[VIRTUAL] A RARE CASE OF RADIATION-PRIMED CRYPTOGENIC ORGANIZING PNEUMONIA () - Oct 13, 2021 - Abstract #CHEST2021CHEST_2222; A high index of suspicion is required in order to diagnose radiation-primed cryptogenic organizing pneumonia. Clinicians must be aware of its chronic and persistent presentation if left untreated, as early diagnosis will prevent significant physical and psychological morbidity.

[VIRTUAL] INTRALOBAR BRONCHOGENIC PULMONARY SEQUESTRATION DIAGNOSED INTRA-OPERATIVELY () - Oct 13, 2021 - Abstract #CHEST2021CHEST_1836; Interestingly, a magnetic resonance imaging (MRI) study of the chest obtained at the time of the patient's original pneumonia 25 years ago did not reveal anomalous or aberrant vasculature, so the diagnosis of BPS was questioned. Ideally, the diagnosis should be made pre-operatively, as an uncommon intra-operative complication is pulmonary hemorrhage due to unrecognized systemic arterial supply.

[VIRTUAL] INFECTED MEDIASTINAL MATURE TERATOMA () - Oct 13, 2021 - Abstract #CHEST2021CHEST_1648; Teratoma rarely presents with superimposed infection, which may be complicated by rupture. Definitive management is surgical excision of teratoma and antibiotic targeting the organism.

[VIRTUAL] MANAGEMENT OF PULMONARY ABSCESS: FIBRINOLYTICS AND DRAINAGE? () - Oct 13, 2021 - Abstract #CHEST2021CHEST_1424; The chest tube was removed, and she was then discharged on PO Amoxicillin/Clavulanic acid for four weeks... The role and safety of fibrinolytic therapy in pulmonary abscess needs to be evaluated and potential surgery may be avoided in select cases.

His immunosuppressive regimen included tacrolimus, mycophenolate, and prednisone...He clinically improved after treatment with broad-spectrum antibiotics and was ultimately discharged on an extended course of amoxicillin clavulanate (later transitioned to trimethoprim-sulfamethoxazole)... Diagnosis of pulmonary nocardiosis in an immunocompromised patient is challenging given the wide range of clinical and radiographic presentations.

Although these events are rare, they should be on our radar in clinical practice.

Journal: Prevalence, Antimicrobial Susceptibility, and Molecular Characterization of Escherichia coli Isolated From Raw Milk in Dairy Herds in Northern China. (Pubmed Central) - Oct 12, 2021 All of the E. coli isolates were susceptible to gentamicin...Nine non-pathogenic E. coli isolates also carried β-lactamase resistance genes, which may transfer to other pathogenic E. coli and pose a threat to the farm's mastitis management projects. Our results showed that most of E. coli were multidrug resistant and possessed multiple virulence genes, which may have a huge potential hazard with public health, and antibiotic resistance of E. coli was prevalent in dairy herds in Northern China, and ampicillin should be used cautiously for mastitis caused by E. coli in Northern China.

Clinical, Journal: Antibiotic treatment in acute exacerbation of COPD: patient outcomes with amoxicillin vs. amoxicillin/clavulanic acid-data from 43,636 outpatients. (Pubmed Central) - Oct 10, 2021 Our results showed that most of E. coli were multidrug resistant and possessed multiple virulence genes, which may have a huge potential hazard with public health, and antibiotic resistance of E. coli was prevalent in dairy herds in Northern China, and ampicillin should be used cautiously for mastitis caused by E. coli in Northern China. In AECOPD, empirically adding clavulanic acid to amoxicillin is not associated with a better outcome; it seems safe for these patients to be treated with amoxicillin alone.

Despite a large decrease in the number of outpatient visits during the pandemic, rates of prescribing for URI decreased minimally.

Journal: A novel hemA mutation is responsible for a small-colony-variant phenotype in Escherichia coli. (Pubmed Central) - Oct 2, 2021 However, the phenotype quickly reverted during sequential sub-culturing in liquid growth media. As hemA is not found in mammalian cells, and disruption of the gene results in a significant fitness cost, it represents a potential target for novel drug development specifically for the treatment of catheter-associated urinary tract infections caused by E. coli.

Retrospective data, Journal: Evaluation of antimicrobial prescriptions in dogs with suspected bacterial urinary tract disease. (Pubmed Central) - Oct 2, 2021 Decreases in duration and increased use of recommended first-line antimicrobials were encouraging. Common drug choices and durations should still be targets for antimicrobial stewardship programs that aim to optimize antimicrobial use, concurrently maximizing patient benefits while minimizing antimicrobial use and use of higher tier antimicrobials.

Antibiotic use is necessary in animal injuries? () - Oct 1, 2021 - Abstract #EUSEM2021EUSEM_4793; There are several antibiotics that can be used for antibiotic prophylaxis, but the trend is generally to use amoxicillin-clavulanate as the first choice... This systematic review showed that the use of antibiotic prophylaxis against animal injuries reflected controversy, it should be a standardized process and there is a lack of research studies where there is scientific evidence of whether its use is really effective or there are other factors, such as the disinfection of the lesion itself that, after a protocol of action, would prevent the use of antibiotic prophylaxis being recommended and thus avoid excess health costs and resistance to different microorganisms.

- |||||||||| Evolution of empirical antibiotic therapy among septic patients in an emergency department () - Oct 1, 2021 - Abstract #EUSEM2021EUSEM_2969;
The initiation of early antibiotic therapy in the ED among septic patients has improved significantly from 2014 to 2018. There is a progressive decrease in the use of levofloxacin, in favor of the use of third generation cephalosporins and carbapenemics, the rest of antibiotic therapy that is used seems punctual in selected cases.

Can bedside pulmonary ultrasound replace other tests to diagnose pneumonia? () - Oct 1, 2021 - Abstract #EUSEM2021EUSEM_2725; Brief clinical history: A 25-year-old woman who came to the Emergency Room (ER) with dyspnea of minimum efforts, audible wheezing, aphonia and cough with greenish sputum and fever around 37.5-38.5 ºC with 7-days of evolution and with poor evolution even though he was taking amoxicillin+clavulanic acid...It was decided to carry out treatment with Levofloxacin due to suspicion of Pseudomonas or atypical germs, with clinical and imaging resolution in 10 days...It is also relatively cheap, widely available (and increasingly, and more portable), and has no side effects. This work highlights the use of bedside thoracic ultrasound for the differential diagnosis between pneumonia and atelectasis, thus reducing the number of CT scans or fiberoptic bronchoscopies.
