Following transplantation, she was decannulated from ECMO; however, over the next 12 hours, oxygenation deteriorated requiring reinstitution of VV-ECMO for presumed severe primary graft dysfunction. Despite treatment with broad spectrum antimicrobial coverage with piperacillin/tazobactam, ciprofloxacin, linezolid, micafungin, voriconazole, and ganciclovir, she failed to improve and developed complex bilateral pleural effusions.

We propose that replication fork collapse at carrier-size (CGG) repeats can trigger break-induced replication, which results in simultaneous repeat length changes and mutagenesis at a distance.

Identification of patient factors associated with CMV reactivation would allow for optimization of the study population.Methods : We performed a secondary pooled analysis of two prospective cohorts with sepsis: an observational cohort of ICU patients (n=40) and the placebo cohort from a randomized, double-blind trial of ganciclovir to prevent CMV reactivation in acute critical illness (n=66)...We did not identify other patient variables associated with subsequent CMV reactivation.Conclusion : CMV reactivation in seropositive adults with sepsis is independently and quantitatively associated with clinically-important outcomes, including death or continued hospitalization by day 28, ventilator-, ICU-, and hospital-free days. These effect sizes provide key data to inform design parameters of future interventional trials.

When only those who received > 28 days of low dose prophylaxis (N=6, 6%) were evaluated, there was a trend towards an increased hazard of CMV infection (HR=1.001, 95% CI 0.999-1.004, P=0.18; Table 2).Conclusion : CMV D+ LTR are frequently exposed to inappropriately low CMV prophylaxis dosing. This does not appear to significantly increase the risk for CMV infection, though prolonged subtherapeutic exposure merits further exploration as a risk factor for CMV outcomes in higher-risk patients.

This infection requires a differential diagnosis of consciousness impairment that develops in a patient with lymphoid malignancy during chemotherapy.

This raises the question if CMV testing should be done prior to initiation of immunosuppressive therapy in these patients and if CMV testing should be done regularly in patients with refractive UC flares.

A significant improvement in kidney and liver functions was observed after successful treatment of the infection.
