    [VIRTUAL] Primary Pulmonary Lymphangiectasia in an Otherwise Healthy 3-Year-Old Girl (ATS 2020 Virtual) -  Jul 6, 2020 - Abstract #ATSI2020ATS-I_5740;
    The working diagnosis was an atypical pneumonia and she was discharged on azithromycin...She had one similar exacerbation in the last six months which responded to oral furosemide and prevented the need for supplemental oxygen or non-invasive ventilation...There are no reports of PPL presenting after the newborn period. Therefore, PPL should be considered in otherwise healthy children with intermittent episodes of severe respiratory distress, hypoxemia, and crackles who are unresponsive to conventional treatment.

    [VIRTUAL] Naloxone-Induced Pulmonary Edema (ATS 2020 Virtual) -  Jul 6, 2020 - Abstract #ATSI2020ATS-I_5318;
    He was started on Non-invasive Positive Pressure Ventilation (NIPPV), and diuresed with Furosemide, with subsequent improvement of respiratory status...The adverse drug reaction occurred after Naloxone was given, as he had normal oxygenation with no evidence of respiratory distress prior to Naloxone administration. He had no history or physical exam findings suggestive of heart failure, or flash pulmonary edema secondary to malignant hypertension.Similar cases have been described in the literature, and were successfully treated with diuretics, and in some cases, additional opioids.This case demonstrates a very rare but serious side effect of Naloxone.

    [VIRTUAL] Acute Lung Toxicity by Nitrofurantoin (ATS 2020 Virtual) -  Jul 6, 2020 - Abstract #ATSI2020ATS-I_5310;
    Early diagnosis is vital but often difficult, as most patients will initially be treated for pneumonia, pulmonary embolism, heart failure exacerbation, and other pathologies. Delaying the correct diagnosis and initiating unrelated empiric treatments will increase morbidity and mortality.

    [VIRTUAL] Naloxone-Induced Acute Non Cardiogenic Pulmonary Edema Is It Dose Dependent (ATS 2020 Virtual) -  Jul 6, 2020 - Abstract #ATSI2020ATS-I_5301;
    Naloxone remains the optimal drug to reverse opioids overdose but can rarely cause non-cardiogenic acute pulmonary edema, which appears to be dose dependent. Healthcare providers should use the least effective dose of naloxone.

    [VIRTUAL] Breathing Bad: A Case of Malignant Transudative Pleural Effusion (ATS 2020 Virtual) -  Jul 6, 2020 - Abstract #ATSI2020ATS-I_5236;
    The patient was started on azithromycin, ceftazidime, prednisone, and nebulizers...She deferred thoracentesis initially and opted to try furosemide which did not improve her radiologic findings...The existing literature shows malignant transudative pleural effusions to be rare and thus cytology for cases of transudative effusion to be low yield. Despite having a history of diastolic dysfunction and fluid overload in the past, it is important to still perform cytology despite appearances in order to rule out malignancy.

    [VIRTUAL] Pleuro-Pericardial Effusion Leading to Pericardial Tamponade as First Presenting Sign in Chronic Myelo-Monocytic Leukemia (ATS 2020 Virtual) -  Jul 6, 2020 - Abstract #ATSI2020ATS-I_5214;
    Case presentation A 93-year-old man, prior smoker, with a history of emphysema presented with complaint of dyspnea, gradually progressive, seen at an outside facility, sent home with Furosemide for bilateral pleural effusion, symptoms didn't improve, felt more dyspneic even on minimal exertion...Underwent percutaneous drainage with a total of 200 ml drained, started on colchicine for possible pericarditis...The treatment includes symptom relief with hematopoietic stem cell transplantation, hydroxyurea, and hypomethylating agent. Timely identification and disease directed treatment would significantly reduce the morbidity and mortality associated with the disease and prevent future life-threatening complications.

    [VIRTUAL] Bosutinib-induced Pulmonary Arterial Hypertension in a Patient with Chronic Myelogenous Leukemia (ATS 2020 Virtual) -  Jul 6, 2020 - Abstract #ATSI2020ATS-I_4648;
    Though not a standard therapy for PAH, imatinib has been noted to improve pulmonary hemodynamics and exercise capacity in small studies. Given the the growing number of cases of both dasatinib and bosutinib induced PAH, regular screening for pulmonary toxicity for patients taking certain TKIs should be considered as previous review articles have suggested.

