  • ||||||||||  Journal:  Presumed Choroidopathy of IgG4-Related Disease Discovered During 16-Year Follow-Up of a Patient With Polycystic Kidney Disease. (Pubmed Central) -  Nov 4, 2024   
    At the age of 52 years, she switched to candesartan 8 mg daily and began to have tolvaptan (a selective competitive vasopressin receptor 2 (V2) antagonist) 90 mg daily for polycystic kidney disease with liver cysts...In the following four years to the latest visit at 61 years, she kept stable but high levels of serum IgG4 around 300 mg/dL. Serum IgG4 measurement is helpful to make a clinical diagnosis and, hence, a clinical decision since the spectrum of IgG4-related disease remains obscure.

Inflammatory aortitis should be considered as a possibility during the assessment of inflammatory back pain to mitigate the risks of delayed diagnosis.

    Risk factors include high cumulative dose (100mg/m2), weekly regimen, co-administration of gemcitabine or radiation and pre-existing lung disease...Given the clinical presentation, negative infective work-up and CT findings, a diagnosis of docetaxel-induced pneumonitis was made and started on oral prednisolone 1mg/kg/day...CONCLUSION Docetaxel-induced pneumonitis, while rare, can lead to life-threatening event. This case underscores the importance of maintaining high index of suspicion among patients who present with respiratory symptoms after administration of docetaxel as prompt recognition and timely initiation of corticosteroid treatment lead to favourable outcome.

    Before referral, the cat was treated with immunosuppressive doses of oral prednisolone, intermittently for 6 mo, and had responded well when the immunosuppressive dose was maintained...Key clinical message: In this case, an adult cat had a chronic, progressive history of lower motor neuron weakness and absent spinal reflexes; biopsies revealed a neurogenic pattern of muscle fiber atrophy and histologically normal peripheral nerve and intramuscular nerve branches. Although reports of motor neuron disease are rare in the veterinary literature, this case report highlights the importance of muscle and nerve biopsies that lead to a presumptive diagnosis of motor neuron degeneration.

To our knowledge, this is the first case of nemolizumab-exacerbated BP.

Further studies are needed to establish long-term efficacy and safety of this treatment modality.

Given the many inherent risks and complications associated with immunosuppression in this patient group, targeted therapies such as dupilumab and omalizumab may provide a safe and efficacious option, either as single agents or in severe refractory cases such as this one, as combination therapy.
