    Amlodipine-induced gingival hypertrophy, a less known side effect to dermatologists () -  Jul 3, 2023 - Abstract #WCD2023WCD_6122;
    BACKGROUND: Multiple factors may lead to Gingival hypertrophy (GH), including taking certain drugs (Phenytoin, Ciclosporin) but is uncommon for calcium channel blockers (CCB)...OBSERVATIONS: A 73-year-old man followed for basal cell carcinoma of the zygomatic region, touching the facial nerve, is in complete remission after partial excision and Vismodegib...We advise on replacing the CCB by Irbesartan / Hydrochlorothiazide and dental care...The prevalence of GH on calcium channel blocker varies according to the type of CCB, Amlodipine being estimated between 1.1 and 3.3% compared to 20% for Nifedipine...The earliest possible detection of GH is desirable. CCBs must therefore be known to dermatologists as being responsible for GH

    Journal:  Characterization and Validation of a New 3D Printing Ink for Reducing Therapeutic Gap in Pediatrics through Individualized Medicines. (Pubmed Central) -  Jun 28, 2023
    The print cartridges were stable for up to 72 h after processing, and orodispersible printlets containing 6 mg to 24 mg of hydrochlorothiazide can be produced using the same print cartridge and during the same printing process with guaranteed content and chemical stability. The proposed workflow for the development of new printing inks containing APIs will allow the optimization of feedstock material and human resources in pharmacy or hospital pharmacy services, thus speeding up their development and reducing costs.

    Journal:  Case report: Gitelman syndrome with diabetes: Confirmed by both hydrochlorothiazide test and genetic testing. (Pubmed Central) -  Jun 20, 2023
    GS patients often have abnormal glucose metabolism, which is mainly caused by hypokalemia, hypomagnesemia, and secondary activation of RAAS. When a patient is diagnosed with GS and type 2 diabetes, sodium-glucose cotransporter 2 inhibitors (SGLT2i) can be used to control the blood glucose level and assist in raising blood magnesium.

    Although we found few differences in the cost of hospitalization when comparing drugs with similar indications and the same adverse reaction, there were specific drug-ADR pairs that merit attention and consideration of interventions to improve safe and appropriate medication use. Evaluation of the effect of those interventions on the incidence of ADRs is an area for future study.

    Pheochromocytoma - A Silent Killer (ENDOExpo) -  May 11, 2023 - Abstract #ENDO2023ENDO_1956;
    Interestingly, prior to admission, patient's BP was not controlled (ranges 140s-170s/60s-90s) even on three antihypertensives (Hydrochlorothiazide (HCTZ), Valsartan, & Amlodipine)...On discharge he was maintained on Prazosin, valsartan, and amlodipine for hypertension and Lantus 28-unit qAM along with Aspart 4-unit premeal for diabetes. This case highlights the importance of familiarity with the different presentations of pheochromocytoma (i.e. uncontrolled and challenging hyperglycemia, persistent uncontrolled hypertension, without any obvious physical signs) to healthcare providers as it can be mistreated if left undiagnosed.

    Cystic Pheochromocytoma (ENDOExpo) -  May 11, 2023 - Abstract #ENDO2023ENDO_1955;
    Uniform guidelines for surgical management of these lesions have not been well established. Patients with symptoms such as abdominal pain, biochemical abnormalities, complications such as pressure on adjacent organs or rupture with cyst content leakage, suspicion of malignancy, cysts larger than 5 cm or increasing size of the cyst should be considered for surgical intervention (5, 6).In patients presenting with abdominal pain and hypertension who are found to have a cystic adrenal lesion pheochromocytoma should be considered.

    Treating Pheochromocytoma for Glycemic and Blood Pressure Control (ENDOExpo) -  May 11, 2023 - Abstract #ENDO2023ENDO_1933;
    Her medical regimen consisted of linagliptin 5mg daily, metformin 1000mg twice daily, insulin glargine 40 units daily, losartan 50mg twice daily, amlodipine 10mg daily and hydrochlorothiazide 25mg daily...Additionally, UACR improved to 1352.94 mg/g and empagliflozin was added to avoid diabetic kidney progression...Detection of resistant HTN should had prompted earlier detection of pheochromocytoma in this patient. As physicians, we should be aware of endocrine causes of diabetes and HTN, such as Cushing syndrome, Acromegaly, Primary Aldosteronism, and Pheochromocytoma and Paraganglioma, as treating these may lead to better quality of life, and less disease-related complications and polypharmacy.

