[VIRTUAL] Allergic Contact Dermatitis to Sodium Metabisulfite in Lidocaine with Epinephrine () - Jan 30, 2021 - Abstract #CDAI2020CDA-I_86; His usual medications included esomeprazole, celecoxib, pregabalin, acetaminophen and hydroxyzine.Physical examination revealed numerous erythematous papules and vesicles on poorly-circumscribed, eczematous plaques...Results of his patch test read at 96-hours: sodium metabisulfite (+++), bacitracin (+++), polymyxin-B (+++), tixocortol-21-pivalate (+++), pramoxine (++) and linalool (+).ConclusionsThe acute eruption within one day postoperatively, along with clustering of lesions around lidocaine infiltration sites and positive patch test results, suggest that the patient had developed a systemic contact dermatitis to sodium metabisulfite, present in the lidocaine with epinephrine administered perioperatively...He was started on topical clobetasol ointment and recovered in a few weeks.Learning Objective1.True allergy (Type I hypersensitivity) to local anesthetic is rare.2.Sodium metabisulfite is an antioxidant present in local anesthetic solutions with vasoconstrictors, and can be a cause of allergic contact dermatitis.3.Sodium metabisulfite can also be present in other topical creams, such as hydrocortisone and ketoconazole creams.4.Patients with contact dermatitis to sodium metabisulfite should be patch tested for plain lidocaine and plain epinephrine (without the added sodium metabisulfite) to determine allergy status.Takeaway MessageSodium metabisulfite is present, as an antioxidant, only in local anesthetic solutions with vasoconstrictors.
