Retrospective data, Journal: Lorazepam timing for acute convulsive seizure control (LoTASC). (Pubmed Central) - Jan 5, 2021 Our study suggests that lorazepam administration for acute convulsive seizures not meeting convulsive status epilepticus criteria may lead to iatrogenic vital instability and need for ICU admission.

Journal: Incidence of and Characteristics Associated With Long-term Benzodiazepine Use in Finland. (Pubmed Central) - Jan 5, 2021 The findings of this population-based cohort study conducted in Finland suggest that the incidence of subsequent long-term BZDR use in individuals who initiate use of BZDRs is high, especially among older persons, and that the specific BZDR used initially is associated with the development of long-term BZDR use and should be carefully considered when prescribing BZDRs. The observed factors that appear to be associated with development of long-term BZDR use also should be considered in clinical decision-making when starting and monitoring BZDR treatment.

[VIRTUAL] Medication use evaluation of dexmedetomidine in critically ill patients with alcohol withdrawal symptoms at a community teaching hospital () - Dec 26, 2020 - Abstract #ASHP2020ASHP_4120; Purpose: Dexmedetomidine is an intravenous, centrally-acting alpha-2 adrenergic agonist currently recommended by the American Society of Addiction Medicine guidelines as an adjunct to benzodiazepine therapy in the critically ill patient population to control autonomic hyperactivity and anxiety or for resistant alcohol withdrawal...Appropriate initiation of dexmedetomidine will be defined with the following criteria: continued presence of alcohol withdrawal symptoms as evidenced by a Clinical Institute Withdrawal Assessment for Alcohol (CIWA-Ar) score greater than or equal to 15 despite total benzodiazepine equivalent use of greater than 10 mg lorazepam equivalents in 1 hour or greater than 40 mg lorazepam equivalents in 4 hours and continued use of benzodiazepine symptom-triggered therapy as needed per provider-specified CIWA-Ar score criteria...Reviewers will determine if dexmedetomidine initiation was appropriate for each patient using pre-specified criteria. Univariate descriptive statistics will be performed to describe the patient population and the outcomes identified in this protocol.

[VIRTUAL] Potential drug-drug interaction between terbinafine and aripiprazole: two cases () - Dec 26, 2020 - Abstract #ASHP2020ASHP_2896; Medications at time of admission included: albuterol inhaler as needed, aripiprazole 15 mg daily, aspirin 81 mg daily, cholecalciferol 2000 units daily, ferrous sulfate 325 mg daily, fluticasone-umeclidinium-vilanterol 100-62.5-25 mcg/puff daily, lisinopril 10 mg nightly, lorazepam 1 mg every eight hours as needed for anxiety or insomnia, metoprolol tartrate 25 mg twice daily, pantoprazole 40 mg before breakfast, and terbinafine 250 mg daily...Medications at time of admission included: adalimumab 40 mg every week, aripiprazole 15 mg daily, clonazepam 2 mg twice daily as needed for anxiety or panic, diclofenac 75 mg twice daily, doxepin 6 mg nightly, dulaglutide 0.75 mg every week, ergocalciferol 50,000 units twice a week, escitalopram 20 mg daily, fexofenadine 60 mg twice daily, fluticasone propionate/salmeterol inhaler (250-50 mcg/dose) twice daily, glipizide 10 mg daily, lisinopril 10 mg daily, omeprazole 20 mg daily, pramipexole 0.125 mg nightly, and terbinafine 250 mg daily...We compare our patients’ aripiprazole C/D ratios against populations who have undergone pharmacogenetic testing and other control groups in the absence of CYP 2D6 or 3A4 inhibiting medications. Through the use of therapeutic drug monitoring, pharmacogenetic data, other clinical data, and a tool to determine drug-drug interactions, we suggest terbinafine probably does not increase aripiprazole concentrations to a clinically relevant degree.

Recognition of these symptoms is critical to initiating appropriate therapy.