    A Case of Nephrotic Syndrome Presenting With Hyperlipidemia. (ENDOExpo) -  May 11, 2023 - Abstract #ENDO2023ENDO_1897;
    The abnormalities of serum lipids seen in nephrotic syndrome are largely due to the impaired clearance rather than biosynthesis and can lead to multiple complications. Making the diagnosis of nephrotic syndrome is very important as it is associated with significant cardiovascular and renal disease.

    Complicated Course and Management after Denosumab Discontinuation: A Case Report (ENDOExpo) -  May 11, 2023 - Abstract #ENDO2023ENDO_1831;
    Past bone health history was notable for treatment with alendronate for approximately 11 years as a premenopausal woman...Labs were only notable for hypercalciuria for which hydrochlorothiazide (HCTZ) was initiated...Thus in 1/2022, she started romosozumab (ROMO) in an attempt to prevent new VFx...In 5/2022, she received a dose of zoledronic acid (ZOL)...In addition, this report does not support transitioning from long-term DMAb to ROMO in order to prevent a rapid increase in the bone resorption marker, CTX. Rather, treatment with ZOL every 12 weeks based on frequent CTX monitoring successfully maintained BMD stable following long-term DMAb discontinuation.
  • ||||||||||  A Case of Pheochromocytoma Initially Presenting as Takotsubo-like Cardiomyopathy (ENDOExpo) -  May 11, 2023 - Abstract #ENDO2023ENDO_823;
    Background: Takotsubo-like cardiomyopathy (TCM) is a known but rarely seen clinical complication associated with pheochromocytoma that is important to recognize.Case Presentation: A 68 year old woman with a history of hypertension on verapamil, lisinopril-hydrochlorothiazide, clonidine, hyperlipidemia, asthma, and chronic kidney disease presents for nausea and vomiting associated with pressure like chest pain...She was discharged home on beta blocker and losartan therapy...Alpha blocker tamsulosin was initiated and beta blocker therapy was discontinued...Earlier recognition of a potential pheochromocytoma would also have discouraged the use of a beta blocker initially until pheochromocytoma was ruled out. An individualized, multidisciplinary approach helps lead to a timely diagnosis and appropriate treatment decisions for these rare tumors.

    COVID-19 Pneumonia and Primary Hyperparathyroidism Exacerbation (ENDOExpo) -  May 11, 2023 - Abstract #ENDO2023ENDO_737;
    However, a significant rise in Ca approaching parathyroid crisis range was seen shortly thereafter, despite normalization of Cr.Whereas hypocalcemia has been noted in most cases of Ca dysfunction associated with COVID-19 infection, in this case, we postulate that the COVID-19 infection may have triggered parathyroiditis, thus exacerbating the underlying PHPT.One such mechanism is the use of receptors like angiotensin-converting enzyme 2 (ACE2) and transmembrane protease serine 2 (TMPRSS2) by COVID-19 virus, which are found on endocrine glands including the parathyroid. Similarly, COVID-19 virus exerts its inflammatory effects using IL-6, which is also a key cytokine that causes hypercalcemia by stimulating bone resorption.Furthermore, COVID-19 infection is notorious for causing nausea/vomiting, and diarrhea, culminating in dehydration which worsens hypercalcemia.It is therefore necessary that physicians foresee impending complications of COVID-19 infection with concurrent parathyroid disease and take early steps to prevent and treat them.

    Journal:  Angiotensin II-Related Activation of Scleral Fibroblasts and Their Role on Retinal Ganglion Cell Death in Glaucoma. (Pubmed Central) -  Apr 28, 2023
    A significant increase in the number of RGCs and decrease in glial cell activation was found in the retina after the losartan treatment. These findings suggest that AngII plays a role in scleral fibrosis after systemic hypotension and that inhibiting AngII could modulate the tissue properties of the sclera, resulting in the protection of RGCs.
