Results We describe a case of polyomavirus BK nephropathy in an 82 year-old woman with marginal cell non-Hodgkin’s lymphoma in previous years, who had nine courses of RCVP (rituximab/ cyclophosphamide/ vincristine/ prednisone) five year before presentation and a subsequent trial of six courses of FCR (fludarabine, cyclophosphamide, rituximab) two years later...Oral leflunomide was initiated with poor response, viral load remaining high at 1,03 x 107 a month after initiation of therapy...If left untreated, we believe that high BK viral loads can become clinically symptomatic and cause endothelial dysfunction resulting in renal TMA as we described here for the first time. More studies are needed to define the best therapeutic approach to BK virus nephropathy in non-transplant patients.

At week 48, SAA (p<0.0001) and IL-6 (p<0.001) remained significantly lower only in patients receiving GC and leflunomide. At weeks 24 and 48, CD11b (p=0.048) and CD62L (p=0.036) were significantly reduced in patients on combinatory therapy, compared to patients receiving only GC.ConclusionsNeutrophil CD62L and CD11b, together with inflammatory parameters (e.g. SAA, IL-6) could represent an informative panel of biomarkers for GCA activity, additionally lowered by leflunomide.

Results show that DNA-damage occurred via photoinduced oxidative mechanism. Also, the potency of the studied drugs was examined on calf-thymus real DNA samples using TbCl fluorescence with ketoprofen and leflunomide being the most photogenotoxic anti-inflammatory drugs.
