A 9 year-old male child with history of dextrocardia with situs inversus totalis, s/p repair for Tetralogy of Fallot, and developmental delay was transferred to our center for his annual ECHO under sedation as the child was unable to cooperate during his physical exam. In view of this incomplete knowledge of the child's current cardiac status and inability to obtain IV, he was premedicated with oral versed and taken to the operating room where he was induced with sevoflurane in slow and controlled manner to avoid cardiovascular instability.

We present the case of a 61-year-old male who underwent general anesthesia with sevoflurane for elective CABG surgery and developed acute liver failure and acute kidney injury postoperatively, which was initially suspected to be vasoplegic-induced ischemic or congestive hepatopathy, but these were excluded by further investigation Sevoflurane-induced hepatotoxicity was considered due to lack of other causes, timing of the liver injury, and the potential mechanism of immune and inflammatory response of sevoflurane in the liver. This case highlights the need for clinicians to recognize the possibility of rare but serious complications associated with sevoflurane use in patients undergoing general anesthesia.

Restoring this balance by pharmacologically enhancing excitatory neurotransmission and inhibiting the tonic current may represent an effective therapeutic option for perioperative neurocognitive disorders.

Sevoflurane, compared with propofol, increased anti-inflammatory biomarkers in monocytes, but not in alveolar macrophages, enhanced monocyte/alveolar macrophage phagocytic capacity and increased neutrophil migration in in vitro experimental sepsis. Both propofol and sevoflurane protected lung epithelial and endothelial cells.
