    The lidocaine rapidly reversed the symptoms of pain and pressure, and the low dose of epinephrine, within 2 hours, significantly reversed the allergic periorbital and eyelid edema. No further symptoms evolved, suggesting that the epinephrine terminated the allergic cascade.

    A 42-year-old female with a history of multiple sclerosis on glatiramer acetate injections presented to dermatology clinic with a two-week history of painful skin changes on left hip that started one day after her last glatiramer acetate injection...The patient was diagnosed with embolia cutis medicamentosa and started on triamcinolone 0.1% ointment daily for two weeks, vaseline, and warm compress...Other injectables linked to Nicolau syndrome include NSAIDs (especially diclofenac), IM benzathine penicillin, local anesthetics such as lidocaine, and, less commonly, injectable vitamins including Vitamin K and cyanocobalamin...Early pharmacological interventions, such as vasodilators (nitroglycerin or nifedipine) to enhance blood flow, and anticoagulants to prevent vascular occlusion, may also be considered. In severe cases, surgical debridement may be required for extensive necrosis.

    The incidence of endophthalmitis after cataract surgery has significantly decreased over the past twenty-three years in our tertiary care center. The use of PVI before surgery along with systematic intracameral cefuroxime has allowed us to achieve very low rates of endophthalmitis.

    ?Surgically used doses of papaverine and verapamil, but not lidocaine, have lasting negative effects on arterial vasodilatory function despite the acute effects of the drugs having worn off. These findings, in conjunction with the spasmolytic properties of each drug, may help guide the selection of an optimal topical vasodilator for use during microvascular surgeries.

    In addition, topical anesthesia of the upper airway was performed using a specifically designed device called the Trachospray, which nebulizes lidocaine into a fine mist that precipitates in the upper airway of the patient upon inhalation...This combination led to adequate anxiolysis and sedation, preservation of spontaneous breathing throughout the procedure, good conditions for successful intubation, and amnesia for the endoscopy and intubation. Further research is needed to establish the superiority of any of these techniques in comparison to other accepted methods.

    Retrospective data, Journal:  Efficacy and safety of topical antibiotic therapy in patients with acute external bacterial otitis: results of a retrospective study (Pubmed Central) -  Dec 27, 2024   
    Out of 2545 patients included in the analysis 92.1% received a combination drug containing polymyxin B sulfate, neomycin sulfate and lidocaine hydrochloride (Anauran) as monotherapy, whereas 7.9% received it in combination with other antimicrobials...Eradication of pathogens was observed in 95.1% of cases. In total 99.6% of patients reported completely no side effects during the treatment course suggesting high level of effectiveness and safety of the drug.

    Participants were randomly assigned to either the cold vibrating device or topical anesthetic (4% liposomal lidocaine; standard of care)...The cold vibrating device was not considered non-inferior to the topical anesthetic cream for pain management in children during a needle-related procedure in the ED. As topical anesthetic creams require an application time of 30?min, cost approximately CAD $40.00 per tube, are underused in the ED setting, the cold vibrating device remains a promising alternative as it is a rapid, easy-to-use, and reusable device.

    Patients who received preparation of the nasal mucosa with TCLE, compared with TE, were less likely to report severe pain or receive an opioid analgesic in the postanesthesia recovery room and had faster anesthetic recovery. This observation from our large clinical practice indicates that use topical and local anesthetic during endoscopic sinus surgery may have benefit for ambulatory ESS patients.

    The choice of analgesic(s) and the route of administration should be guided by the pain characteristics, as well as by the patient's comorbidities, organ function and medications. Several directions have been highlighted to optimise pharmacological pain management in older individuals: (1) before starting pain treatment adequately detect and assess pain and always perform a full geriatric assessment, (2) consider kidney function systematically to adjust the doses of analgesics and avoid the risks of overdose, (3) start with the lowest dose of an analgesic and increase it gradually under the control of the effect, (4) involve the older persons and family in their treatment, (5) reevaluate pain regularly during treatment and (6) combine pharmacological treatment with non-pharmacological approaches.