    [VIRTUAL] 'Broken Heart' from Nearly Drowning (ATS 2020 Virtual) -  Jul 6, 2020 - Abstract #ATSI2020ATS-I_4332;
    Patient was diuresed with 40mg of Lasix, but the auscultory findings and lower extremity edema persisted... We hypothesize that this stressful sympathetic-mediated event, as well as hypoxemia secondary to the submersion injury, could have induced myocardial injury resulting in takotsubo cardiomyopathy

    [VIRTUAL] An Interesting Case of Incidental Lung Mass (ATS 2020 Virtual) -  Jul 6, 2020 - Abstract #ATSI2020ATS-I_4321;
    The patient was started on Milrinone and furosemide with progressive improvement of his clinical condition and radiographic resolution of the left hilar opacity...Conservative treatment of the cause leads to the opacity resolution. Awareness of this vanishing tumor and its causal relation to congestive heart failure is important to avoid unnecessary diagnostic investigation and treatment

    [VIRTUAL] Management of Mixed Septic and Cardiogenic Shock in a Post-Partum Woman with Pre-Eclampsia and Profound Hypoxemia (ATS 2020 Virtual) -  Jul 6, 2020 - Abstract #ATSI2020ATS-I_4303;
    This case demonstrates the rapidity with which young, healthy women can improve from remarkable morbidity by early detection and treatment of the underlying disease alone. Although paralysis, Swan-Ganz catheter, ECMO, and/or mechanical support would have been reasonable interventions for the management of cardiogenic and septic shock, we were able to avoid them and their associated risks with aggressive diuresis and timely antibiotics alone.

    [VIRTUAL] A Flail Mitral Valve: A Case Report of an Acute Respiratory Distress Syndrome (ARDS)-Mimic (ATS 2020 Virtual) -  Jul 6, 2020 - Abstract #ATSI2020ATS-I_4294;
    Dobutamine and dopamine were added for refractory hypotension...She was started on nitroprusside and furosemide infusions...This case highlights the utility of point-of-care ultrasound as a diagnostic tool in the early management of patients with suspected ARDS. The use of bedside ultrasound rapidly changed this patient's trajectory and expedited her management in the hospital.

    [VIRTUAL] High Altitude Pulmonary Edema at Moderate Altitude in a Patient with Mitral Valve Prolapse/Regurgitation (ATS 2020 Virtual) -  Jul 6, 2020 - Abstract #ATSI2020ATS-I_4099;
    The patient received a single dose of furosemide, and rapidly improved...To our knowledge this is the only reported case of HAPE associated with MVP/MR at less than 2500 m. Sub-clinical changes in the pulmonary capillary bed related to the patient's regurgitant mitral flow may have pre-disposed the patient to HAPE, and impaired valve function probably delayed fluid clearance. This unique case demonstrates that MVP/MR may be a risk factor for HAPE at altitudes of less than 2,500 m.
  • ||||||||||  [VIRTUAL] Refractory Bronchospasm for Two Weeks in a Current Smoker (ATS 2020 Virtual) -  Jul 6, 2020 - Abstract #ATSI2020ATS-I_4028;
    On day 8, albuterol was reduced to every 2 hours, ketamine was stopped and furosemide and aformoterol were begun...By day 12, the patient improved on as-needed albuterol, with scheduled aformoterol, budesonide, azithromycin and methylprednisolone (latter twice-daily)...In true RB, the greatest benefit has been seen with volatile anesthetic gases, with most literature in the pediatric population. It can be logistically challenging to arrange for anesthetic gas administration in the ICU.

    [VIRTUAL] Tetrahydrocannabinol Vaping-Induced Acute Respiratory Distress Syndrome (ATS 2020 Virtual) -  Jul 6, 2020 - Abstract #ATSI2020ATS-I_3231;
    Initially, he was treated with intravenous furosemide for presumed pulmonary edema...Patient continued to improve on methylprednisolone only...Patient was discharged home on a taper of oral prednisone for 4 weeks...The pathogenies of vaping-induced lung injury is not well understood. Further investigation is warranted to determine the vaping agents responsible for lung injury and the role of adding THC to e-cigarettes fluids especially post-market and individually altered cartridges.

