The greater increase in prescription cost with GXR was balanced by a greater increase in overall medical costs with AAPs, resulting in no overall difference in total health care cost between the two treatments.

His positive symptoms did not improve despite being treated with risperidone, olanzapine, and paliperidone...One week after the third injection, a psychiatric examination revealed a significant improvement in the positive symptoms, and his caregivers confirmed an increase in the social interaction of the patient. Although we cannot postulate on a single exact mechanism for the increased efficacy of clozapine and aripiprazole combination, we may suggest that, at least for a subgroup of patients with schizophrenia who respond clinically to clozapine at a suboptimal level, combination with aripiprazole may be an effective therapeutic strategy.

In contrast, the memory impairment was blocked by the atypical APDs clozapine and olanzapine in a dose-dependent manner while risperidone was effective only at the highest dose tested (1 mg/kg)...These results indicate that atypical but not typical APDs can ameliorate NMDAR-mediated memory impairments and support the view that atypical APDs such as clozapine can modulate glutamatergic memory dysfunctions through 5-HT receptor mechanisms. These findings suggest that atypical APDs may improve cognitive impairments related to glutamatergic dysfunction relevant for emotional memories in schizophrenia.

Although over time the standard treatment for this condition has been the typical antipsychotic pimozide, this option has been losing importance due to concerns about its safety profile, its prolongation of the QTc interval, and a high risk of extrapyramidal symptoms...The patient had depressive symptoms reactive to the vascular event, presenting severe extra-pyramidal effects with the introduction of amisulpride...Results : The patient maintained an exuberant delusional condition for about 1 year, with difficulty in therapeutic adherence, partly due to the occurrence of side effects with antipsychotics such as risperidone, cariprazine and paliperidone (namely excessive somnolence, agitation and hyperprolactinaemia), as well as the lack of insight...Additionally, as she gained some insight, the patient started antidepressant treatment with a daily dose of mirtazapine 15mg, as well as quetiapine 50mg every night, which helped stabilize mood and sleep cycles...However, this case represents a particular therapeutic challenge due to the occurrence of side effects with different antipsychotics that made adherence to therapy even more difficult. The introduction of a long-term injectable in these patients seems to be a good alternative, with aripiprazole presenting a relatively safe profile and with good results in blurring delusional ideas.

She has a history of psychiatric appointments lasting 6 years from 93 to 99 years old with the diagnose of dysthymia but the only medication that remained, after being discharge, was quetiapine 25mg in SOS...During the inpatient treatment she was medicated with minimal doses of risperidone (0,5mg) and sertraline (25mg), with rapid improvement of her clinical condition with remission of the persecutory delusions, regularization of sleep and mood improvement, eventually getting discharged willingly to go back to her house...Taking the previous statement in consideration, those cases must be correctly diagnosed and treated. Regarding the treatment it is advisable to strictly follow the recommendations to start low and go slow as this is a population with frequent comorbidities and especially vulnerable to side effects.

Introduction/Aim A multicentre, 8-week, single-arm, open-label, pragmatic trial explored the acceptance and performance of using the Digital Medicine System (DMS) with health care professionals (HCPs) and adult subjects with schizophrenia, schizoaffective disorder (SAD), or first episode psychosis (FEP) on an oral atypical antipsychotic (aripiprazole, olanzapine, quetiapine, or risperidone)...Using the DMS, subjects’ medication adherence reached 86.6% on average when subjects were having good patch coverage. The acceptance and performance of DMS is considered safe and there were no SAEs associated with its use.
