    His pertinent medications upon admission included haloperidol decanoate long-acting injectable 300 mg intramuscularly every 4 weeks for schizoaffective disorder, divalproex sustained release 1000 mg by mouth twice daily for mood, glycopyrrolate 1 mg by mouth daily for secretions, memantine 5 mg by mouth twice daily for memory, olanzapine 15 mg by mouth twice daily for thought clarity, haloperidol 5 mg by mouth twice daily as needed for agitation, hydroxyzine 25 mg by mouth three times daily as needed for anxiety, and trazodone 50 mg by mouth at bedtime as needed for sleep...There were multiple attempts to control his behaviors, including administering a combination of intramuscular haloperidol 5 mg, lorazepam 1 mg, and diphenhydramine 50mg, but his behaviors continued...This case supports the benefits of using naltrexone for impulse behaviors. Although naltrexone is FDA approved only for alcohol and opioid dependence, it may be useful for veterans who exhibit impulse behaviors that cause distress to the veteran, their family, their caregivers, or clinicians.

Of the medication classes reviewed, atypical antipsychotics have the best evidence for effectiveness, however, the overall magnitude of treatment effect is modest and must be balanced with risk of serious adverse events including death.

The results of a large double blind study also including valproate are also imminent, and together likely to change practice in benzodiazepine-resistant SE.
