    We report a case of a hospitalized patient with unregulated fentanyl use who underwent a successful transdermal buprenorphine induction for 48 hours to initiate BUP-XR with minimal levels of withdrawal and without precipitating opioid withdrawal. The approach described could provide a practical, patient-centered, accelerated induction strategy that, once independently validated, could considerably facilitate the use of BUP-XR.

These measures are also discussed in reference to other, comparable types of public health-oriented emergency health or survival care standards, while considering the extreme contexts of an ongoing, acute drug-death-crisis in Canada.

    Important studies herein determined the optimum ratio of the template/functional monomer, with subsequent evaluations of selectivity of the MIP for the template and fentanyl versus the commonly encountered narcotics such as methamphetamine, cocaine, and heroin.     This study showed a large percentage of prescriptions made in the off-label (50.7%) and unlicensed (37.2%) form in the Neonatal Intensive Care Unit, corroborating the worrying world scenario. The most exposed neonates were precisely the most vulnerable ones and, among the most commonly prescribed medications, ampicillin and gentamicin stood out in off-label form and caffeine in unlicensed form.

    Although it was previously thought of as a benign self-limiting condition, it is now known to be associated with significant morbidity and mortality.Case: A 26-year-old male patient with past medical history of Fentanyl abuse now on methadone, presented with features of opioid withdrawal...Due to excessive agitation and vomiting, he was given IV haloperidol and ondansetron...He was diagnosed with takotsubo cardiomyopathy and was initiated on dapagliflozin, metoprolol succinate and losartan...Approximately 4.5% of these patients can die during their hospitalization and 4% can die within 30 days of discharge. So, clinicians must keep a high index of suspicion for this syndrome in patients with history of substance use disorder, especially in the setting of troponin elevation.
