HYPEREOSINOPHILIA IN A PATIENT WITH UNCONTROLLED ASTHMA (Hall B (1st Floor); Monitor 07) - Nov 2, 2021 - Abstract #ACAAI2021ACAAI_867; Discussion Hypereosinophilia is a serious condition with potentially severe systemic consequences. Asthma rarely causes marked elevations in peripheral eosinophils, however in this case, an individual with uncontrolled asthma and hypereosinophilia demonstrated complete resolution of his eosinophilia with medical management of his asthma.

MOTOR TICS, AN UNKNOWN SIDE EFFECT OF INTRANASAL FLUTICASONE (Hall B (1st Floor); Monitor 01) - Nov 2, 2021 - Abstract #ACAAI2021ACAAI_708; Exogenous suppression of the HPA axis has been proposed as a cause, as patient’s with tics have lower levels of evening cortisol. Care should be taken when initiating fluticasone in this population as there is likely a link.

Journal: Exhalation Delivery Systems for Application of Intranasal Corticosteroids. (Pubmed Central) - Oct 27, 2021 Further research is needed to investigate the long-term outcomes of EDS-FLU and to compare the effects of EDS-FLU with ESS. Exhalation delivery system with fluticasone provides an additional effective treatment modality for patients suffering from CRS.

Journal: Treatment for eosinophilic esophagitis. (Pubmed Central) - Oct 20, 2021 Empiric elimination is presently the preferred dietary approach. Swallowed steroids is the standard of care to treat EoE with medication; however, there are several promising drugs currently undergoing clinical trials.

Review, Journal: Eosinophilic Esophagitis: A Review. (Pubmed Central) - Oct 16, 2021 EoE has a prevalence of approximately 34.4/100 000 worldwide. Treatments consist of proton pump inhibitors, topical steroids, elemental diet, and empirical food elimination, with esophageal dilation reserved for patients with symptomatic esophageal narrowing.

[VIRTUAL] INTRACTABLE COUGH CAUSING SPONTANEOUS RECTUS SHEATH HEMATOMA () - Oct 13, 2021 - Abstract #CHEST2021CHEST_2394; His OTC medications were stopped and he was given a course of oral prednisone along with fluticasone and azelastine nasal spray with dramatic improvement and complete resolution of cough. This case highlights the potential of sustained vigorous cough to cause abdominal wall hematomas even in the absence of anticoagulants.

[VIRTUAL] ISOLATED SEGMENTAL BRONCHIAL STENOSIS RELATED TO CROHN'S DISEASE () - Oct 13, 2021 - Abstract #CHEST2021CHEST_2166; He later transitioned to adalimumab in 2017...His bronchitis was managed with inhaled fluticasone and as needed albuterol.Computed tomography (CT) in February 2019 for surveillance of pulmonary nodules showed stable nodules with interval appearance of concentric narrowing in the proximal segmental bronchus to the superior segment of the left lower lobe (LLL)...The patient was treated with a course of prednisone and levofloxacin and was started inhaled budesonide and formoterol... This case highlights the role of interventional pulmonology in management of a rare case of isolated segmental bronchial stenosis related to CD.

[VIRTUAL] ALL THAT WHEEZES IS NOT ASTHMA () - Oct 13, 2021 - Abstract #CHEST2021CHEST_1574; Refractory asthma can be a red herring and should prompt further investigation. For a young patient with seronegative EGPA, every attempt should be made to reduce steroid dependence and ensure close follow-up.

Clinical, Journal: Medication adherence in patients with asthma using once-daily versus twice-daily ICS/LABAs. (Pubmed Central) - Sep 19, 2021 The proportion of patients achieving PDC ≥ 0.5 or PDC ≥ 0.8, and treatment persistence were significantly higher for FF/VI versus B/F and FP/SAL (all p < 0.001). In this real-world study, patients initiating FF/VI had better adherence and lower risk of discontinuing treatment versus B/F or FP/SAL, suggesting that once-daily ICS/LABA treatment might improve adherence and persistence compared with twice-daily alternatives.

Clinical, Review, Journal, HEOR, Real-world evidence: Inhaled Formoterol-Fluticasone Single Inhaler Therapy in Asthma: Real-World Efficacy, Budget Impact, and Potential to Improve Adherence. (Pubmed Central) - Sep 9, 2021 The present review focuses on some of the, hitherto, less explored aspects of this combination inhaler such as real-world efficacy, impact on budget allocation, results of switch-over therapy, and potential to improve adherence to asthma treatment. It also provides practical recommendations on positioning it in real-world asthma management.

Eosinophilic Esophagitis: An Unusual Cause of Dysphagia After Radiation Therapy (Shoreline Exhibit Hall) - Sep 8, 2021 - Abstract #ACG2021ACG_2865; He was treated with radiation therapy and chemotherapy containing cisplatin in 2014...He had improvement in symptoms on pantoprazole 40mg twice daily and fluticasone 220 mcg , 2 puffs swallowed twice daily for a month...It is important to consider this diagnosis in patients with radiation esophagitis with suspicion for eosinophilic esophagitis as they should undergo cautious dilation. Despite normal endoscopic appearance, biopsies should be considered to rule out eosinophilic esophagitis even in older adults with history of radiation exposure.Figure: Linear furrows and esophageal rings consistent with eosinophilic esophagitis.

Efficacy of Topical Corticosteroids in Achieving Histologic, Clinical, and Endoscopic Remission in Patients With Eosinophilic Esophagitis: A Systematic Review and Meta-Analysis of Randomized Controlled Trials (Shoreline Exhibit Hall) - Sep 8, 2021 - Abstract #ACG2021ACG_1570; Topical NSAIDs were effective in inducing both complete (OR 44.75, 95% CI [20.54, 148.53], P< 0.00001) and partial histologic response (OR 30.80, 95% CI [9.98, 95.06], P< 0.00001) compared with placebo. Subgroup analysis showed no significant differences among the mode of topical NSAIDs delivery (oral suspension, spray, or orodispersible tablet) or the NSAID agent used (budesonide or fluticasone) in achieving complete histologic response (P=0.2 and P=0.4, respectively).

Topical Steroids for Eosinophilic Esophagitis: A Systematic Review and Meta-Analysis (Shoreline Exhibit Hall) - Sep 8, 2021 - Abstract #ACG2021ACG_506; The meta-analysis for budesonide versus placebo clinical remission showed that budesonide had increased odds for clinical remission (OR:6.15, 95% CI: 1.63, 23.15) with 5 of 6 studies favoring budesonide. Comparisons for budesonide versus placebo histological response showed that budesonide had increased odds for histological change (OR: 41.96, 95% CI: 13.09, 134.51) with all 5 studies favoring budesonide.
