    ACE Inhibitor-Associated Isolated Small Bowel Angioedema: A Rare Cause of Abdominal Pain (Exhibit Hall) -  Jul 29, 2023 - Abstract #ACG2023ACG_4646;
    We present a very rare case of Lisinopril associated isolated small bowel angioedema Case Description/ A 47-year-old female presented with severe upper abdominal pain associated with nausea and vomiting...It is often missed as a cause of abdominal pain which leads to unnecessary work-up and sometimes, high-risk surgical interventions. Though self- limiting, if ACE-I induced isolated angioedema is not diagnosed and managed early, continued use may lead to significant morbidity in affected patients Figure: Mild bowel wall thickening and mucosal enhancement of the jejunum and ileum and small amount of ascites

    A Rare Case of Visceral Angioedema Secondary to Angiotensin-Converting Enzyme Inhibitor (Exhibit Hall) -  Jul 29, 2023 - Abstract #ACG2023ACG_4636;
    It was presumed that the edema was likely medication-induced and she was asked to stop taking Lisinopril...It would avoid unnecessary invasive procedures such as exploratory laparotomy, intestinal biopsy, and resection. Figure: CT Abdomen with mural thickening of the proximal small bowel loops in the left upper quadrant with the bowel wall measuring up to 1 cm.

    Unexpected Finding: Pseudomelanosis Duodeni on Surveillance Endoscopy in a Patient With Hemosiderosis (Exhibit Hall) -  Jul 29, 2023 - Abstract #ACG2023ACG_3894;
    The patient's pertinent outpatient medications include iron carbonyl hydralazine, furosemide, and lisinopril...Differentiating between PD, iron pill-induced mucositis, clofazimine related pigmentation, diffuse malignant melanoma, brown bowel syndrome and eosinophilic enteritis is crucial, as the management and prognosis vary significantly...(B) Histopathology showed brown-black pigment deposition in lamina propria macrophages of duodenum on H&E 40x stain. (C)The tissue stained positive for Iron on Iron Stain 20x.

    He had been taking lisinopril 40 mg daily for 7 years prior to presentation...Recurrent abdominal pain with bowel wall edema should include consideration of ACEi-induced intestinal angioedema to avoid unnecessary testing and to decrease patient morbidity. Figure: Figure: CT abdomen and pelvis demonstrating thickening of (A) the cecum and (B) the small bowel loops with associated mesenteric edema.
  • ||||||||||  Protein-Losing Enteropathy: An Isolated Presentation of Lupus Nephritis (Exhibit Hall) -  Jul 29, 2023 - Abstract #ACG2023ACG_3131;
    She had 1 prior hospital admission with a similar complaint and was diagnosed with a UTI, colitis, anemia, and unspecified fever, and was treated with a course of cefuroxime...Patient was started on high dose steroid taper, hydroxychloroquine, azathioprine, and lisinopril with subsequent improvement...Endoscopic biopsies aid in excluding other disease processes, but may not may not define the etiopathogenesis of protein-loss enteropathy, and in many cases, they are normal or show nonspecific findings. Early and aggressive steroid and immunosuppressant therapy is necessary to improve prognosis.

    Dermal facial fillers and COVID-19 vaccination: About a case () -  Jul 3, 2023 - Abstract #WCD2023WCD_5176;
    Delayed inflammatory reactions cases related to HA dermal fillers are likely to become prevalent due to the ongoing popularity of HA dermal fillers and as the rate of SARS COV2 infections and vaccination is increasing around the world. Therefore, Aesthetic medicine practitioners should be aware of the risk of DIR and patients who wish to receive a HA soft tissue filler should be informed of the risk of COVID-19 and infection-related adverse reactions prior to treatment.

    Journal:  Identification and Characterization of Novel ACE Inhibitory and Antioxidant Peptides from Sardina pilchardus Hydrolysate. (Pubmed Central) -  Jun 10, 2023
    Molecular docking was used to screen for peptides with ACE inhibitory activity, resulting in the identification of 11 peptides with higher -CDOCKER ENERGY and -CDOCKER INTERACTION ENERGY scores than lisinopril...After experimental verification, it was found that all six of these peptides have antioxidant activities, and we also found that the SPH and ultrafiltration fractions of SPH had antioxidant activities. These findings suggest that Sardina pilchardus may be a potential source of natural antioxidants and ACE inhibitors for the development of functional foods, and using LC-MS/MS in combination with an online database and molecular docking represents a promising, effective, and accurate approach for the discovery of novel ACE inhibitory peptides.

    Despite of the factors predictive for severe clinical manifestations of TSC (TSC2 mutation, polycystic kidney changes), the successful control of cardiological and neurological TSC-manifestations was obtained in the reported case. Initiation of sirolimus therapy in infancy might have positive impact on the clinical outcome, thus additional studies are needed to establish the optimal treatment of newborns with TSC.
