Find That Bleed! A Case of Hemobilia Due to Cystic Artery Pseudoaneurysm in a Patient With Persistent Melena (Exhibit Hall) - Jul 29, 2023 - Abstract #ACG2023ACG_2563; He was hospitalized one-week prior due to a similar episode at which point he was found to have a dieulafoy lesion in the transverse colon and CT angiography incidentally revealing cholecystitis, which was managed conservatively with amoxicillin/clavulanate for 7 days...Thus, it is important to have a high index of suspicion and include visceral artery aneurysms in the differential for patients presenting with GI bleeding. - |||||||||| A Perforated Appendix Masquerading as Crohn (Exhibit Hall) - Jul 29, 2023 - Abstract #ACG2023ACG_2160;
He was discharged on a course of amoxicillin-clavulanate, and the abscess drain was removed after two weeks...Unique to this case, symptoms were prolonged over a two-week period, appendix was normal on initial CT scan, and TI inflammation with fistulizing abscess formation. Although, complicated appendicitis rarely presents as terminal ileitis with fistulizing abscess formation, it should be included in the differential diagnosis of such a presentation, as prompt recognition would improve patient care, decrease further complications, and decrease patient stress of being misdiagnosed with CD.

- |||||||||| Beyond the Yellow Haze: Navigating Gilbert Syndrome and Hepatitis (Exhibit Hall) - Jul 29, 2023 - Abstract #ACG2023ACG_1579;
Amoxicillin-clavulanate is a common cause of drug-induced liver injury (DILI), along with other medications such as chlorpromazine, macrolides, tetracyclines, and antiepileptics...EBV can cause self-limiting hepatitis in immunocompetent individuals, while CMV is typically associated with hepatitis in immunocompromised individuals and rarely affects immunocompetent individuals. Although hepatitis in individuals with Gilbert syndrome may present with more significant hyperbilirubinemia, the overall course and prognosis of the condition are similar to those without Gilbert syndrome.
- |||||||||| A Unique Case of Spyglass Cholangioscopy-Associated Bacteremia (Exhibit Hall) - Jul 29, 2023 - Abstract #ACG2023ACG_1344;
She started on IV Vancomycin 1g daily and IV ampicillin-sulbactam 3g every six hours based on culture results and sensitivities...She was discharged on oral amoxicillin-clavulanic acid 875-25mg for ten days...Since bacteremia is a significant complication of this procedure, future studies can be performed to analyze outcomes of bacterial translocation in patients treated with antibiotics either before or after this procedure. Thus, our case demonstrates the importance of further research into approaches to mitigate the associated risk of bacteremia following spyglass cholangioscopy.
- |||||||||| Incidental Finding of an Appendiceal Abscess on Screening Colonoscopy (Exhibit Hall) - Jul 29, 2023 - Abstract #ACG2023ACG_707;
She denied tobacco or alcohol use, and her only medications were vitamin D and trazodone for sleep...The patient was started on a 10-day course of amoxicillin-clavulanate and referred to a surgeon...In addition to an abscess, the differential diagnosis of a mucinous appendiceal lesion includes simple mucoceles, serrated polyps, low-grade or high-grade appendiceal mucinous neoplasms, and mucinous adenocarcinomas; thus, surgical resection of any localized appendiceal mucinous lesion is recommended to rule out a potentially malignant lesion. Figure: A protruding appendiceal lesion with white pus noted on colonoscopy exam.

Journal: Bifidobacterium breve PRL2020: Antibiotic-Resistant Profile and Genomic Detection of Antibiotic Resistance Determinants. (Pubmed Central) - Jul 29, 2023 Recently, a strain of B. breve (PRL2020) has shown to be resistant to amoxicillin and amoxicillin-clavulanate (AC) by applying the microdilution protocol according EFSA guidelines...The results of our tests demonstrated that the strain PRL2020 is indeed endowed with amoxicillin- and AC-resistant properties and that it is also insensitive to ampicillin...The similarity among the AR determinants found was studied through nucleotide sequence alignment, and it was possible to verify not only the absence of genes explaining these features in the flanking regions but also the presence of genetic sequences (rpoB and erm(X)) putatively responsible for rifampicin and erythromycin resistance...Analysis of the flanking regions of these genes revealed possible mobile elements upstream and downstream only in the case of the erm(X) gene, but the features of the Insertion Sequences (IS) are described as not to cause horizontal transfer. Our findings on strain PRL2020 demonstrate that its AR profile is compatible with antibiotics when taken with the aim of reducing the risk of dysbiosis.

(Pubmed Central) - Jul 12, 2023 Furthermore, a significant decrease in the inhibition zone was detected for amoxicillin and amoxicillin/clavulanic acid (AMC)...Specifically, low-dose X-rays made Staphylococcus aureus resistant to marbofloxacin and increased its resistance to penicillin. Similarly, Salmonella Enteritidis became resistant to both marbofloxacin and enrofloxacin, and showed reduced sensitivity to amoxicillin and AMC.

- |||||||||| Journal: A national study confirms that Escherichia coli from Australian commercial layer hens remain susceptible to critically important antimicrobials. (Pubmed Central) - Jul 11, 2023
Resistance was observed for amoxicillin-clavulanate (9.1%), ampicillin (16.2%), ciprofloxacin (2.7%), florfenicol (2.4%), gentamicin (1.0%), streptomycin (4.7%), tetracycline (37.8%) and trimethoprim/sulfamethoxazole (9.5%)...Two MCR E. coli isolates with ciprofloxacin-resistance were found to be carrying known resistance genes including aadA1, dfrA1, strA, strB, sul1, sul2, tet(A), blaTEM-1B, qnrS1 and tet(A). Overall, this study found that E. coli from layer hens in Australia have low rates of AMR, likely due to strict control on antimicrobial usage achieved by the sum of regulation and voluntary measures.
- |||||||||| Preclinical, Journal: Effects of Repeated in-vitro Exposure to Saudi Honey on Bacterial Resistance to Antibiotics and Biofilm Formation. (Pubmed Central) - Jul 10, 2023
The data highlight the positive impact of prolonged in-vitro exposure to Saudi honey (Sumra and Sider) for wound-associated bacteria since they displayed a significant increase in their sensitivity profiles to the tested antibiotic and a reduction in their ability to form biofilm. The increased bacterial sensitivity to antibiotics and a limited tendency toward biofilm formation would suggest the great potential therapeutic use of this Saudi honey (Sumra and Sidr) to treat wound infections.

The genetic surroundings of the detected genes suggested their inclusion in mobile genetic elements that might be transferrable to other bacteria. These findings suggest that, despite showing high susceptibility to several antibiotics, F. alocis might obtain new antimicrobial resistance traits due to its acceptance of mobile genetic elements with antibiotic resistance genes in their genome.
