    Clinical, Journal:  Benzodiazepines vs barbiturates for alcohol withdrawal: Analysis of 3 different treatment protocols. (Pubmed Central) -  Dec 21, 2019   
    During benzodiazepine shortages, phenobarbital is a safe and effective treatment alternative for alcohol withdrawal. Incorporating phenobarbital into a benzodiazepine based protocol or as sole agent led to similar rates of ICU admission, length of stay, and need for mechanical ventilation in patients treated for alcohol withdrawal in the emergency department.

    Although the definitive treatment of cannabinoid hyperemesis syndrome is discontinuing the causative agent, medical management that includes rehydration is important to prevent complications. Common antiemetic medications are ineffective, but some studies have shown haloperidol and lorazepam to be effective in treating acute symptoms.

    The patient ultimately responded to quetiapine with addition of lorazepam following seizure provocation.Case 2: Patient is a 24-year-old right-handed woman with a learning disability, adjustment disorder with anxiety, methamphetamine use disorder in early remission, and right temporal lobe epilepsy in the setting of periventricular nodular heterotopia...She required physical restraint, haldol and lorazepam IM...The patient’s mood and agitation improved with scheduled risperidone and resumption of her antiseizure medications including gabapentin and clobazam...Cannabis withdrawal with added acute stress of brain surgery and seizure provocation should be taken very seriously as the consequences may be life threatening. We recommend that the presurgical process for epilepsy surgery involves a designation of a medical power of attorney, extensive patient education surrounding the hospitalization, and period of abstinence from cannabis, confirmed by urine toxicology.

    Intranasal midazolam and intravenous lorazepam were specified as first line benzodiazepines (BZD), second line medication options were expanded (see Figure 1), and midazolam was recommended as the initial third line continuous infusion...Considering all second line medications given (with and without SE), more valproic acid (p=0.0001) and less levetiracetam (p=0.008) were provided post-pathway, but this was not associated with reduced frequency of fosphenytoin and phenobarbital dosing (p=0.957)...While different second line medications were prescribed post-pathway, the overall incidence of requiring one and two doses of second-line medication was unchanged. Further analyses will evaluate whether these changes were associated with changes in patient outcome.

The rapid and more gradual cessation of AED precipitates similar rates of SC and SE.

    Results 157 (59.5%) initiated levetiracetam (LEV), 49 (18.6%) lamotrigine (LTG), 21 (8%) oxcarbazepine (OXC), 11 (4.2%) phenytoin, 5 (1.9%) topiramate, 12 (4.5%) carbamazepine, (CBZ) 2 (0.8%) lacosamide, 5 (1.9%) valproic acid, 1 (0.4%) pregabalin, and 1 (0.4%) zonisamide.Of the 157 started on LEV, 63 (31.6%) remained on the drug in monotherapy, 76 (48%) switched to another AED, 12 (7.6%) added an AED in conjunction, and 6 (3.8%) ceased AED usage...By the end of the study, pts were receiving the following AEDs alone or in combination: LEV 36.0%, LTG 37.5%, CBZ 6.8%, OXC 17.0%, PHT 0.8%, topiramate 2.3%, lacosamide 7.2%, valproic acid 1.9%, zonisamide 4.9%, lorazepam 0.8%, clobazam 0.4%, clonazepam 0.4%...The likelihood of remaining on initial monotherapy was twice as high for a sodium channel blocker as for LEV in the first 2 years of treatment. Unlike alcohol withdrawal seizures, SESA syndrome typically requires chronic treatment with AEDs as recurrence is common.

    Patient’s seizure medications were held, and on the third night, he was noted to have a cluster of seven nocturnal seizures prompting IV diazepam use for a total of 15mg.The patient continued to have seizures requiring additional BZDs, total of 12mg of IV lorazepam and 4mg of IV midazolam over the course of 7 – 8 hours...His ASDs were continued at home doses (brivaracetam 100mg TID, carbamazepine 600-400-400, and perampanel 18mg QHS)...At that point his clobazam was stopped and switched to zonisamide and his seizure frequency decreased to 0-1 per night...Conclustions Paradoxical worsening of frontal lobe seizures with benzodiazepine usage can occur. BZDs still remain the first line for acute seizure treatment, however additional precautions should be taken when used for sleep activated frontal lobe seizures.
