    We found that elevation of C1q is not maintained in adulthood, following chronic morphine exposure (PND 90-595), but we see a significant downregulation of expression in cortex following an exposure paradigm of escalating doses of morphine (20-100 mg/kg for 5 days; which induces dependence as evidenced by withdrawal symptoms in adults.     Hospital length of stay decreased from 8.92 to 5.72 days, ICU days decreased from 1.52 to 0.42 days, and narcotics for the first five hospital days were significantly decreased from 130.13 to 71.2 morphine milligram equivalents...Likewise, there is limited data regarding opioid-sparing anesthesia techniques. Our protocol produced promising hospital length of stay and reduced opioid administration during the first five hospital days without increasing 30-day readmission rates.

    Opioid user types were described by episode number, median total day supply, median total MME (morphine milligram equivalents) prescribed, first prescribing specialist and first opioid prescribed...First opioid prescribed across all user types was codeine (50%-70%), tramadol/tapentadol (19%), and hydromorphone (5%-15%)...Factors associated with longer episodes included initial prescription of oxycodone (HR 0.82) or hydromorphone (HR 0.85) relative to codeine; first prescriber speciality: psychiatry (HR 0.79) or diagnostic services (0.89) relative to general practice; and increasing age (HR 0.97 by 5-year increments). Conclusions This population-level datal provides a better understanding of the role of prescriber types/prescriptions and factors that contribute to increased likelihood of long-term use.

    We found that restrictive post-operative opioid prescribing was not associated with differences in longitudinal symptom burden among women undergoing minimally invasive gynecologic surgery. These results provide the most comprehensive picture to date of post-operative symptom recovery under different opioid prescribing approaches, lending additional support for existing recommendations to reduce opioid prescribing following gynecologic surgery.

There were significant psychosocial issues affecting patients, families and staff in the context of the pandemic.What are the implications for practitioners?This study highlights the need for well-functioning working relationships between the palliative care consultancy service and other hospital teams that can be leveraged at a time of crisis, such as a pandemic, to provide optimal palliative care to patients.

    The trials published to date show overall that tapentadol does not provide a clinically significant analgesic improvement compared to existing treatments, for which the safety profile is much better known.     We demonstrate that both allyl and alkyne arylazido derivatives of IBNtxA efficiently radio-photolabeled mMOR-1 in cell lines and MOR-1 protein complexes expressed either exogenously or endogenously, as well as found in mouse brain. In future, design and application of such radio-photolabeling ligands with a conjugated handle will provide useful tools for further isolating or purifying MOR-1 to investigate site specific ligand-protein contacts and its signaling complexes.

    This work demonstrates the antinociceptive, anxiolytic and antidepressant effects of 5-fluoro-2-oxindole, suggests a new strategy to enhance the antinociceptive actions of morphine and proposes a new mechanism of action of oxindoles during chronic neuropathic pain. Patients undergoing isolated midurethral sling placement filled a median of 150 cumulative MMEs, and the proportion of patients filling perioperative opioid prescriptions increased over the study period.

Therefore, naloxone facilitates contextual learning and memory in a receptor-independent and Tet1-dependent manner, which provides new understanding on the receptor-independent functions of opioids.

    Episode duration/dosage criteria was used in 90 studies, with 7.5% by Morphine Milligram Equivalents + days' supply and 32.2% by other "episode" combination definitions. COT definitions were applied in musculoskeletal CNCP (60.8%) most often, and typically in adults aged 18 to 64 (69.6%)...Perspective: This study summarized the most common, current definition criteria for chronic opioid therapy (COT) and recommends adoption of consistent definition criteria to be utilized in practice and research. The most recent literature trends toward shorter duration criteria overall, suggesting that COT definition criteria are increasingly stringent.
