    Journal:  Quantum and Classical Evaluations of Carboxylic Acid Bioisosteres: From Capped Moieties to a Drug Molecule. (Pubmed Central) -  Jan 17, 2023
    For the first time, the AED was used to evaluate bioisosterism in an FDA-approved drug molecule, furosemide, and in five analogues of this medicine...The distinct power of the AED tool in quantitatively and precisely measuring the similarity among bioisosteric groups is contrasted with the relatively ambiguous bioisosteric evaluations through the classical qualitative electrostatic potential (ESP) maps. The ESP maps were demonstrated to fail, even qualitatively, in depicting the similarities, in some cases.

    Nephrotic Syndrome in Cambodian Children (Virtual) -  Jan 12, 2023 - Abstract #ISNWCN2023ISN_WCN_48;
    Albumin infusion was given in 26.09% of cases, Furosemide 32.61% and Lopril 13.04%...Conclusions In our study, the pattern of nephrotic syndrome and response to treatment was similar to other studies. The initial presentation, laboratory profile, and therapeutic response were consistent with typical childhood nephrotic syndrome.

    Journal:  Raising Awareness of Acute Kidney Injury: Unfolding the Truth. (Pubmed Central) -  Jan 11, 2023
    Considering the substantial impacts of AKI in ICU patients, early implementation of preventive measures should be an essential program which consists of developing AKI risk stratification in the ICU and encouraging the use of novel AKI biomarkers (TIMP-2, IGFBP-7, Cystatin C, IL-18,  KIM-1 and NGAL) as well as other risk stratification tools (clinical risk prediction scores, computer algorithms, furosemide stress test)...However, later reports suggested that AKI is actually prevalent in patients with COVID-19, particularly in ICU patients. AKI is now considered as a common complication of COVID-19 and it is also associated with adverse outcomes, including development or worsening of comorbidities, yet little is known about the pathogenesis or optimal management of COVID-19-associated AKI.

    Journal:  Initiating Continuous Renal Replacement Therapy in Patients With Transurethral Resection of Prostate Syndrome: A Case Report. (Pubmed Central) -  Jan 10, 2023
    CRRT has demonstrated efficacy in the treatment of chronic hyponatremia in patients with renal failure, and can slowly and continuously correct water-electrolyte imbalance, acid-base imbalance, and volume overload. TURP syndrome with severe hyponatremia and pulmonary edema was diagnosed; accordingly, the patient was treated with 3% hypertonic saline, furosemide, and CRRT, without the development of overt neurological sequelae.

    Many studies have tried to explain this phenomenon under different experimental conditions, but no detailed explanation of the yellowing phenomenon has been provided. This work, unlike the others, provides an overall view and explanation of the behavior of FUR in relation to the yellowing phenomenon, both in the solution and in solid state, considering several aspects, such as light exposure, presence of oxygen, and moisture effects.

    Cardiac magnetic resonance imaging and serological studies can work in conjunction to determine etiology of non-ischemic cardiomyopathy. Early recognition will facilitate prompt, disease-specific intervention leading to improved symptomatology.

    COMPLEX CORONARY ARTERY TO PULMONARY ARTERY ARTERIOVENOUS MALFORMATION IN THE ABSENCE OF RIGHT CORONARY ARTERY (Poster Hall_Hall F) -  Jan 4, 2023 - Abstract #ACCWCC2023ACC_WCC_5547;
    This case highlights a rare form of congenital coronary artery fistula involving both LAD and LCx draining into PA and RV with the absence of RCA. In addition, medical management with goal directed medical therapy can improve outcomes in these patients.

    PREVENTION OF CONTRAST-INDUCED NEPHROPATHY WITH THE RENALGUARD SYSTEM: A META-ANALYSIS (Poster Hall_Hall F) -  Jan 4, 2023 - Abstract #ACCWCC2023ACC_WCC_2909;
    Background: The RenalGuard System is a device that administers matched IV normal saline with furosemide-induced diuresis with the aim of decreasing the risk of contrast-induced nephropathy (CIN) in patients undergoing cardiac procedures... RenalGuard can reduce the risk of CIN in patients undergoing cardiovascular procedures when compared to other strategies of volume expansion; however, the results of this study do not demonstrate significant reduction in the risk of RRT, in-hospital mortality, or 30-day MACE for these patients.

    MINOXIDIL INDUCED PLEURO-PERICARDIAL EFFUSION IN END-STAGE RENAL DISEASE (Poster Hall_Hall F) -  Jan 4, 2023 - Abstract #ACCWCC2023ACC_WCC_2399;
    Cardiac tamponade is a life-threatening condition that requires prompt management of the condition and the underlying illness. Minoxidil is one of the medications that has been implicated in causing pleuro-pericardial effusion, particularly among patients with ESRD.

    CONTINUOUS INFUSION VS BOLUS OF LOOP DIURETICS IN CARDIOGENIC SHOCK (Poster Hall_Hall F) -  Jan 4, 2023 - Abstract #ACCWCC2023ACC_WCC_1141;
    These findings may represent a quality improvement opportunity in the care of patients hospitalized for ADHF. Use of Cont-LD in CS is associated with a greater negative fluid balance and diuretic efficacy which may contribute to improve cardiac performance and reduce end-organ complications associated with CS (less AKI and shorter mechanical ventilation).

    OPTIMAL INITIAL DIURETIC DOSE IN ACUTE DECOMPENSATED HEART FAILURE USING A HOME DOSE MULTIPLIER (eAbstract Site) -  Jan 4, 2023 - Abstract #ACCWCC2023ACC_WCC_864;
    At a HDM between 2.5 and 3.9, there appears to be a positive effect of increasing weight loss (P = 0.039) without the deleterious effects of creatinine change (P = 0.87) on the primary composite outcome. As a result, a diuretic dose between 2.5 to 3.9 times a patient's home diuretic dose may be the optimal initial hospital dose in patients taking under the equivalent of 80 mg of furosemide at home.

    Preclinical, Journal:  The loop diuretic torasemide but not azosemide potentiates the anti-seizure and disease-modifying effects of midazolam in a rat model of birth asphyxia. (Pubmed Central) -  Jan 1, 2023
    Loop diuretics such as furosemide and bumetanide, which act by inhibiting the Na-K-2Cl cotransporter NKCC2 at the thick ascending limb of the loop of Henle, have been shown to exert anti-seizure effects...Because of the urgent medical need to develop new treatments for neonatal seizures and their adverse outcome, we evaluated the effects of azosemide and torasemide, administered alone or in combination with phenobarbital or midazolam, in a rat model of birth asphyxia and neonatal seizures...We assume that either an effect on NKCC1 at the blood-brain barrier and/or cells in the periphery or the NKCC2-mediated diuretic effect of torasemide are involved in the present findings. Our data suggest that torasemide may be a useful option for improving the treatment of neonatal seizures and their adverse outcome.
  • ||||||||||  eplerenone / Generic mfg.
    Journal:  Effect of eplerenone on clinical stability of Japanese patients with acute heart failure. (Pubmed Central) -  Jan 1, 2023
    Our data suggest that torasemide may be a useful option for improving the treatment of neonatal seizures and their adverse outcome. Eplerenone treatment improved the clinical stability particularly during short period following hospitalization for AHF, translated by lower diuretic doses, natriuretic peptide levels, indirect markers of filling pressure and venous congestion, and a smaller LV volume.
