandemics.
