    Preclinical, Journal:  Transcriptomic Analysis of Long Non-coding RNA-MicroRNA-mRNA Interactions in the Nucleus Accumbens Related to Morphine Addiction in Mice. (Pubmed Central) -  Jun 22, 2022   
    The correlation between the relative expression of lncRNAs, miRNAs and mRNAs was analyzed to further validate predicted ceRNA networks, and the Lnc15qD3-miR-139-3p-Lrp2 ceRNA regulatory interaction was determined. These results suggest that the comprehensive network represents a new insight into the lncRNA-mediated ceRNA regulatory mechanisms underlying morphine addiction and provide new potential diagnostic and prognostic biomarkers for morphine addiction.

    These effects may involve targeting of genes important for the synaptic transmission, such as Grin1 (NMDAR subunit 1). Our findings demonstrate an important role for NAc novel-m009C in regulating METH reward, reveal a novel molecular regulator of the actions of METH on brain reward circuitries and provide a new strategy for treating METH addiction based on the modulation of small non-coding RNAs.

    Furthermore, combined treatment of inhibition of the BLA to vHip circuit and KOR antagonists mitigated anxiety-like behaviors and prevented stress-induced CPP reinstatement after morphine withdrawal. These results revealed a previously unknown circuit associated with the emotional component of opioid withdrawal and indicated that restoration of synaptic deficits with KOR antagonists might be effective in the treatment of anxiety associated with morphine withdrawal.

    This study offers confirmation that a significant group of patients are prescribed long-term opioid medication for chronic pain at doses which are unlikely to be effective in reducing pain but are likely to have harmful consequences. The findings offer a simple, reliable and practical method of data extraction to identify these patients individually from routinely collected prescribing data, which will help in monitoring and treating individuals and establishing the problem prevalence.

    Journal:  Long-term Risk of Overdose or Mental Health Crisis After Opioid Dose Tapering. (Pubmed Central) -  Jun 21, 2022   
    Participants were US adults who underwent opioid dose tapering from 2008 to 2017 after a 12-month baseline period of stable daily dosing of 50 morphine milligram equivalents or higher and who had at least 1 month of long-term follow-up during a postinduction period beginning 12 months after taper initiation...In conditional negative binomial regression analyses, adjusted incidence rate ratios for the postinduction period compared with the pretaper period were 1.57 (95% CI, 1.42-1.74) for overdose or withdrawal and 1.52 (95% CI, 1.35-1.71) for mental health crisis. These findings suggest that opioid tapering was associated with increased rates of overdose, withdrawal, and mental health crisis extending up to 2 years after taper initiation.

    For any obese patient, the implementation of difficult intubation protocols and the use of protective ventilation (low tidal volume 6-8mL/kg of ideal body weight, moderate positive end-expiratory pressure of 10cmH20, recruitment manoeuvres in absence of contra-indications), with morphine sparing and semi-seated positioning as much as possible are recommended, associated with a close postoperative monitoring...The occurrence of intraoperative awareness is indeed more frequent in the obese patient than in the non-obese patient. Appropriate prophylaxis against venous thromboembolism and early mobilisation are recommended, if possible included in an early rehabilitation protocol, to further reduce postoperative complications.

    Milligram morphine equivalents (MME) per patient-ICU-day provided by eleven surgical intensivists were analyzed...Despite no significant differences in age, sex, or Elixhauser Comorbidity Index, there was a 45% difference between the high- and low-prescribing physicians in the LVG (253.7 vs 175.4 MME/pt-ICU-day; P = .008). This difference was not observed for patients in the SVG (74.3 vs 93.1 MME/pt-ICU-day; P = .141) nor based on intensivist specialty (LVG: 217.9 vs 209.5 MME/pt-ICU-day; P = .8) (SVG: 79.0 vs 93.3 MME/pt-ICU-day; P = .288).

    Our conclusions are consistent with other reviews and suggest that, while the reliability of the evidence base is low, given the absence of important differences within this analysis, it seems unlikely that larger head-to-head studies of oxycodone versus morphine are justified, although well-designed trials comparing oxycodone to other strong analgesics may well be useful. For clinical purposes, oxycodone or morphine can be used as first-line oral opioids for relief of cancer pain in adults.

    Retrospective data, Journal:  Prospective Implementation of Standardized Post-Hepatectomy Care Pathways to Reduce Opioid Prescription Volume after Inpatient Surgery. (Pubmed Central) -  Jun 18, 2022   
    Implementation of post-hepatectomy care pathways was associated with a 50% reduction in last 24-hour OME, which, combined with a standardized discharge calculation, was associated with an overall 75% reduction in discharge opioid volumes and tripled opioid-free discharges. These data suggest that no-cost, reproducible pathways can be considered in abdominal operations with similar incisions/length of stay to decrease variation in opioid dosing while prioritizing patient-centric opioid needs.

