We present a case of transient dynamic PH in the setting of methamphetamine use, which caused severe hypoxic respiratory failure, and abated with cessation of amphetamines and aggressive control of hypertension. Methamphetamine is classified as a “likely” risk factor for PH.(1) Studies have demonstrated worse prognosis in patients with meth-PH compared to idiopathic PH.(2,3) PH should be considered as a differential when encountering respiratory failure in patients who use methamphetamines.

METHODS 65 year-old female with history of hypertension on lisinopril, CKD stage 3a with baseline creatinine of 1.16 mg/dl,alcohol use disorder,presented with abdominal pain,poor oral intake and vomiting for two weeks...RESULTS Kidneys’ ability to conserve water leads to urine supersaturation and carries an inherent predisposition to nephrolithogenesis,which can manifest as asymptomatic crystalluria,Randall’s plaque formation or nephrolithiasis.Calcium oxalate crystals,the most common constituent of kidney stones,are recognizable by their dumbbell and envelope shapes.Picket fence-shaped crystals may be seen in ethylene glycol poisoning.Extreme urinary supersaturation forming large X-shaped crystals have been described in in vitro studies [1] but is exceptionally rare in clinical setting.In our case, this is largely attributable to gastrointestinal loss and alcohol consumption causing severe volume depletion. CONCLUSION This case highlights the rare formation of large X-shaped calcium oxalate crystals in extreme urinary supersaturation, which has only been reported in in vitro studies.

CONCLUSION Nephrologists must be aware of the drug interactions between monoclonal antibodies and SGLT-2 inhibitors and the associated risk of serious infections.

Due to the severity of angioedema and the potential progression to airway compromise, it is crucial to closely monitor patients receiving tissue plasminogen activator.
