Aggressive treatment strategies to lower serum ammonia and bacterial source control may help prevent irreversible brain injury with poor neurologic outcome.

Diff antigen and toxin, and oral vancomycin was started...It would be worthwhile to avoid or reduce opioids in patients that carry risk factors of CDI and are admitted with sepsis. Alternatively, CDI testing should be considered in a patient who is taking opioids that shows signs of colitis without diarrhea.

    A 45-year-old woman who went to the emergency room due to persistent irritative cough, sometimes accompanied by mucus described as "reddish", despite treatment antibiotic with Amoxicillin and Azithromycin started 48 hours before...Treatment with Levofloxacin 500mg iv is started, cross-tests are requested to carry out a transfusion of 2 red blood cell concentrates and oxygen therapy is started with nasal glasses at 2 lpm...The majority of patients with respiratory symptoms and infiltrates on chest radiography will have an infectious pathology, but we must know other syndromes that can present with a similar clinical picture and are much more infrequent in diagnosis, as in the case presented to us. The knowledge of these entities and a quick approach in the management of these patients can greatly improve the associated prognosis.

    Brief clinical history: A 25-year-old woman who came to the Emergency Room (ER) with dyspnea of minimum efforts, audible wheezing, aphonia and cough with greenish sputum and fever around 37.5-38.5 ºC with 7-days of evolution and with poor evolution even though he was taking amoxicillin+clavulanic acid...It was decided to carry out treatment with Levofloxacin due to suspicion of Pseudomonas or atypical germs, with clinical and imaging resolution in 10 days...It is also relatively cheap, widely available (and increasingly, and more portable), and has no side effects. This work highlights the use of bedside thoracic ultrasound for the differential diagnosis between pneumonia and atelectasis, thus reducing the number of CT scans or fiberoptic bronchoscopies.

    All isolates contained genes encoding for aminoglycoside (aph(6)-Id, aph(3')-lIb, aph(3″)-Ib), beta-lactam (blaPAO), tetracycline (tet(G)), fosfomycin (fosA), chloramphenicol (catB7), sulphonamide (sul1), quaternary ammonium (qacEdelta1) and fluoroquinolone (crpP) resistance...These findings suggest that genomes of P. aeruginosa ocular clonal strains with >95% nucleotide identity isolated twenty years apart had changed over time with the acquisition of resistance genes. The pattern of gene mutations also varied with more insertions and deletions in their chromosomal genes which confer resistance to antibiotics.

    These two cases indicate that quinolone exposure can be a risk factor for vertebral artery dissection. Considering the possible mechanism, it is better to avoid the prescription of quinolones to patients who have insufficient connective tissues to avoid vertebral artery dissection.
