    Relative infant doses were 0.35% to 0.53% and 0.09% to 0.18%, for prednisone and prednisolone, respectively. Prednisone and prednisolone exhibit dose- and concentration-dependent pharmacokinetics during pregnancy, and infant exposure to these agents via breast milk is minimal.

Early escalation of immunosuppression may be of benefit in the absence of a rapid response to corticosteroids.

    The patient was commenced on intravenous methylprednisolone for 3 days followed by a switch to oral prednisolone...Eventually, systemic azathioprine and infliximab were commenced with frequent review by ophthalmology and immunology...Ocular manifestations can occur in 70% of patients, comprising retinal vasculitis, anterior uveitis, iridocyclitis, chorioretinitis, scleritis, keratitis, vitreous haemorrhage, optic neuritis, conjunctivitis, retinal vein occlusion and retinal neovascularisation. A tailored multidisciplinary approach is required, with corticosteroids being the mainstay of treatment.

Both oral and IV suprapharmacological doses of corticosteroids are equally effective in the treatment of drug-induced AIN, if used early.

    Please see the attcahed graph image below Discussion Although there is concern about concomitant use of ritonavir and tacrolimus, this case suggests minimal interaction only 40 hours after the last dose, with appropriate tacrolimus concentrations and an exposure to tacrolimus (area under the curve of 91.2 ng.h/mL) comparable to patients receiving tacrolimus not on ritonavir. We conclude that if a patient on antiretroviral therapy is taken off a ritonavir-boosted protease inhibitor regimen when starting or taking tacrolimus therapy, the standard dosing should be considered at the time of the change along with close therapeutic drug monitoring.

    Recently, we showed that exposure of zebrafish to the glucocorticoid prednisolone during fin regeneration impacts negatively on the length, bone formation, and osteoblast function of the regenerate...In particular, acidification of membrane-enclosed organelles such as lysosomes is inhibited. Taken together, our data indicate that continued synthetic glucocorticoid exposure in zebrafish deteriorates cellular trafficking processes in the regenerating fin, which interferes with appropriate tissue restoration upon injury.
