    Despite a small sample size, these insights offer valuable information into the resistance patterns of PLWHIV in South Africa, failing NNRTI-based first-line ART regimens. Cross-resistance within NNRTIs may be more prevalent with doravirine than recorded in previous clinical trials, this potentially compromising their role as an option for patients failing EFV-based regimens, particularly with the introduction of dolutegravir into first-line ART programmes in South Africa.

    Background: Two-drug regimen (2DR) in naive (DTG + lamivudine (3TC)) and treated-experienced HIV patients (DTG+3TC, DTG+RPV, CAB (cabotegravir)+RPV) have been positioned as another strategy in clinical practice. Switching to DTG plus RPV in STR in treatment-experienced HIV patients is an effectiveness strategy, with a favourable lipid profile, which does not influence the inflammatory markers.

    Review, Journal:  The future of long-acting cabotegravir plus rilpivirine therapy: Deeds and misconceptions. (Pubmed Central) -  Aug 24, 2022   
    The combination of at least two baseline factors among HIV-1 subtype A6/A1, a body mass index ≥30 kg/m, and RPV resistance associated mutations, was associated with an increased risk of CVF at week 48. Available data indicate that this long-acting therapeutic strategy is attractive and potent, thus defining the most appropriate patient and how to handle practical issues is warranted.

    Our estimates of VF > 50 copies/mL and resistance in participants treated with RPV were similar to those reported by other studies. RPV safety profile was favourable with less frequent discontinuation due to toxicity than EFV (especially for CNS).

    In replication competition experiments in the presence of the NNRTI rilpivirine (RPV), V179I modestly enhanced Y181C HIV-1 or Y181V HIV-1 replication compared to viruses without V179I...In RPV selection experiments, the frequency of V179I in HIV-1 was accelerated in CD4+ T cells expressing higher APOBEC3F and APOBEC3G levels. Our results provide evidence that V179I in HIV-1 RT can arise due to APOBEC-mediated G to A hypermutation and can confer a selective advantage to drug-resistant HIV-1 isolates in the presence of some NNRTIs.

    Novel RT inhibitors such as islatravir, MK-8504, MK-8507, MK8583, IQP-0528, and MIV-150 will be also highlighted. Future development may focus on the new generation of novel antiretroviral inhibitors with higher bioavailability, longer elimination half-life, more favorable side-effect profiles, fewer drug-drug interactions, and higher activities against circulating drug-resistant strains.

    Retrospective data, Journal:  Dolutegravir plus rilpivirine: benefits beyond viral suppression: DORIPEX retrospective study. (Pubmed Central) -  Jun 22, 2022   
    In acquired immunodeficiency syndrome-diagnosed patients, we found a significant increase in the CD4+ T-cell count at week 48 (log OR = 41.7, P = .0038), but no significant changes in the CD8+ T-cell count (log OR = -23.4, P = .54). No differences were found in the CD4/CD8 ratio between the acquired immunodeficiency syndrome subgroup and sex or age.In patients with controlled treatment, dual therapy with DTG plus RPV slightly improved the immune status during the first 48 weeks after switching, not only in terms of CD4+ T-cell count but also in terms of CD8+ T-cell count, with persistently high rates of viral control.

    Journal:  Synergistic interactions of repurposed drugs that inhibit Nsp1, a major virulence factor for COVID-19. (Pubmed Central) -  Jun 22, 2022   
    Multiple synergistic combinations of compounds that significantly inhibited Nsp1 action were identified. Among the most promising combinations are Ponatinib, Rilpivirine, and Montelukast, which together, reversed the toxic effects of Nsp1 to the same extent as null mutations in the Nsp1 gene.

    The association between the episode of alopecia and DOR/TDF/3TC was scored as probable according to the Naranjo causality scale. We concluded that alopecia was probably related to DOR but whether it is self-limiting, cannot be predicted at this stage.

    Generic competition will become possible immediately, in the case of Etravirine, and by the second semester of 2022 for Rilpivirine, FTC/Rilpivirine/TDF and Raltegravir. For the Brazilian HIV/Aids treatment program, this decision represents an opportunity both for budget savings and for the incorporation of new drugs at affordable prices.

    Other treatments for COVID-19 studied here did not demonstrate significant DDIs with ARVs. As new COVID-19 therapeutics emerge, and given the prevalence of HIV worldwide, DDIs with ARVs must continue to be assessed.

    Of 956 enrolled, 673 switched to BIC/F/TAF, 148 switched to F/TAF plus DTG, 51 switched to DTG/ABC/3TC, 48 switched to DTG/RPV and 36 switched to DTG/3TC...Compared to switching from integrase strand transfer inhibitors (INSTIs), switching from efavirenz (EFV) was associated with significantly higher annualized weight gain post-switch (+3.11 kg/year, p<0.001), whereas switching from RPV was associated with significantly lower annualized weight gain post-switch (-2.17 kg/year, p=0.022)... In this real-world cohort, switching to a BIC vs. DBR were both associated with lower annualized weight gain post-switch that was not significantly different at Week 48; however, patients switching from EFV gained significantly more weight, while those switching from RPV gained significantly less weight post-switch compared to those switching from INSTIs.
