    In ascitic cirrhosis, reduced volaemia, secondary aldosteronism and adrenergic hyperfunction, especially when exacerbated by potassium-depleting diuretics (furosemide), contribute to tubular retention of water and dilutional hyponatremia. Low-dose sympatholytic agents are as effective as V 2 antagonists to achieve aquaretic effects in this setting.

    Acute kidney injury is a frequent complication after hip fracture surgery in elderly diabetic patients and is associated with increased 1-year mortality; however, it was not found to be associated with angiotensin-converting enzyme inhibitor/angiotensin receptor blocker pre-fracture treatment. Adding R-to-D to Gal-3 and BNP, a single pre-discharge strategy testing seemed to obtain a satisfactorily predictive value in alive HF patients discharged after an ADHF episode.

    TLS was diagnosed & treated with intravenous fluids for dilution, rasburicase for hyperuricemia, furosemide for renal dysfunction & minimal urine output, & insulin for hyperkalemia...Early recognition of TLS is key for treatment & involves awareness of the propensity for development of TLS in large tumor burdens, rapidly dividing tumors, multiple treatment sessions, & pretreatment renal dysfunction. Oliguria after IR treatment of HCC should raise the suspicion for possible TLS & not be easily dismissed as the common but less fatal contrast induced nephropathy.

    Two weeks prior, he had atrial fibrillation treated with ablation, apixaban, and propafenone...He was treated with IV diltiazem and furosemide with improvement of SOB...Unlike previous cases where jaundice, pruritis, and abdominal pain were presenting symptoms; hepatotoxicity was incidentally discovered in our patient. Although very rare, clinicians should be aware of propafenone-associated hepatotoxicity and routine monitoring of hepatic function may be warranted.

    ...Models are constructed for three variants of the SONHR group: primary benzene sulfonamide derivatives (e.g. diuretic drugs furosemide and hydrochlorothiazide), N-phenyl substituted 4-amino-N-phenylbenzenesulfonamide analogues (e.g. the sulfa antibiotic sulfadiazine) and phenylsulfonylureas (e.g. insulin secretagogue, glimepiride)...Using bond lengths as a powerful descriptor of ionization feasibility, we also identify that literature values for drug compounds celecoxib, glimepiride and glipizide are inaccurate...For glimepiride, our corrected value denotes a percentage of ionization at intracellular pH, which is only now in excellent agreement with its known therapeutic efficacy. We propose that linear relationships between bond lengths and pK should emerge for any set of congeners, thus providing a powerful method of pK prediction in instances where pK data exist for close congeners, thereby obviating the need for thermodynamic cycles.

    ...We investigated this issue using the NKCC1 inhibitors bumetanide and furosemide, which are commonly used loop diuretics...Immunofluorescence analyses of myofiber cross-sections revealed more large myofibers after exercise, but this was impaired by daily intraperitoneal bumetanide injections (0.2 or 10 mg/kg/day). NKCC1 plays an essential role in myogenesis and exercise-induced skeletal muscle hypertrophy, and sarcopenia in patients with renal or heart failure may be attributable to treatment with loop diuretics.

    It was revealed that an ototoxic regimen with kanamycin twice daily for 15 consecutive days is safe, well tolerated and produces an early significant hearing threshold shift detected by DPOAE in a small cohort of mice. The study compared ABR and DPOAE in mentioned regimens for the first time and illustrated that DPOAE is well suited for detecting hearing threshold shifts in high frequencies before ABR threshold shifts occur in accordance with predominating outer hair cell damage mainly in the basal turn of the cochlea.
