    Journal, Surgery:  Decreased Opioid Use and Equivalent Pain Score Outcomes After Robotic Urologic Surgery Using a Simplified Opioid Minimization Protocol: A Safety-Net Hospital Experience. (Pubmed Central) -  Nov 17, 2023   
    Total opioids received within 30 days of surgery decreased by 68% in the OM vs OC cohort (median [IQR] 32.5 [7.5-65] vs 100 [30-173] Morphine Milligram Equivalents (MME), P < .001)...Pain severity did not differ between cohorts on POD2 (median [IQR]: OM=3/10 [2-5], OC=3.5/10 [2-6]; P = .5) or POD7 (median [IQR]: OM=2/10 [0-3], OC=1/10 [0-3]; P = .8), and POD7 satisfaction with pain management remained high for both cohorts (P = .8). Our simplified opioid minimization protocol decreased total opioid use after robotic urologic surgery by 68% without compromising pain or satisfaction.

    Due to chronic fractures, patient took Tylenol #3 every 6 hours as needed (prn) and ibuprofen 600mg every 8 hours prn at home...Postoperatively, he was started on methocarbamol 500mg every 6 hours, acetaminophen 500mg every 6 hours prn, morphine 4mg every 4 hours prn, and Norco 5-325 every 4 hours prn which was later increased to Norco 7.5-325 to manage acute on chronic pain... Individualized approach to pain management is needed in patients with congenital osteodystrophy who suffer from acute on chronic fractures needing surgical and non-surgical intervention.

    The efficacy of topical ketamine was assessed by averaging the self-reported pain ratings (scale from 0-10) documented by nursing staff every 3-6 hours as well as measured morphine milliequivalents (MME) of opioids taken on post-operative days one, two, three, and the day of discharge.Setting: Inpatient hospitalAssessment/ Topical ketamine administration allowed for dose reduction of multimodal opioid regimens for all three patients (Case 1 exhibited an 87.3% reduction in MME; Case 2 exhibited a 100% reduction in MME; Case 3 exhibited a 72.4% reduction in MME). The findings demonstrated in this case series may warrant prospective research for the use of topical ketamine for patients experiencing post-amputation phantom limb pain.

    This study aimed to determine the association between non-opioid analgesic use and total opioid load, as measured using morphine milligram equivalents (MMEs), during inpatient rehabilitation for traumatic brain injury...Non-opioid medications that were reviewed in the study included acetaminophen, amitriptyline, baclofen, diclofenac, gabapentin, ibuprofen, lidocaine, methocarbamol, nortriptyline, and pregabalin...Results also showed that the mere presence of methocarbamol (regardless of daily or total dosage) had a significant effect on the ?MME. Conclusions Based on these findings, physicians may want to consider prescribing acetaminophen, gabapentin, or methocarbamol for patients admitted for inpatient rehabilitation following traumatic brain injury who require high amounts of opioids.

    These results indicate that the pivotal interactions at the dimerization interface between MD2 and TLR4* in M6G-bound (TLR4/MD2) were considerably weaker than those in M3G-bound (TLR4/MD2), which partially explains why M6G fails to activate TLR4 signaling. The discoveries from this study will offer valuable insights for the advancement of next-generation TLR4 small molecule modulators based on opioids.

Assay characteristics included modest sample preparation, rapid chromatography, high specificity and small sample volume with a processing time of 12 h. The assay described here can be applied for diagnostic laboratories and in forensic settings as well.
