    Retrospective data, Journal:  Budesonide versus systemic corticosteroids in IgA Nephropathy: A retrospective, propensity-matched comparison. (Pubmed Central) -  Jul 8, 2020   
    The median reduction in hematuria at 24-month was 72% (IQR: -90%; -45%) in the budesonide group and 73% (IQR: -85%; 18%) in the corticosteroid group, respectively (P = .22, for between group difference). Treatment with budesonide was well tolerated with minimal side effects.Budesonide (Budenofalk) was effective in the treatment of patients with IgAN at high-risk of progression in terms of reducing proteinuria, hematuria and preserving renal function over 24 months of therapy.

    [VIRTUAL] Successful Management of Pediatric Plastic Bronchitis in an African Context (ATS 2020 Virtual) -  Jul 6, 2020 - Abstract #ATSI2020ATS-I_7088;    
    CASE REPORT A seven-year old known to have SCD - on daily hydroxyurea, folate and penicillin V - presented with a two-day history of cough, fever and difficulty in breathing...He was admitted to the intensive care unit, commenced on intravenous high-dose ceftriaxone, amikacin, and maintenance fluids...He completed 10 days of meropenem-vancomycin...He was discharged home to complete his azithromycin/fluconazole course, continue incentive spirometry, daily inhaled budesonide, and salbutamol when necessary...DISCUSSION A high index of clinical suspicion made bronchoscopy necessary leading to an optimal clinical outcome for our patient. We report this case to alert health workers in our context that plastic bronchitis in ACS occurs and can have a good outcome if managed correctly.

    In 54 patients (average 39.6±1.1 years old) with mild persistent asthma and osmotic airway hyperresponsiveness, the lung function and airway response to DPE (∆FEV 1 ) were assessed initially and after 24 weeks of controller treatment with combination ICS/LABA (budesonide/formoterol 160/4,5 mcg/dose on 2 inhalations per day)...In patients with a negative response to DPE, FEV 1 was 96.6±2.84% (p=0.48); ∆FEV 1 was -4.4±1.4% (p=0.049), 6 patients had exercise-induced bronchoconstriction. Conclusion : In asthma patients with osmotic airway hyperresponsiveness and exercise-induced bronchoconstriction, controller antiinflammatory therapy does not effectively normalize airway responsiveness to dosed physical exercise.

    The combination of in vitro and in vivo data indicates that only surfactant + budesonide is capable of both, reaching a remote area and downregulating inflammation. It is concluded that exogenous surfactant fortified with budesonide may be beneficial as an anti-inflammatory therapy in diseases with chronic and/or overwhelming inflammation such as ARDS and bacterial pneumonia.

It can be logistically challenging to arrange for anesthetic gas administration in the ICU.

In conclusion, pediatric patients who fail to respond to conventional treatment for recurrent wheezing warrant further evaluation for other etiologies.

    In the present work, we link real-world emulative in vitro measurements of inhaler performance with numerical models of lung deposition and pharmacokinetics for budesonide (BUD) delivered from three marketed dry powder inhalers (DPIs) to illustrate how such a process can inform device/formulation design...Conclusions Coupling emulative in vitro experimentation with the modeling of regional lung deposition, ASL, and drug disposition allows for predictions of systemic drug exposure. The present method may prove useful in drug development.

    Findings illustrated the importance of performing diagnostic testing, including routine fecal examination, to rule out infectious causes of diarrhea before beginning empirical treatment with glucocorticoids such as budesonide. Further, repeated fecal examinations, including Baermann tests, should be considered if a positive response to glucocorticoids is not observed.

    The limited available evidence suggests that budesonide/formoterol reliever therapy has greater potency and efficacy than budesonide/formoterol fixed dose maintenance plus SABA reliever therapy in reducing the risk of a severe exacerbation. This is an important concept which has the potential to guide clinical practice in asthma, although the small number of studies available highlights the need for further research to better define these pharmacological properties.
