    Journal:  The relationship between nasal and conjunctival cultures of antimicrobial-resistant isolates of methicillin-resistant Staphylococcus aureus. (Pubmed Central) -  Apr 29, 2022
    The bacterial composition, the proportion of multi-drug resistant bacteria among the pathogens, and the high risk for amputation should be fully considered in the initial empirical medication, and broad-spectrum antibacterials are recommended. This study showed a significant relationship between conjunctival and nasal cultures of MRSA, MRCNS, and LVFX-R Corynebacterium spp., suggesting that nasal cavity culture is a potentially useful screening method for detecting resistant bacteria, especially MRSA, in the conjunctiva.

    Retrospective data, Journal:  Emergence of Hypervirulent ST11-K64 Klebsiella pneumoniae Poses a Serious Clinical Threat in Older Patients. (Pubmed Central) -  Apr 29, 2022
    In conclusion, higher MICs of the last line antibiotic agents (ceftazidime-avibactam, tigecycline) might be a critical consideration in the clinical management of older patients where the concentration of these toxic antibiotics matters because of underlying comorbidities. Caution regarding KPC-2-producing ST11-K64 CR-hvKP as being new significant "superbugs" is required as they are widespread, and infection control measures should be strengthened to curb further dissemination in nosocomial settings in China.

    Journal:  A single-cell nanocoating of probiotics for enhanced amelioration of antibiotic-associated diarrhea. (Pubmed Central) -  Apr 27, 2022
    Armored probiotics have shown the ability to colonize inside the gastrointestinal tracts of levofloxacin-treated rats, which significantly reduced antibiotic-associated diarrhea (AAD) resulting from the levofloxacin-treatment and improved some of the pre-inflammatory symptoms caused by AAD. This nanoarmor strategy represents a robust platform to enhance the potency of therapeutic bacteria in the gastrointestinal tracts of patients receiving antibiotics and to avoid the negative effects of antibiotics in the gastrointestinal tract.

    Journal:  Amphiphilic cyclic peptide [WKR]-Antibiotics combinations as broad-spectrum antimicrobial agents. (Pubmed Central) -  Apr 26, 2022
    Cyclic [WKR] was subjected to combination (physical mixture or covalent conjugation) with meropenem as a model antibiotic to study the impact of the combination on antimicrobial activity...Combinations studies of [WKR] with an additional 9 commercially available antibiotics showed significant enhancement in antibacterial activity for all tested combinations, especially with tetracycline, tobramycin, levofloxacin, clindamycin, daptomycin, polymyxin, kanamycin, and vancomycin...These data indicate that [WKR] improves the antibacterial activity, presumably by facilitating the internalization of antibiotics and their interaction with the intracellular targets. This study introduces a potential strategy for treating multidrug-resistant pathogens by combining [WKR] and a variety of classical antibiotics to improve the antibacterial effectiveness.

