Ixazomib is an oral proteasome inhibitor used in the treatment of multiple myeloma...Use of ixazomib is increasing, as are rates of multiple myeloma. It is therefore useful for dermatologists to recognize this potential effect and be aware that conservative management with topical steroids, temporary cessation of treatment or dose reduction may be adequate to alleviate the rash and allow continuation of ixazomib treatment.

Thereafter, treatment with quetiapine, lithium and venlafaxine was discontinued and treatment with intravenous fluids, diazepam and dantrolene was initiated...He was then medicated with lorazepam 2.5 mg three times daily, and after 3 days of continued improvement, lithium was reintroduced (800 mg/day)...He was ultimately transferred to a Psychiatric ward where venlafaxine 150 mg was reintroduced and risperidone was later initiated (slowly titled until 2 mg per day), with an improvement of the depressive and psychotic symptoms, without the development of NMS signs during the internment and on the next months of follow up...Moreover, NMS as already been associated with other drugs, namely mood stabilizers and antidepressants, and that´s why lithium and venlafaxine were also initially discontinued. This case highlights that one should always be watchful to the signs of NMS, even in patients taking antipsychotics rarely associated with this syndrome.

We did not change the patient's antipsychotic treatment, which was haloperidol 10 mg/day...This finding gradually disappeared after the dose of lorazepam had been lowered...Epileptic seizures, suspected neuroleptic malignant syndrome, magnetic resonance imaging and EEG abnormalities should be red flags for psychiatrists [1]. Autoimmune aetiologies should be considered when the clinicians notice these findings and symptoms and advanced tests must be performed.

Implementation of an institutional guideline and order set for alcohol withdrawal incorporating high dose gabapentin tapers was associated with a decreased median daily lorazepam use as well as hospital LOS; however, retrospective design and inherent limitations require larger prospective trials to validate results.
