He denies any prior episodes of acute pancreatitis and has no symptoms of chronic pancreatitis such as chronic abdominal pain or steatorrhea. In conclusion, this case brings a question to whether DPP-4 inhibitors trigger pancreatitis in patients who have other risk factors to develop pancreatitis such as history of alcohol abuse or whom are on diuretics, etc. Future studies should be done to evaluate whether patients with a prior history of ethanol abuse, with no history of acute or chronic pancreatitis, have a greater likelihood of developing pancreatitis following the initiation of DPP-4 inhibitors.

Steroids should be avoided in patients with known pheochromocytoma or in those with an adrenal adenoma without a negative biochemical workup for a pheochromocytoma as it could precipitate an adrenergic crisis possibly by inducing catecholamine synthesis and release and sensitizing target organs to the effects of catecholamines.

These data suggest that lisinopril/lisinopril + hydrochlorothiazide, losartan/losartan + hydrochlorothiazide and valsartan/valsartan + hydrochlorothiazide alone or in combination with amlodipine are equally effective and well tolerated for the reduction of both systolic and diastolic blood pressure and improve arterial stiffness in patients with essential hypertension.

The second found reduced cardiovascular morbidity with atenolol compared to lisinopril in mostly black hypertensive hemodialysis patients...A large cohort of hemodialysis patients with new-onset heart failure showed better survival when treated with carvedilol, bisoprolol or metoprolol...On top of that, many observations suggested that beta-blockers were associated with a reduced rate of sudden cardiac death in hemodialysis patients but recent data show a link between bradycardia and sudden cardiac death questioning the benefit of beta-blockade in this population. Finally, what we know for sure so far is that beta-blockers should be avoided in patients with intradialytic hypotension associated with bradycardia.

Nickeleit – The University of North Carolina at Chapel Hill A 60 year-old Caucasian male with a 10-year history of hypertension treated with Lisinopril and Hydrochlorothiazide was referred to an outside institution for evaluation of de-novo onset nephrotic range proteinuria 5 months prior to admission...Due to a clerical/accessioning error the biopsy was classified as a “transplant biopsy” and a C4d stain was added to the IF panel; C4d showed diffuse global pseudolinear (3+-4+) staining along glomerular capillary walls; no characteristic extra glomerular/peritubular capillary C4d staining was present – see accompanying IF and IHC images with stains for C4d.
