    Journal:  Uropathogenic Pan-Drug-Resistant Providencia rettgeri Infection in a Patient With an Indwelling Urinary Catheter: A Case Report. (Pubmed Central) -  Dec 11, 2024
    Despite the organism's resistance to broad-spectrum antibiotics, clinical improvement was observed with levofloxacin treatment, underlining the potential discrepancy between in vitro resistance patterns and in vivo response, particularly in urinary tract infections (UTIs) where urine drug concentrations are pivotal. This case underscores the necessity for heightened surveillance, stringent infection control measures, and judicious antibiotic use in managing infections caused by pan-drug-resistant pathogens like P. rettgeri, challenging the traditional paradigms of antimicrobial resistance and therapy in hospital settings.

    Treatment then followed with bendamustine and abdominal radiation which resulted in partial response, and subsequent epcoritimab without significant response...Following leukapheresis, she underwent bridging therapy with 1C of rituximab/polatuzumab throughout which her blood counts remained stable...For closer monitoring, she was hospitalized for lymphodepleting chemotherapy (LDc) with brentuximab and Tisa-cel reinfusion...She was briefly treated with broad spectrum antibiotics, sulfamethoxazole-trimethoprim and levofloxacin prophylaxis, 300mcg of filgrastim for 5 days, and 11 days of aminocaproic acid...Her course was also complicated by cytomegalovirus (CMV) gastritis and esophageal candidiasis treated with fluconazole and ganciclovir...CAR-T treatment modifications may also include stem cell boosts to encourage hematopoiesis for persistently profound cytopenias, and tailoring lines of chemotherapy as to avoid LDc-specific agents prior to CAR-T to maintain tumor-sensitivity. Our single-center experience at the CTFM supports the safety of CAR-T in bloodless medicine patients with DLBCL.

    Infectious Complications and Antibiotic Prophylaxis during Induction Therapy with Venetoclax Plus Azacitidine for Previously Untreated AML: A Real-World Experience () -  Dec 7, 2024 - Abstract #ASH2024ASH_8052;
    Outcomes were assessed only during the initial induction cycle, for a minimum 30 days, or up to day 60 if delayed neutrophil recovery occurred.Antibacterial prophylaxis was used in 81 patients (76 levofloxacin, 5 amoxicillin-clavulanate); 2 were already on IV antibiotics for a pre-existing infection, and 10 received no prophylaxis...Antifungal prophylaxis was used in 89 patients (70 posaconazole, 18 fluconazole, 1 voriconazole)...Age, secondary AML or ELN risk group also did not predict for bacteremia, FN, early death or readmission.Conclusion : The use of antibacterial prophylaxis during ven-aza induction is associated with very low rates of bacteremia. However, readmissions for other suspected or proven infections, including culture-negative febrile neutropenia and pneumonia, are still relatively frequent.
