We present a case of a BD patient with profound disruption in sleep/wake activity and show that lithium potently corrected her circadian rhythm disturbances. Taken together, these data suggest that lithium's therapeutic mechanism involves the maintenance of ER homeostasis via increased MANF gene expression mediated by the AP-1 transcription factor.

Our data showed: (a) lithium levels are often maintained at the lower end of the recommended therapeutic range, (b) patterns of lithium results and testing frequency were comparable across three UK sites with differing models of care and, (c) re-test intervals demonstrate a noticeable peak at the recommended 3-monthly, but not at 6-monthly intervals. Many tests were repeated outside expected frequencies, indicating the need for measures to minimise inappropriate testing.

Introduction Published evidence describes the appearance of manic episodes in patients who suffer localized brain lesions with no prior psychiatric history. Objectives A case report is presented alongside a review of the relevant literature regarding the relationship between Bipolar disorder and strokes. Methods We present the case of a 54-year-old man who, after suffering a pontine hemorrhage, developed a depressive mood for which he was treated with Sertraline 50 mg...He was treated with Risperidone 2 mg and the antidepressant was withdrawn...He was treated again with antidepressants (Citalopram 30mg) and lithium was introduced in the absence of a total response. Results Mania secondary to brain lesions has been observed in multiple studies, where an association is made mainly with lesions at the frontal, temporal, subcortical limbic brain areas and in lesions causing hypofunctionality on the right side. Most of the cases described occurred in male patients with no prior psychiatric record and with associated vascular risk. Conclusions It is important to carry out an exhaustive medical history to be able to identify the cases of secondary mania so as not to ignore the underlying neurological condition in the approach.

One of them benefited from a combination of clozapine and lithium with excellent response. Conclusions Bipolar disorder comorbid with ID in adolescents is a difficult diagnostic entity and particularly hard to manage.
