We are presenting a rare case of Cladophialophora pneumonia in a post renal transplant patient.Case: A 66-year-old female with a significant past medical history of diseased donor renal transplant (DDRT) in 2008 due to lupus nephritis and stage IV chronic kidney disease on immunosuppressants including Tacrolimus and Prednisolone...Initial CT scan head showed temporoparietal lesion with surrounding edema and started on empiric coverage with Vancomycin, Cefepime, Flagyl and steroids...She was started on IV Voriconazole and steroids were tapered off...At that time IV Liposomal amphotericin B was added...Surgical resection with prolong antifungal treatment appears to be the only effective treatment, prognosis is poor as have 70 % mortality. Biopsy of any suspected pulmonary lesions in immunocompromised patients is the key for early diagnosis.

Chemotherapy with R-EPOCH (Rituximab, Etoposide, Prednisolone, Oncovin, Cyclophosphamide, Hydroxydaunorubicin) has produced satisfactory improvement in the patient’s symptoms.DiscussionThe existence of an association between sarcoidosis and lymphoma has been previously examined, with one study finding a 11-fold increase in the incidence of lymphoma in patients with sarcoidosis.1-3 The pathophysiology is uncertain, but has been hypothesized to involve a global amplification of a T-cell subset and increased interleukin-2 production, which in turn acts as B-cell growth factor, causing a predisposition to malignant transformation.4 IVBCL is an uncommon subset of Diffuse Large B-Cell Lymphomas (DLBCL), involving B-cell proliferation in small vessel lumina. Our literature review yielded one case of sarcoidosis associated with IVBCL4; our case uniquely demonstrates the occurrence of sarcoidosis-lymphoma syndrome with cerebral IVBCL.ConclusionClinicians should be aware about association of sarcoidosis and lymphoma, and remain vigilant, given the symptomatology overlap and life-threatening nature of the disease.

This study provided pilot data for future randomized controlled trials to evaluate CMV preventive strategies in selected high-risk patients with glomerulonephritis or renal vasculitis.

However, the sample size was small, and future larger studies or meta-analyses are required to demonstrate conclusively if innate immune genetics such as IL6 influence the risk of acute rejection after kidney transplantation.

Recent evidence suggests that it is not always necessary to use high-dose steroids (>0.5 mg/kg/d) to attain remission; and side effects may also be minimised through more targeted therapy with the less systemically-absorbed corticosteroid, budesonide. The authors offer advice on the stratification of treatment for these patients and suggest changes to improve the services available for people with autoimmune hepatitis in the UK.
