    An ESPB (compared to opioids, sham treatment, other regional anaesthetic techniques) probably does not reduce postoperative pain intensity at rest 24 hours after surgery, but may not have an effect on block-related adverse events. In contrast, our results showed a robust reduction in morphine consumption and opioid related adverse events (e.g. PONV and pruritus).

    Risk factors of the rebound pain trajectory included age ≤ 65 (odds ratio, OR: 1.89; 95% confidence interval, CI: 1.12 – 3.20), female (OR: 2.28; 95% CI: 1.24 – 4.19), and NRS ≥ 4 on the postoperative day 0 (OR: 3.44; 95% CI: 1.65 – 7.15).Patients in the group 2 also tended to consume more IVPCA morphine (p < 0.001) and had longer LOS (p < 0.001) than those in the group 1, controlling for the effects of other selected predictors of IVPCA consumption and LOS.Conclusion(s): There were notable proportions of patients with a rebound pain trajectory after the discontinuation of IVPCA for pain control following spine surgery. Older age, female and moderate to severe pain noted after surgery were risk factors and more aggressive pain management strategies like early initiation of multimodal analgesia should be considered in these high-risk patients to improve pain control quality and reduce the incidence of rebound pain.

    Anesthesia induction was by using target-controlled infusion (TCI) of Remifentanil at blood concentration 4ng/ml, TCI of Propofol at blood concentration 6-7μg/ml, rocuronium 1mg/kg and the trachea will be intubated after 90sec of manual ventilation with 100% O2...For postoperative analgesia, we were administered intraoperatively 2gr paracetamol as well as 0,1-0,2mg/kg morphine...Tramadol was given in 5 patients in PACU for VAS scores of 5 and one patient needed PCA of morphine to manage the pain. No nausea, vomiting or itching were recorded.Conclusion(s): Continuous wound infusion of ropivacaine for the control of pain after spinal surgery provided high levels of postoperative analgesia and reduced the requirements of opioids without totally excluding them.

    For postoperative analgesia, both groups would be prescribed tramadol and morphine if necessary... Based on this study, we found that PCA-PIB group had lower NRS score and consumed fewer postoperative opioid in 72 hours. In conclusion, patient control ESP block was proved to be a better way for pain relief of adult cardiac surgery.

Bilateral ESPB as a component of general anaesthesia in spinal surgery reduces the quantity of narcotic analgesics used during surgery and in the postoperative period and depresses the stress response to surgical trauma, hyperalgesia, and chronisation of acute pain.

Ultrasound guided postsurgical intramuscular QLB+TFPB using 10 ml 0.25% bupivacaine produces more postoperative analgesic effect and less postoperative opioid consumption when compared to intramuscular TFPB in patients underwent laparotomy under general anesthesia.

    There was no statistically significant difference between the three groups in terms of the morphine doses taken during the 24 hours postoperatively (p>0.05)...When comparing the fascial area blocks within themselves, it was observed that there was no significant difference between the VAS scores of the ESPB and SAPB groups. However, the ESPB showed a superiority over the SAPB in terms of consumption of additional analgesics and development of postoperative atelectasis.

This data is important in tailoring an anesthetic plan and in the process of informed consent.

    The animals were assessed for morphine dependence by monitoring naloxone hydrochloride precipitated withdrawal somatic signs.Results showed that animals receiving TAT-Gap19 before morphine injection demonstrated a significant reduction in several signs of morphine withdrawal Our findings suggest that hippocampal Cx43 may be involved in morphine-induced behavioral responses. Therefore, gliotransmitter release by astrocytes seems to be a mechanism which is engaged in addictive-like behaviors.

    Thus, exposure to morphine resulted in a long-lasting shift from hippocampal toward striatal dominance during learning, an effect that may be associated with lasting alterations in hippocampal astrocytes. Cocaine produced changes in the same direction, suggesting that use of a higher dose or longer duration of exposure might produce effects comparable to those seen with morphine.

    Our current results showed that morphine (5 μg) microinjected into the LS produced a stable feeding response...The present paper contributes by suggesting that LS GABA and μ-opioid receptors elicit eating by inhibiting LS neurons that themselves inhibit eating. Importantly, this work informs lateral septal research which may shed light on disordered eating included binge eating and anorexia.

    Collectively, these data support the therapeutic potential of KOR antagonists to support abstinence in OUD and ameliorate opioid withdrawal. Nasal heroin-assisted treatment can be a feasible therapeutic option for individuals with severe opioid use disorder who crave the fast onset of effect of diacetylmorphine but are unable to inject intravenously.

    Patient medical records for the year were analyzed by provider type (physician, NP, or PA) for differences in providers' rates of prescribing opioid and non-opioid medications, as well as characteristics of the opioid prescriptions (e.g., high daily morphine milligram equivalent [90 MME/day] dose, long-term opioid therapy [90 days])...In a national sample of veterans with chronic pain, NPs and PAs did not have higher odds of opioid prescribing. Concerns of NP or PA opioid overprescribing may be addressed by considering evidence that patients of these providers are not at higher odds of receiving an opioid prescription.

    While opioid prescribing habits after surgery seem highly varied and arbitrary, we have identified key predictors that highlight biases in surgeon opioid prescribing patterns. Surgeons tend to prescribe significantly larger amounts of opioids to younger, male patients and those of certain ethnic backgrounds, and surgeons with fewer years in practice are more likely to prescribe more opioids.

This study suggests that policies limiting access to specific prescription opioids led to overall reductions in publicly funded prescription opioid use, particularly in chronic oxycodone recipients, without immediate evidence of changes in opioid-related ED visits.
