She previously had similar chest pain 3 months ago at which time Dobutamine stress test did not show any signs of ischemia and ejection fraction (EF) was estimated at 60%...Patient was discharged with carvedilol and lisinopril...However, it has been speculated that catecholamine excess with concomitant coronary vasospasm leads to methamphetamine associated cardiomyopathy.1 Methamphetamine induces vigorous vasoconstriction which potentially results in severe vasospasm of the coronary arteries that can present as angina.2 A study of 20 methamphetamine users and 21 age-matched controls showed reduced nitroglycerin-mediated vasodilation in the methamphetamine users, suggestive of smooth muscle dysfunction and reduced nitric oxide sensitivity from methamphetamine.3 Chen reported a 19 year old male with history of methamphetamine abuse who was found to have global slow-flow of all coronary systems in the absence of significant stenosis.4 Such dysfunction in the absence of significant stenosis have not been well documented.4 With increasing worldwide use of methamphetamine and its cardiotoxic effects, further research on the mechanism of methamphetamine induced vasospasm is warranted. Here, we present a rare case of methamphetamine induced coronary vasospasm.

Medications at time of admission included: albuterol inhaler as needed, aripiprazole 15 mg daily, aspirin 81 mg daily, cholecalciferol 2000 units daily, ferrous sulfate 325 mg daily, fluticasone-umeclidinium-vilanterol 100-62.5-25 mcg/puff daily, lisinopril 10 mg nightly, lorazepam 1 mg every eight hours as needed for anxiety or insomnia, metoprolol tartrate 25 mg twice daily, pantoprazole 40 mg before breakfast, and terbinafine 250 mg daily...Medications at time of admission included: adalimumab 40 mg every week, aripiprazole 15 mg daily, clonazepam 2 mg twice daily as needed for anxiety or panic, diclofenac 75 mg twice daily, doxepin 6 mg nightly, dulaglutide 0.75 mg every week, ergocalciferol 50,000 units twice a week, escitalopram 20 mg daily, fexofenadine 60 mg twice daily, fluticasone propionate/salmeterol inhaler (250-50 mcg/dose) twice daily, glipizide 10 mg daily, lisinopril 10 mg daily, omeprazole 20 mg daily, pramipexole 0.125 mg nightly, and terbinafine 250 mg daily...We compare our patients’ aripiprazole C/D ratios against populations who have undergone pharmacogenetic testing and other control groups in the absence of CYP 2D6 or 3A4 inhibiting medications. Through the use of therapeutic drug monitoring, pharmacogenetic data, other clinical data, and a tool to determine drug-drug interactions, we suggest terbinafine probably does not increase aripiprazole concentrations to a clinically relevant degree.

The overestimation of GFR in African Americans/Black patients, especially high-risk patients, can lead to major clinical consequences.

The availability and use of CGM in monitoring patient’s glycemic control helps guide appropriate therapy adjustments and is a major asset to both practitioner’s and patients.

At 12 weeks of observation, the total score decreased from 44.5±2.6 to 38.4±2.1 in group A; from 45±2.9 to 38±2.2 in group B; and from 50.9±3.2 to 42.7±2.8 in group C (р<0.05). The risk of acute MI and repeated hospitalization for CHF during the year of observation, as evaluated according to the Kaplan-Meier method, decreased in both bisoprolol and ivabradine combination treatment groups.Conclusion The inclusion of bisoprolol and ivabradine into the background therapy of CHF-PEF patients with stable IHD provided an improvement of QoL and a decrease in the risk of hospitalization for acute MI and CHF during the year of observation.

In the present study, 3D printed films and cylindrical polyprintlets were loaded with flexible, therapeutic dosages of two distinct drugs (amlodipine and lisinopril) across concentration ranges of 1-5% w/w and 2-10% w/w, respectively...X-ray powder diffraction (XRPD) and thermogravimetric analysis (TGA) showed that sintering partially transformed the phase of both drugs from the crystalline to amorphous forms. For the first time, we report a non-destructive, RTR quality control of two separate active ingredients in a single 3D printed drug product using NIR spectroscopy, overcoming a major barrier to the integration of 3D printing into clinical practice.

Case Description Our patient is a 55 years old African American lady with PMH of HTN,HIV on remission with Emtricitabine ,Tenofovir alafenamide , CKD with baseline creatinine 1.0-1.5 mg/dl...Patient started on Lisinopril then switched to Losartan to maxmum dose , proteinuria improved to 1.5 gram in February 2019 but increased again to 10.9 gram on November 2019,serum creatinine rose to 2.32 mg/dl...Acthar gel may improve proteinuria due to its anti-inflammatory properties similar to steroid and can be considered as steroid sparing agents in difficult situations such as underlying HIV or other immunosuppressive diseases like HCV, HBV, malignancy. Ue conclude that, combination of RAAS blockade and Acthar gel can be a reasonable treatment regimen in complex case of primary membranous GN with coexistent HIV.

Though the case is confounded by (1) chronic kidney disease and (2) hydroxychloroquine use, this case illustrates pure kidney failure in a patient with minimal pulmonary disease and suggests a renal specific disease due to SARS-CoV2.