    HIGH EFFICACY OF 14-DAY VONOPRAZAN-BASED QUADRUPLE THERAPY FOR HELICOBACTER PYLORI ERADICATION IN AREAS WITH HIGH CLARITHROMYCIN RESISTANCE: A PROSPECTIVE RANDOMIZED STUDY (VQ-HP TRIAL) (Poster Hall - San Diego Convention Center) -  Apr 25, 2022 - Abstract #DDW2022DDW_6058;
    Methods This preliminary prospective randomized study was operated to compare H. pylori eradication between 7-day and 14-day vonoprazan-based quadruple therapy containing vonoprazan 20 mg twice daily, bismuth subsalicylate 1,024 mg twice daily, metronidazole 400 mg 3 times daily and tetracycline 500 mg 4 times daily...Antibiotic resistance was 37.5% for clarithromycin, 28.5% for metronidazole and 37.5% for levofloxacin...Conclusion 14-day vonoprazan-based quadruple therapy provide an excellent cure rate for H. pylori infection in areas with high clarithromycin resistance regardless of CYP3A4/5 genotype. This 14-day regimen could be use as alternative first line treatment for H. pylori eradication.Keywords: Helicobacter pylori, Vonoprazan, Quadruple therapy.
  • ||||||||||  THE SHORT-TERM AND LONG-TERM IMPACTS OF H. PYLORI ERADICATION ON THE ANTIBIOTIC RESISTANCE AND RESISTOME OF GUT MICROBIOTA- A MULTICENTER RANDOMIZED TRIAL (29CD - San Diego Convention Center) -  Apr 25, 2022 - Abstract #DDW2022DDW_5444;
    Methods : H. pylori infected patients who failed after one treatment were eligible in this open labeled, multicenter, randomized trial, and were randomized to receive (1) Levofloxacin sequential therapy (EAML): esomeprazole 40 mg and amoxicillin 1 g for the first 7 days, followed by esomeprazole 40 mg, metronidazole 500 mg, and levofloxacin 250 mg for another 7 days (all twice daily); or (2) Bismuth quadruple therapy (BQ): esomeprazole 40mg twice daily, bismuth tripotassium dicitrate 300 mg four times a day, tetracycline 500mg four times a day, and metronidazole 500mg three times a day, for 10 days...The resistance rates to levofloxacin, ciprofloxacin, ampicillin and various cephalosporins of E. coli were significantly increased in the EAML group than in the BQ group at week 2, which were restored to basal state at week 8 and 1 year (Figure 1)...Conclusion : Levofloxacin sequential therapy and bismuth quadruple therapy are similarly effective in the second-line treatment of H. pylori infection. The transient increase of antibiotic resistance/ resistome and perturbation of diversity of fecal microbiota can be restored to basal state months after treatment, supporting the safety of H. pylori eradication for gastric cancer prevention.

    SUCCESS OF HELICOBACTER PYLORI GUIDELINE-BASED TREATMENT OF NEWLY DIAGNOSED AND PREVIOUSLY TREATED PATIENTS (Poster Hall - San Diego Convention Center) -  Apr 25, 2022 - Abstract #DDW2022DDW_3154;
    Second and/or third line therapies are PBMT or PPI-amoxicillin-levofloxacin (PAL) and the fourth line PPI-amoxicillin-rifabutin (PAR) for 10 days...PAR performs well as rescue therapy and may be considered as an appropriate 3rd-line treatment. Antibiotic resistance in H. pylori is common to clarithromycin, metronidazole, and levofloxacin and frequently accounts for treatment failures.
  • ||||||||||  A RETROSPECTIVE 8 YEAR ANALYSIS OF HELICOBACTER PYLORI RESISTANCE PATTERNS IN A TERTIARY CENTRE – EVOLVING PARADIGMS IN SALVAGE THERAPY (ePoster - DDW Virtual) -  Apr 25, 2022 - Abstract #DDW2022DDW_1404;
    There was a decline in Amoxicillin and Clarithromycin resistance rates, from 9% and 63% respectively in the year 2013-2017 to 0% and 50% respectively in the year 2018-2021...There was no significant difference in resistance rates with gender. Conclusion Given the extremely low amoxicillin resistance rate of Helicobacter pylori locally, we suggest using high dose dual therapy (amoxicillin + PPI) as the next line salvage therapy following failure of at least 2 empirical eradication therapies, particularly in patients who either have an inconclusive culture result or declined gastroscopy for culture directed eradication therapy.

  • ||||||||||  Journal:  Molecular epidemiology of antimicrobial-resistant Pseudomonas aeruginosa in a veterinary teaching hospital environment. (Pubmed Central) -  Apr 23, 2022
    The MLST approach and single nucleotide polymorphism analysis revealed a link between resistant strains from ward rooms and those from examination, wound care, and operating rooms. The improvement of routine cleaning, especially of sink environments, and the continued monitoring of antimicrobial resistance of P. aeruginosa in veterinary hospitals are necessary to prevent the spread of resistant clones and ensure infection control.
