Clinical, Journal: Efficacy of a local anesthetic gel infusion kit for pain relief after minimally invasive colorectal surgery: an open-label, randomized clinical trial. (Pubmed Central) - Oct 21, 2022 This study compared the effectiveness of postoperative pain relief from the application of a local poloxamer 407-based ropivacaine hydrogel (Gel) to the incision site with continuous infusion-type ropivacaine administration (On-Q) in patients undergoing laparoscopic colorectal surgery...The outcome measures, which were assessed for 72 h after surgery, included the total amount of fentanyl consumed via IV-PCA (primary endpoint), and the amount of rescue analgesia (pethidine) and postoperative pain intensity assessed using a numeric rating scale (NRS) [secondary endpoints]...There was also no significant difference in the frequency of analgesic rescue medication use (P = 0.213) or NRS scores (postoperative 6 h, P = 0.860; 24 h, P = 0.333; 48 h, P = 0.168; and 72 h, P = 0.655) between the two groups. The Gel, which continuously delivers a local anesthetic to operative sites, can thus be considered an effective device for analgesia and pain relief for midline incisions in laparoscopic colorectal surgery.

Review, Journal: Safe sedation during diagnostic and therapeutic flexible bronchoscopy in Japan: A review of the literature. (Pubmed Central) - Oct 13, 2022 Emphasis has been placed on safety aspects, such as patient monitoring, the precautions that should be taken in patients with certain conditions, prevention and management of complications, adequate staffing, and optimal sedation and disinfection. In conclusion a combined opioid (fentanyl or pethidine) and midazolam sedation is optimal for diagnostic and therapeutic flexible bronchoscopy in Japanese patients.

Journal: The Pain Control Efficacy of Zolpidem versus Melatonin after Intervertebral Disc Herniation Surgery under General Anaesthesia: A Randomised Clinical Trial. (Pubmed Central) - Oct 11, 2022 Data including (i) pain (visual analog scale) and sedation (Ramsay sedation scale) scores during recovery and at all five initial 24-h time points (two, four, six, 12, 24); (ii) adverse events inclusive of mild nausea and dizziness, pethidine consumption; and (iii) ongoing haemodynamic parameters, including heart rate, blood pressure, and oxygen saturation were collected...The findings taken together lent a strong support for the highly encouraging efficacy of both drugs in affording adequate analgesia at 24 postoperative hours without any adverse events needing to be thought of. Therefore, both zolpidem and melatonin were promising postoperative pain relievers, while no drug is demonstrably superior to the other.

Our study findings contribute to the increasing national and international evidence that initiatives such as Labour Hopscotch can promote and advocate for women to be active and mobile during labor to support physiological birth.

Retrospective data, Review, Journal: The efficacy of topical sucralfate in improving pain and wound healing after haemorrhoidectomy procedure: A systematic review, meta-analysis, and meta-regression of randomised clinical trials. (Pubmed Central) - Jul 24, 2022 Mean Difference -1.64 (95% CI -2.38, -0.91), P < .0001], and wound healing rate at 28-day post-operative [RR 1.45 (95% CI 1.25-1.68), P < .00001]. Topical sucralfate alleviated pain, improved wound healing, and minimised the usage of pethidine and diclofenac compared to placebo.

With this report we aim to alert health professionals to this rare syndrome and to important implications in the approach to these patients, being possible to undergo invasive procedures with caution to patient’s monitorization and to choose the safest possible drugs to administer.

Comparison of the efficiency of ketamine and propofol alone and in combination for prevention of post spinal shivering in elderly (Virtual Platform - Poster section) - Jun 17, 2022 - Abstract #Euroanaesthesia2022Euroanaesthesia_493; Concerning the shivering score we found significant lower grading in all study groups versus placebo (p<0,01). Group KP proved its superiority by significantly lower severity scores (p<0,05) compared to each of the groups K and P, which were similar in this respect, as well.The incidence of sedation, the occurrence of nausea/vomiting and hemodynamic parameters registered similar values in all study groups.Conclusion(s): The combination of low doses of ketamine and propofol as intravenous bolus proved a good safety profile and a superior efficiency compared to ketamine and propofol as single agents in preventing post-spinal shivering in elderly undergoing lower abdominal surgery.

Obstetric anaesthetic management of a patient with a peroxisome biogenesis disorders (PEX10) (Poster eBoard 4) - Jun 17, 2022 - Abstract #Euroanaesthesia2022Euroanaesthesia_129; This is the first case report outlining obstetric anaesthesia management of a patient with PEX-10, where unpredictability response to treatment was the main concern. The case was complicated by chorioamnionitis but the patient was successfully managed during the perioperative period.This case demonstrates the prolonged response to drugs used in the peripartum period in a pregnant women with PEX10 mutation.

Retrospective data, Review, Journal: Different interventions for preventing postoperative catheter-related bladder discomfort: a systematic review and meta-analysis. (Pubmed Central) - May 18, 2022 This review suggests that dexmedetomidine, gabapentin, tolterodine, tramadol, ketamine, nefopam, oxybutynin, pregabalin, and PNB are effective in preventing postoperative CRBD. Considering the efficacy and adverse effects of all drugs, dexmedetomidine and gabapentin were ranked best.

(Pubmed Central) - Apr 19, 2022 Parturients were asked to choose among four hypothetical forms of labor analgesia: epidural analgesia, pethidine, Entonox and no analgesia, which were defined by: pain score, duration of second stage of labor, risks of instrumental delivery, back pain and permanent nerve injury, and out-of-pocket cost...Parturients prioritized having lower pain and shorter labor experience over risks associated with epidural analgesia. Parturients should be presented with realistic range of risks of side-effects so that they can decide how to balance risks against benefits and costs associated with child labor.

Journal: Efficacy of superficial cervical plexus block versus cervical retrolaminar block both combined with auriculotemporal nerve block in parotid surgeries. (Pubmed Central) - Apr 8, 2022 A total of 40 patients undergoing parotid surgery were prospectively randomized into either the SCPB group (n = 20) or the cervical RLB group (n = 20) using 20 ml of 0.25% bupivacaine plus 5 mcgmL epinephrine...Total intra operative fentanyl consumption and post-operative pethidine consumption in the first 24h were lower in group RLB...There was no significant difference in side effects except for 20% Horner's syndrome in the SCPB group. Cervical RLB is more effective analgesic technique than SCPB, as the cervical RLB showed longer time to first analgesic request, lower intraoperative anesthetic consumption, lower total post-operative pethidine consumption and lower VAS.