    [VIRTUAL] Unprovoked Rhabdomyolysis in Patient on Aripiprazole Therapy (ATS 2020 Virtual) -  Jul 6, 2020 - Abstract #ATSI2020ATS-I_3041;
    The patient was also on 10mg aripiprazole along with olanzapine 10mg daily...Patient was hence admitted to the medical ICU and managed with high-volume intravenous fluids and furosemide to maintain high urine output...And there should be instructions provided for recognition of its symptoms, especially in patients on multiple APs or during initiation of new medication or dose titration. The exact pathophysiology of rhabdomyolysis is unknown; however, in this patient, it was likely due to the new AP medication, as its withdrawal was associated with normalization of clinical and laboratory parameters.

    [VIRTUAL] Beef Liver Supplements: An Unusual Ingestion Leading to Hyperkalemic Cardiac Arrest (ATS 2020 Virtual) -  Jul 6, 2020 - Abstract #ATSI2020ATS-I_3038;
    She received calcium gluconate, insulin, dextrose, and furosemide...She reported taking her home medications as prescribed: pregabalin, buspirone, clonazepam, propranolol, mirtazapine and quetiapine...Dietary supplements are increasingly popular with 53% of Americans reporting use. The combination of widespread use and lack of oversight presents a challenge for physicians and places patients at risk for serious complications.

    [VIRTUAL] Seven Years of a Swollen Tongue: An Atypical Presentation of Hereditary Angioedema (ATS 2020 Virtual) -  Jul 6, 2020 - Abstract #ATSI2020ATS-I_2685;
    She was intubated and admitted to the Intensive Care Unit (ICU) for close observation and was treated with intravenous (IV) methylprednisolone, famotidine, and diphenhydramine...She was treated with varying combinations of as needed IV diphenhydramine, diazepam, furosemide, and corticosteroids, but continued to recur daily...The patient failed trials of Danazol and Icatibant but did have four days without symptoms after undergoing a trial of plasma infusions every 12 hours for 3 days...Management focuses on prevention of further attacks and early termination of new episodes while testing to determine the underlying etiology. Management was previously predominantly supportive, but as better understanding of the subtypes of HA and their underlying etiologies has been discovered, improved acute-stage treatments and some prophylactic management options have been developed such as C1 esterase inhibitors.

    Journal:  Divergent auditory-nerve encoding deficits between two common etiologies of sensorineural hearing loss. (Pubmed Central) -  Jun 29, 2020
    MHL in the present study was induced using furosemide, which provides a validated model of age-related MHL in young animals by reversibly inhibiting the endocochlear potential...Greater processing impairment with noise-induced compared to an age-related (metabolic) hearing-loss etiology may explain heightened speech-perception difficulties in people overexposed to loud environments. These results highlight the need for public policies to prevent noise-induced hearing loss - an entirely avoidable hearing-loss etiology - and for personalized strategies to diagnose and treat sensorineural hearing loss.

    [VIRTUAL] A Case of Pseudotumor Cerebri After Removal and Recurrence of ACTH-Producing Tumor () -  Jun 20, 2020 - Abstract #ENDOI2020ENDO-I_1266;
    The cause of IIH after treatment of cushing’s disease is believed to be mostly due to steroid withdrawal after surgical resection or medical treatment comprising hormonal control of cerebrospinal fluid production and absorption. In our patient we suppose that the persistent weight gain caused by the recurrence of her CD could also contribute to her IIH.
  • ||||||||||  [VIRTUAL] Management of 1,25-dihydroxyvitamin D (1,25-OHD) Mediated Hypercalcemia of Unknown Etiology () -  Jun 20, 2020 - Abstract #ENDOI2020ENDO-I_685;
    Hypercalcemia was initially managed with hyperhydration, furosemide and calcitonin...At that point, a trial of zoledronic acid was done with subsequent resolution of hypercalcemia...Various other treatment options were considered including a repeat dose of bisphosphonate, denosumab, ketoconazole and hydroxychloroquine, but since hypercalcemia was only mild, supportive measures were continued...Infectious workup and bone marrow biopsy were negative.     [VIRTUAL] Presumed Parathyroid Infarction Leading to Remission of Primary Hyperparathyroidism: A Rare Clinical Occurrence () -  Jun 20, 2020 - Abstract #ENDOI2020ENDO-I_650;
    Clinical Lesson: The differential diagnosis for a sudden remission of PTH-dependent hypercalcemia is limited. Parathyroid infarction is a rare condition with paucity of data regarding follow up, but these patients likely need to remain under close long term clinical and biochemical surveillance as recurrence has been documented in the literature.
