    Clinical, Journal:  Furosemide response predicts acute kidney injury in children after cardiac surgery. (Pubmed Central) -  Feb 25, 2020   
    Lower urine flow rate after furosemide administration, when evaluated in a heterogeneous cohort of children from multiple institutions after pediatric cardiac surgery, was independently associated with subsequent acute kidney injury and longer length of stay. Future prospective studies are needed to validate furosemide responsiveness as a predictor of acute kidney injury.

    HistoryA 25-year-old woman with recently diagnosed systemic lupus erythematosus and class IV lupus nephritis confirmed with biopsy and treated with mycophenolate mofetil presented with a 2-day history of progressively worsening edema of her face and lower extremities...She was admitted to the hospital and treated with methylprednisolone, furosemide, and C1 esterase inhibitor...Her laboratory study results preceding the seizure were remarkable for hyponatremia, with a blood sodium level of 122 mEq/L (122 mmol/L) (normal range, 135-145 mEq/L [134-145 mmol/L]), which was corrected to 137 mEq/L (137 mmol/L) over 48 hours. Same-day cerebrospinal fluid analysis was unremarkable, and unenhanced head CT findings (not shown) were normal, with no evidence of intracranial hemorrhage or edema.

    Both FST and RFR can help to improve diagnosis and care of high-risk patients with acute and chronic kidney disease. Sodium-glucose cotransporter 2 inhibitor DAPA predominantly decreased the ECW with a mild increase in urine volume, but the change in the ECW/total body water was smaller than that in patients treated with FR, and larger than that in patients treated with TLV, suggesting that the effects of SGLT2 inhibitors on fluid distribution may differ from those of conventional diuretics.

    To determine the prevalence of DR and its impact on survival among patients with decompensated HF, we prospectively evaluated the prevalence and influence on prognosis of DR (defined as persistent congestion despite ≥ 80 mg of furosemide per day) in a cohort of elderly patients from the Spanish HF registry (RICA) admitted for an acute decompensation of HF...The prevalence of DR in a cohort of elderly patients admitted for acute HF decompensation is 21%. DR is an independent predictor of 1-year mortality.

    However, using logistic regression, we identified no statistically significant predictors for hearing loss.     Unconventionally, Lasix was effective at reducing the edema thought to be secondary to his lymph node resection, which improved his therapy sessions...Conclusions : In the future, despite the added complications of penectomy, chemotherapy, radiotherapy, and lymph node dissections, patients may opt for continued treatment to improve their prognosis. If continued advances are made in penile cancer treatment to increase survivability, including clinical trials, it warrants better studies and guidelines within cancer rehabilitation management and intervention to ensure maximal gain of function and minimization of disability and handicap during those gained survival years.

    In conclusion, oral electrolyte products remain the gold standard for resuscitating diarrheic calves with moderate dehydration and acidemia and will likely perform better than small volumes of IV lactated Ringer's solution. Subcutaneous fluids by themselves are a poor treatment option and should be only be used as supportive therapy following the initial correction of hypovolemia and metabolic acidosis.

The triad of Coomb's positive hemolysis, cANCA vasculitis, and HLH-like reaction associated with a presumed BRSB is described for the first time in the literature and brings up questions for future research regarding the role of immune modulators and complement inhibitors in the treatment of severe loxoscelism as well as the genetic factors that predispose certain individuals to such reactions.

    After training trials consisting of exposure to two identical objects, rats were either treated with hypertonic saline to induce osmotic dehydration, furosemide to induce hypovolemic dehydration, or received a control injection and then object recognition memory was tested by presenting the original and a novel object...Follow up studies determined that loss of ovarian hormones after ovariectomy, but not loss of testicular hormones after castration, resulted in impaired memory performance in the object recognition test after hypovolemic dehydration. This series of experiments provides evidence for a protective role of ovarian hormones on dehydration-induced memory impairments.