    The DNOL value at the begin of the surgery was compared by linear correlation test to the remifentanil and postoperative morphine consumption (indexed). There was neither a significant correlation between the D NOL and remifentanil consumption (Pearson r :0.2628, p : 0.4093, r2:0.06904), nor between the DNOL and 24 hour morphine consumption (Pearson r: 0.03710, r2:0,001377, p : 0.989).Conclusion(s): Despite the fact that the NOL seems to be useful for optimizing opioid consumption during general anesthesia, its initial increase after surgical incision is not predictive for individual analgesic titration during and after general anesthesia, at least not in this particular population.

There was no difference in postoperative morphine consumption at the PACU (ropivacaine 8.9 mg ± 6.0 vs placebo 9.7 mg ± 7.1, p=.62) and on the ward (ropivacaine 9.6 mg ± 9.4 vs placebo 11.6 mg ± 8.5, p=.39).Conclusion(s): The interpectoral-pectoserratus plane block is not effective for postoperative analgesia in patients with neurogenic thoracic outlet syndrome undergoing trans-axillary thoracic outlet decompression surgery.

    All groups received ketorolac (iv, 90mg/day) for 48 hours, metamizol (iv/po, 4g/day) and fentanyl (iv) or oxycodone (po) on request.Primary outcome: Change in Quality of Recovery 15 (QoR-15) score on day 1 compared to baseline.Secondary outcomes: QoR-15 at hospital discharge, PONV, pain scores, return of gastrointestinal function, morphine equivalents.Results and From 06/2018 to 12/2020, we included 133 patients (SSS, 40; TAP, 45; SA, 48)...There were no significant differences in the other secondary outcomes.Conclusion(s): Differences in QoR-Scores on postoperative day 1 compared to preoperative did not differ between the groups. Only in the subgroup undergoing open prostatectomy, QoR-scores at hospital discharge where lower in the group receiving systemic analgesia compared to additional spinal anaesthesia..

    Postoperatively, paracetamol was charted for 99% of patients, although regularly charted in only 74%...Only one child was charted PRN tramadol...Oxycodone use per hour of admission was highest for those patients who received intraoperative fentanyl without a NSAID (0.007mg/kg/hr) and lowest in those patients who received intraoperative long-acting opioid (morphine or oxycodone) +/- fentanyl plus a NSAID (0.004mg/kg/hr).Very poor documentation of pain scores was noted at 6, 12 and 24 hours...Optimising our intraoperative analgesia can promote effective early postoperative pain management. The lack of pain score documentation suggests that further education in this area is essential.Conclusion(s): It may be appropriate to implement a postoperative ‘bundle of care’ in our institution, to more uniformly care for our paediatric adeno-tonsillectomy patients.

Patients in group C experienced statistically less sedation (p<0.05) and they had significantly lower incidence of nausea/vomiting compared to group M (p<0.05).Conclusion(s): According to our findings, intraoperative intravenous infusion of vitamin C contributed to a better control of early postoperative pain by decreasing pain scores, reducing morphine consumption and mitigating the incidence of adverse events compared to magnesium sulfate after elective major abdominal surgery.

    The patient was admitted to the intensive care unit due to uncontrolled pain and was started on transdermal fentanyl (25mcg/hour), dexamethasone (8mg/day), and morphine, 3mg as needed...With the patient’s consent, an echo-guided ESP block was performed at T9 level, with 20mL 0,5% ropivacaine...Hepatic or renal dysfunction limit analgesic options, due to the altered pharmacokinetics. In these cases, regional analgesia plays an important role and should be considered in palliative care patients.

If the patient was randomized in Group RL or RO,Ropivacaine 0.375% (20 ml) was infused at each side 30 minutes before induction of GA and 0.2% (20 ml) 12, 24, 36 and 48 hours after surgery...Regarding OC, total intraoperative remifentanil, postoperative tramadol and morphine administration and mobilization time of the patient was significantly lower in Group RO when compared to CO (p=0.008, p=0.011, p=0.025, p=0.03 respectively)... In this study, we confirmed that ESPB is an effective, simple and safe method which contributes to the reduction of perioperative opioid administration, improvement of quality of perioperative analgesia, and the achievement of enhanced recovery to patients undergoing elective colectomy.
